Book excerpt: La demande pour des soins palliatifs a connu une forte hausse depuis quelques années due notamment à l'augmentation du nombre de Québécois âgés. La restructuration du système de santé québécois favorise le maintien à domicile des personnes en fin de vie afin de leur offrir la possibilité de demeurer dans leur milieu naturel le plus longtemps possible. Afin de permettre ce maintien à domicile, il est nécessaire d'offrir des soins et des services à ces personnes afin d'améliorer leur bien-être durant cette période difficile. Le programme de soins palliatifs à domicile du Centre de santé et des services sociaux de la Vieille-Capitale (CSSSVC) offre des soins et des services personnalisés aux usagers et à leurs proches afin de permettre ce maintien à domicile en toute sécurité et en évaluant le bien-être global de l'usager et de ses proches. Ce projet a pour objectif de mesurer la satisfaction des proches des usagers de soins palliatifs à domicile sur le territoire du CSSSVC, et ce, selon une approche participative avec le milieu. Au total, 60 proches d'usagers décédés ont participé au projet afin de connaître leur niveau de satisfaction face au programme de soins palliatifs à domicile du CSSSVC, de même que leurs commentaires généraux et suggestions afin de saisir les améliorations possibles au programme. La collecte de données s'est faite par l'utilisation d'un questionnaire élaboré à partir du concept de qualité des services de l'Agence de développement de réseaux locaux de services de santé et de services sociaux de Montréal (2005). Ce concept regroupe 12 dimensions dans trois secteurs d'activités (relationnel, professionnel et organisationnel) qui sont mesurées par 44 attentes. Les résultats ont permis de conclure que les répondants étaient très satisfaits du programme de soins palliatifs à domicile. Il est également ressorti que le secteur relationnel regroupait les dimensions ayant obtenu les plus hauts taux de satisfaction, suivi du secteur professionnel et du secteur organisationnel. Ce projet a permis aux intervenants et aux gestionnaires du programme de soins palliatifs à domicile du CSSSVC de mesurer le niveau de satisfaction des proches des usagers du programme, de connaître les points forts et les points faibles du programme de même que les pistes d'amélioration possibles afin de rehausser la qualité des soins et des services du programme. De plus, ce projet vient soutenir le développement d'un mécanisme formel d'évaluation de la satisfaction des proches à être implanté dans les trois CLSC du territoire du CSSSVC. Conséquemment, ce projet permet d'établir une référence afin d'offrir un point de comparaison pour des évaluations futures.

Book excerpt: Introduction : La prise en charge de patients en soins palliatifs à domicile par le médecin généraliste est une situation de plus en plus fréquente avec le développement des réseaux de soins, le plan « cancer » et la carence en place dans les services de soins palliatifs. Objectifs : Dans le but d’aider les médecins généralistes à prendre en charge globalement ce type de patient, le premier objectif de cette thèse a été de créer, à partir d’outils préexistants, un recueil des instruments les plus adaptés au travail du médecin généraliste à domicile et spécifiques des situations les plus fréquentes rencontrées en soins palliatifs. Le deuxième objectif de cette thèse a été de réaliser une pré-enquête de faisabilité auprès d’un échantillon de médecins généralistes.Méthode : Pour répondre au premier objectif, les contraintes que les outils sélectionnés doivent remplir ont été définies, puis une revue de la littérature a permis de dégager les outils devant composer le recueil. Ce recueil a ensuite été révisé puis validé par un groupe d’experts en soins palliatifs à domicile issu d’un réseau de soins palliatifs local. Pour répondre au deuxième objectif, le recueil ainsi qu’une grille d’évaluation ont été remis à 47 médecins généralistes, dont 28 ont répondu.Résultats : Le recueil créé permet d’évaluer: la douleur chez le patient communiquant et non communiquant, la fatigue, la dyspnée, le risque d’escarre, la dénutrition, la dépression et l’angoisse. Ont été ajoutés une proposition de thèmes à aborder avec le patient et une aide à l’utilisation des résultats.82% des médecins interrogés jugent que le recueil leur a permis d’optimiser leur prise en charge. Conclusion :Ce travail de thèse a permis de développer un recueil d’outils spécifiques de la prise en charge à domicile du patient en soins palliatifs par le médecin généraliste. Il a été validé par des experts en soins palliatifs de même que par un échantillon réduit de médecins généralistes
