These surveys are used to evaluate the performance of the TB program, and to assess trends over time. To achieve this objective, data are collected through standards methods in a well-defined study population. This document is meant to provide information on the core survey methods, including diagnostic tests for TB, screening strategies, and case definitions. The target audience includes TB experts and advisers at national and international levels, and investigators involved in prevalence surveys.

The 25 surveys consisted of 13 in Asia and 12 in Africa. Collectively, survey findings have informed the policies, plans and programmatic actions needed to address gaps in TB diagnosis and treatment and to reduce the burden of TB disease. Finally, the 24 countries have a robust baseline for assessing progress towards new global targets set in the UN Sustainable Development Goals (2016–2030) and WHO’s End TB Strategy (2016–2035). The methods, results, successes achieved, challenges faced and lessons learned from the 25 surveys were comprehensively documented in the book. We viewed such a product as a global public good, that should be available to all those with an interest in and commitment to using survey findings, now and in the future e.g. academics, donors, public health officers and national TB programmes. As with implementation of the 25 surveys themselves, the book is the result of a major global, regional and national collaborative and collective effort, with more than 450 contributors from all around the world.

Book excerpt: There have been calls to revisit the experiences of TB screening campaigns that were widely applied in Europe and North America in the mid-20th century, as well as more recent experiences with TB screening in countries with a high burden of the disease, and to assess their possible relevance for TB care and prevention in the 21st century. In response, WHO has developed guidelines on screening for active TB. An extensive review of the evidence has been undertaken. The review suggests that screening, if done in the right way and targeting the right people, may reduce suffering and death, but the review also highlights several reasons to be cautious. As discussed in detail in this book, there is a need to balance potential benefits against the risks and costs of screening; this conclusion is mirrored by the history of TB screening. This publication presents the first comprehensive assessment by WHO of the appropriateness of screening for active TB since the recommendations made in 1974 by the Expert Committee. However, the relative effectiveness and cost effectiveness of screening remain uncertain, a point that is underscored by the systematic reviews presented in this guideline. Evidence suggests that some risk groups should always be screened, whereas the prioritization of other risk groups as well as the choice of screening approach depend on the epidemiology, the health-system context, and the resources available. This book sets out basic principles for prioritizing risk groups and choosing a screening approach; it also emphasizes the importance of assessing the epidemiological situation, adapting approaches to local situations, integrating TB screening into other health-promotion activities, minimizing the risk of harm to individuals, and engaging in continual monitoring and evaluation. It calls for more and better research to assess the impact of screening and to develop and evaluate new screening tests and approaches.

Module 2: screening – systematic screening for tuberculosis disease is an updated and consolidated summary of WHO recommendations on systematic screening for tuberculosis (TB) disease, containing 17 recommendations for populations in which TB screening should be conducted and tools to be used for TB screening. TB screening is strongly recommendations for household and close contacts of individuals with TB, people living with HIV, miners exposed to silica dust, and prisoners. In addition, screening is conditionally recommended for people with risk factors for TB attending health care, and for communities with risk factors for TB and limited access to care (e.g. homeless, urban poor, refugees, migrants). General population screening is recommended in high-burden settings (0.5% prevalence or higher). Symptoms, chest radiography (CXR), and molecular WHO-recommended rapid diagnostic tests for TB are recommended as screening tools for all adults eligible for screening. Computer-aided detection programmes are recommended as alternatives to human interpretation of CXR in settings where trained personnel are scarce. For people living with HIV, C-reactive protein is also a good screening tool. This guideline document is accompanied by an operational handbook, the WHO operational handbook on tuberculosis. Book excerpt: This dissertation examines the relationships between immigration and the incidence of tuberculosis (TB) disease and prevalence of TB infection in California and in the United States (U.S.). The majority of TB cases in California occur among foreign-born persons from countries with high TB burdens. Although the incidence of TB is declining, the decline among immigrants has stalled, and relationships of this trend with changes in immigration patterns are not well characterized. Census data suggest that immigration to California has undergone notable transitions since the 1990s that have shaped the volume, demographic and socioeconomic composition of immigration streams to the state. The first two papers of this dissertation explore the relationships between changes in immigration and the incidence of TB using the decomposition method, and an innovative application of cohort analysis. The third paper examines the only nationally representative survey of new immigrants in the U.S. to determine gaps in TB screening and to assess the prevalence and risk of latent TB infection (LTBI) in immigrant subgroups by country of origin. This unique data source allowed for the assessment of important immigration, health, and socioeconomic variables. The decomposition of the recent decline in the incidence of TB among the foreign-born in California shows that changes in the population composition associated with immigration shifts made a modest contribution to the decline, with the majority of the decline due to group-specific declines in rates. There was a notable difference between Hispanics/Latinos and Asians/Pacific Islanders, whereby the impact of immigration shifts was much greater in the former group. The cohort analysis shows that waves of immigration have had varying impacts on the incidence of TB in California, some cohorts contributing to increasing incidence, and others to decreasing incidence. This suggests that the slowing in the decline of the incidence of TB may be due, in part, to immigration cohort entry effects. Identifying cohorts with elevated risk of TB can help develop and test hypotheses on the epidemiologic reasons for the observed effects, and can provide an evidence base for selecting demographic subgroups in which to prioritize TB screening. The TB screening uptake and TB prevalence analysis confirmed gaps in testing for TB that varied significantly by country of origin. Substantial variation in the prevalence and risk of LTBI was also found, which may inform public health efforts to prioritize the targeted testing and treatment to specific subgroups of highest risk.
