The concluding portion of the text describes how the proctoscopic and colonoscopic examinations are performed. The atlas itself is a topographical stratification of the various diseases that may involve the perianal region, the anus, and the adjacent portions of the large bowel as well as their classification according to morphologic criteria, for example, inflammatory bowel diseases and tumors. Since many pathologic findings in the rectum and sigmoid colon are comparable to those in the upper portions of the large bowel, a duplication of these proctoscopic and colonoscopic illustrations has been avoided.

With the advent of the coloscope it is now possible to examine the large bowel more extensively and effectively than was formerly possible with the use of its rigid forerunner, the rectosigmoidoscope. The earliest rectoscopes, 11 endoscopes" introduced during the 19th century by Segales and Desormeaux (1826, 1853), were conceived for many uses-inspection of the urethra, bladder, uterus, rectum. Successive improvements eventually led to the construction of an instrument designed exclusively for endoscopy of the rectum and sigmoid colon (Bensaude, 1907). Over the following years the rectoscope underwent no substantial change and thus an abundant and homogeneous literature accrued in which the available instruments and their use were described while the endoscopic pathologic nosography and corresponding endoscopic pictures of the rectum and sigmoid colon were codified, illustrated first by simple sketches and later by still and motion photography. The literature contains many excellent publications, some in monograph form.

Brigit van der Werf-Messing, M.D. Professor of Radiology Past Chairman of the International TNM-Committee of the VICC Rotterdam, July 1982 Preface In 1938 the League of Nations Health Organization published an Atlas Illustrating the Division of Cancer of the Uterine Cervix into Four Stages (J. Heyman, ed., Stockholm). Since this work appeared, the idea of visual representation of the anatomical ex tent of malignant tumours at the different stages of their develop ment has been repeatedly discussed. At its meeting in Copenhagen in July 1954, the DICC adopted as part of its programme "the realization of a clinical atlas". How ever, the time to do the planned book of illustrations was not ripe until the national committees and international organizations had officially recognized the 28 classifications of malignant tu mours at various sites as presented in the third edition of the TNM Booklet edited by M. Harmer (TNM Classification of Malig nant Tumours, 1978). This was all the more important since publi cation of the Booklet was followed in 1980 by publication of a Brochure of Checklists, edited by A.H.
