TRACY Late in the 1940s, a virus was isolated from a young patient with a flaccid par alysis in the sleepy Hudson River town of Coxsackie in the state of New York. Within the next few years, it was apparent that this and other similar viruses were not polioviruses but were indeed a new group of viruses, viruses that by the mid- 1950s had been found to be commonly associated with pediatric inflammatory heart disease. Two groups of coxsackieviruses (A and B) were differentiated on the basis of the type of paralysis induced in suckling mice by these viruses. Yet the overall contribution of coxsackieviruses to clinically evident dis ease of humans is still largely an open problem. Following their discov ery, coxsackieviruses were under intense clinical and laboratory scrutiny for a long time. Because of their relationship to polioviruses, the under standing of their structure, biochemistry, biology, and epidemiology ad vanced rapidly as a result of the formidable efforts that eventually led to the defeat of poliomyelitis. The ability of these viruses to infect mice permitted dissection of their pathogenicity in an experimental host and elucidation of conditions that influence its expression. Coxsackieviruses have been progressively associated with an increasing array of widely diverse human diseases. However, only some of the suggested causal correlations have been substantiated with satisfactory certainty. For others, conclusive evidence has so far resisted investigation. Most impor tant, among the latter are chronic maladies, such as dilated car diomyopathy and juvenile diabetes, that demand consideration. In recent times, there has been a partial eclipse of the subject of coxsackieviruses in the medical literature. In addition to the difficulties encountered in pinpointing their pathogenic potential, possible reasons include the general decline of interest in enteroviruses, which ensued after the conquest of poliomyelitis, and the continuous appearance in the limelight of new, more esoteric, and therefore more "appealing" viruses.

Serologic surveys have suggested the existence of Lassa virus infection in Guinea, Central African Empire, Mali, Senegal, Cameroon, and Benin, in addition to earlier identification in Nigeria, Liberia, and Sierra Leone. Chapter 7, Coronaviruses: New studies have confirmed the important role of these viruses in common respiratory illnesses of children and adults. The viruses are now known to contain a single positive strand of RNA. About 50% of corona virus infections result in clinical illness. About 5% of common colds are caused by strain DC 43 in winter. Chapter 8, Cytomegalovirus: Sections on pathogenesis of CMV in relation to organ transplantation and mononucleosis, as well as sections on the risk and features of con genital infection and disease, have been expanded. There are encouraging preliminary results with a live CMV vaccine, but the questions of viral persistence and oncogenicity require further evaluation.

Influenza A viruses are found in humans, pigs, horses, sea mammals, and also in wild aquatic and domestic birds [23]. How are they spread? Between mammals, influenza is an airborne infection, but between birds, influenza can be either an airborne or waterborne infection. Influenza viruses of aquatic birds periodically transmit to domestic birds sometimes with catastrophic effects; this transmis sion can be either airborne or waterborne. Less frequently, avian influenza viruses transmit to mammals and three to four times in the past century this transmission has initiated a pandemic of influenza in humans. The method of spread of avian influenza viruses to mammals remains unresolved, but could be either airborne or waterborne. In this report we will consider recent examples of interspecies transmission of influenza A viruses and the possible prevention of emergence of the next human pandemic which is considered imminent. The reservoirs of influenza A viruses The available evidence indicates that aquatic birds are the reservoirs of all 15 subtypes of influenza A viruses. We will first consider the replication of influenza A in aquatic birds.
