Realization that hematopoietic stem cells give rise to the immune compartment has resulted in clinical trials of hematopoietic stem cell transplantation for patients with autoimmune diseases. Allogeneic hematopoietic transplants are a form of adoptive immunotherapy resulting in beneficial graft versus tumor effects. Large numbers of hematopoietic cells can be collected with ease. Therefore, a renewable source of cells for ex vivo genetic manipulations is readily available. Multiple trials combining hematopoietic transplants and gene therapy are in progress. One such application is the infusion of allogeneic lymphocytes containing a suicide gene to abort graft versus host disease. Hematopoietic stem cell transplantation is in reality the clinical and practical application of cellular therapy. Hematopoietic transplant physicians are by design or by practical application evolving into cell and gene therapy specialists. The excitement and enthusiasm in hematopoietic transplantation is that it offers a door to the future. A future not of drugs or titrating poisonous chemotherapy but rather of cellular and gene therapy. 1 ALLOGENEIC PERIPHERAL BLOOD STEM CELL TRANSPLANTATION FOR HEMATOLOGIC DISEASES Martin Korbling University o/Texas MD. Anderson Cancer Center, Houston, Texas 77030 INTRODUCTION Circulating hematopoietic stem cells have emerged as an alternative to bone marrow (BM) stem cells for allografting. For many years the reconstitutive potential of circulating stem cells was questioned; peripheral blood stem cells (PBSC) were even characterized a waste product (1).

Book excerpt: This book addresses a variety of ethical issues that arise in the care of oncology patients. Many volumes have been written on medical ethics in the past 30 years. However, few have focused on ethical issues specific to the care of cancer patients. This book brings together such a focused examination. The contributors are experienced clinicians, ethicists, medical humanists, and medical educators. The issues raised have direct relevance to the care of oncology patients in treatment as well as research settings. The chapters address issues that are central to contemporary medical practice and medical ethics inquiry. Any practicing clinician will be well aware of the problems of communication and how uncertainty, cross-cultural issues, and religious influences can impact patient care. The limits of care and the role of advance directives and palliative care are common issues that must be addressed in treating patients at the end of life. For oncologists and oncology patients, participation in clinical trials may be a thomy topic, especially when phase I clinical trials are being considered. The impact of managed care and reimbursement issues cannot be avoided in the contemporary patient care and similarly cannot be neglected when considering the ethical ramifications raised. No discussion of ethics in oncology can be complete without attention to the specific challenges raised by the pediatric patient with cancer. All of these topics are explored by the contributors to this book.

Available in PDF, EPUB and Kindle. Book excerpt: Gastrointestinal tract malignancies encompass a spectrum of diseases with marked differences in etiology, incidence, biological behavior, and treatment outcome. The incidence of new gastrointestinal cases recorded yearly for patients residing in the United States is approximately 230,000, representing nearly 20% of all cancer cases. Worldwide, gastrointestinal malignancies are responsible for the largest number of cancer deaths, particularly because of the high incidence of hepatocellular and gastric cancer in other countries. Gastrointestinal Oncology is directed to those most involved in the multidisciplinary approach to the gastrointestinal cancer patient, including medical, radiation and surgical oncologists; gastroenterologists; interventional radiologists; pathologists; oncology nurses; and oncology physicians-in-training.

Available in PDF, EPUB and Kindle. Book excerpt: In recent years, there have been major advances in the treatment of patients with gynecologic malignancies. Perhaps the biggest advances have been in the area of ovarian cancer. Gynecologic Oncology focuses primarily upon this malignancy. This volume discusses cytoreductive surgery; screening for ovarian cancer; chemotherapy; new chemotherapeutic drugs; the controversy regarding the role of high-dose chemotherapy in gynecologic cancers; the hereditary basis for gynecologic malignancies; molecular genetics; molecular biology and new biologic therapies. Other topics covered are the treatment of all stages of cervical cancer, including radiotherapy. In addition, a chapter on advances in the pathology of gynecologic cancers is included. The advances made in the treatment of gynecologic malignancies are due, in part, to the clinical studies performed by many of the contributors to this volume. Clinical advances have been the result of multidisciplinary approaches which involve molecular biologists, pathologists, radiation therapists, surgeons and chemotherapists. Future advances will continue to rely upon collaborative interaction among these different disciplines.

Available in PDF, EPUB and Kindle. Book excerpt: Infection is a major cause of morbidity and mortality in patients with neoplastic disease because of compromised host defenses. These defects result in an increased risk of infection and its complications. The nature of the underlying malignancy, the immunodeficiencies associated with it, and the treatments directed against it are all important determinants of infection. In recent years, the introduction of more intensive chemotherapeutic regimes and the widespread use of bone marrow and peripheral stem cell transplantation have changed the pattern of infection in many patients. Furthermore, the increasing use of central venous access devices and antimicrobial prophylaxis has changed the epidemiology of infection in these patients as well. The goal of this volume in the Cancer Treatment and Research series is to emphasize that while the management of infection in cancer patients is common, it is constantly changing. With the increasing complexity of these patients, optimal management requires a multidisciplinary approach. Ultimately, it is hoped that this book will assist clinicians in the diagnosis, management, and prevention of infection in order to optimize care for patients with cancer.
