Book excerpt: Le remplacement a longtemps été considéré uniquement comme un passage obligatoire avant une installation libérale. Dans le contexte actuel de mesures gouvernementales visant à favoriser l'installation ou le regroupement dans les zones sous-médicalisées, nous avons réalisé une enquête auprès des remplaçants en médecine générale et des omnipraticiens récemment installés en Picardie. Celle-ci met en évidence un retard croissant à l'installation ainsi qu'une attractivité relative de l'exercice libéral face aux attraits d'un mode d'exercice salarié. Il ressort que les facteurs déterminants à cette installation chez les jeunes praticiens qui redoutent l'isolement d'un exercice solitaire sont au moins autant d'ordre personnel que professionnel. Il a été mis en évidence que si les mesures proposées (dont certaines ne sont pas fixées de façon définitive à ce jour) ne semblent pas suffisantes pour induire une modification marquée des comportements d'installation, il semble néanmoins possible de réduire les inégalités. Nous ne pouvons que souhaiter l'efficacité de telles mesures ce qui permettrait d'éviter une remise en cause de la liberté d'installation qui nous semble constituer un des fondements de la médecine libérale actuelle
