Although the Army's primary focus is to fight and win our nation's wars, it is often employed in stability and support operations. In stability and support operations, the Army executes missions in both peace and conflict: what combat does occur is limited to the minimum necessary to support the political objectives. The primary recipients of combat service support (CSS) in stability and support operations are likely to be civilians (US or foreign), rather than US combat forces as in war. THE CHS planner must be capable of adapting traditional methods of health care delivery, leveraging technology, and establishing new procedures to meet the challenges presented.

The numbers of personnel, medical equipment, and unit capabilities for providing organic AHS support also varies depending upon the parent formation/organization. It is essential for AHS planners to understand how AHS resources are arrayed across the battlefield and the capabilities and limitations of the various medical assets that are used across the range of military operations in support of the warfighting functions in the conduct of unified land operations and in pursuit of decisive action in any operational environment (OE). To facilitate this understanding, this publication uses the infantry brigade combat team (IBCT) base table of organization and equipment (TOE) for illustrative purposes only. Variances will exist between what is included in this publication and the actual modified TOEs of deployed units. These variances may be due to updates of areas of concentration (AOCs), military occupational specialties (MOSs), and military grades and modifications made to unit's TOE which result in the unit's modified TOE or updates to the base TOEs reflecting Total Army Analysis findings and judgments. Where significant differences exist in the base TOEs of the BCTs, an explanation of these differences is provided.

While the Department of Defense (DOD) is not a provider of first resort in disasters, requests for support from U.S. forces may be required when military-unique capabilities (such as lift capability, engineering, or deployable medical support) exist that can expedite relief efforts during urgent, life-threatening situations. This manual is a two-part publication. Part one of the ATP discusses AHS support to stability tasks and part two covers medical support to DSCA tasks. Part two of the manual, which provides a separate discussion of AHS support to DSCA tasks, the National Response Framework (NRF), National Disaster Recovery Framework, and the DOD's role in the National Disaster Medical System (NDMS). Army Techniques Publication 4-02.42 consists of seven chapters- Chapter 1 provides an overview of stability in operations, the primary stability tasks, national and DOD-level guidance, and the importance of ensuring that AHS support to stability tasks is regionally focused and conducted in consonance with the combatant commander's theater engagement strategy. Chapter 2 discusses the Department of State's Post-Conflict Reconstruction Essential Tasks matrix as it relates to the Army primary stability tasks. The chapter also provides the doctrinal description for three of the five Army stability tasks, medical aspects of the supporting initial and transformational response tasks, and corresponding health service support (HSS) and force health protection (FHP) considerations for each task. Chapter 3 discusses the employment of AHS assets in support of the primary stability tasks, the medical aspects of building partner capacity, the role of civil affairs, legal considerations, and AHS support to operations with a stability focus. Chapter 4 provides medical planning considerations for AHS support to joint operations and stability tasks to include transition and end state considerations. Chapter 5 provides a brief overview of the primary DSCA tasks, national and DOD-level guidance. This chapter also provides a brief description of medical aspects of the NRF, National Disaster Recovery Framework, and the NDMS. Chapter 6 provides legal considerations that may apply when providing medical support to DSCA tasks and a discussion of the support provided for each of the 10 medical functions. Chapter 7 describes the interorganizational and interagency coordination required in support of DSCA tasks, the process for requesting DOD assistance for support to civil authorities, some of the NDMS medical resources that may be employed during a disaster, as well as some of the participating organizations that may be involved in the relief effort.

Book excerpt: In 2004-2006, the U.S. government acted to revise the way that the planning and implementation of Stabilization, Security, Transition, and Reconstruction (SSTR) operations are conducted. The primary emphasis of the changes was on ensuring a common U.S. strategy rather than a collection of individual departmental and agency efforts and on mobilizing and involving all available U.S. government assets in the effort. The proximate reason for the policy shift stems from the exposing of gaps in the U.S. ability to administer Afghanistan and Iraq after the U.S.-led ousters of the Taliban and Ba'athist regimes. But the effort to create U.S. government capabilities to conduct SSTR operations in a more unified and coherent fashion rests on the deeper conviction that, as part of the U.S. strategy to deal with transnational terrorist groups, the United States must have the capabilities to increase the governance capacities of weak states, reduce the drivers of and catalysts to conflict, and assist in peacebuilding at all stages of pre- or post-conflict transformation. According to the Joint Operating Concept for Military Support to SSTR operations, these operations are civilian-led and conducted and coordinated with the involvement of all the available resources of the U.S. government (military and civilian), nongovernmental organizations (NGOs), and international partners. Although military assets are an essential component of many SSTR operations, specific military goals and objectives are only a portion of the larger SSTR operation.

Although Joint doctrine describes the capabilities of the Military Health System as a taxonomy of care, this description does not adequately address how the AMEDD must organize and equip its forces to successfully accomplish the health care delivery in the noncontiguous operational environment. Therefore, the AHS is in consonance with and supports the concept of the taxonomy of care, but AMEDD support is discussed in terms of capability packages specifically designed to support Army formations. Although the Military Health System is an interrelated system which may share medical services, capabilities, and specialties among the Service components, it is not a joint mission command system. Each Service component develops its medical resources to support its Service-specific mission. This results in the development of different types of organizations with varying levels of capability, mobility, and survivability. Although joint medical resources may have similar nomenclature to describe the unit, they are not usually interchangeable.

Book excerpt: This publication provides doctrine for the Army Health System (AHS) in support of the modular force. TheAHS is the overarching concept of support for providing timely AHS support to the tactical commander. It discusses the current AHS force structure modernized under the Department of the Army (DA)-approved Medical Reengineering Initiative and the Modular Medical Force that is designed to support the brigade combat teams (BCTs) and echelons above brigade (EAB) units.As the Army's AHS doctrine statement, this publication identifies medical functions and procedures that are essential for operations covered in other Army Medical Department (AMEDD) proponent manuals. This publication depicts AHS operations from the point of injury, illness, or wounding through successive roles of care within the area of operations (AO) and evacuation to the continental United States (CONUS)-support base. It presents a stable body of operational doctrine rooted in actual military experience and serves as a foundation for the development of AMEDD proponent manuals on how the AHS supports unified land operations.The AHS mission falls within two warfighting functions: sustainment and protection. To clearly delineate the two AHS missions of health service support (HSS) and force health protection (FHP), this publication is divided into three parts.Part One, AHS, provides a holistic view of the entire AHS and the complexities and interdependence of each medical function in successfully accomplishing the AMEDD's mission to conserve the fighting strength. This part of the manual describes and provides operational guidance on the AHS's EAB mission command headquarters, as well as the medical aspects of the Law of Land Warfare.Part Two, HSS, discusses the three mission sets of casualty care, medical evacuation, and medical logistics (MEDLOG). Casualty care encompasses all of the medical functions involved with direct patient care activities to include diagnostic medical laboratories, while medical evacuation and MEDLOG are separate medical functions.Part Three, FHP, encompasses preventive medicine, veterinary services, all of the preventive aspects of combat and operational stress control (COSC) and dental services, and area medical laboratory (AML) including the testing of suspect biological and chemical warfare agent specimens and samples.This publication is for use by commanders and their staffs and command surgeons. It is to be used as a guide in obtaining, as well as providing, AHS in an AO. Information in this publication is applicable to decisive actions in support of unified land operations. It is compatible with the Army's sustainment and protection doctrine and is in agreement with Joint Publication (JP) 4-02.Due to the nature of the medical profession which is highly regulated throughout both the civilian and military communities, AMEDD doctrine is heavily influenced by—• United States and international law.• Policy guidance in the form of Army regulations (ARs) and Department of Defense (DOD) policy promulgated in the form of DOD directives (DODD) and instructions (DODI) and other documents.• Medical standards established by civilian organizations (such as the Joint Commission on the Accreditation of Health Care Organizations).• Technical guidance from both military and civilian organizations charged with medical/scientific oversight responsibilities.Throughout this publication, as appropriate, reference is made to the major policy guidance impacting the specific topic.
