Book excerpt: Objectif : Cette étude s'est intéressée à l'apport des Equipes Mobiles de Soins Palliatifs (EMSP) dans la prise en charge des patients âgés hospitalisés en situation palliative. Méthode : Sur une période de 6 mois, nous avons étudié des patients hospitalisés en gériatrie pour lesquels l'EMSP avait été sollicitée. Résultat : 190 patients ont été inclus dans l'étude. L'âge moyen était de 89 ans. Le diagnostic principal d'appel était la présence d'une polypathologie dans la moitié des cas. Il y avait 22,6% de cancer. Une décision de limitation de traitement fut prise avec l'EMSP pour 47,9% des patients. L'intervention de l'EMSP était associée à une augmentation significative de la prescription des médicaments de palier 3, du Midazolam, de la Scopolamine, des prémédications avant les soins et des soins de bouche. Book excerpt: Introduction : Le vieillissement de la population impacte la prise en charge dans les services de spécialités. L'évolution des techniques et des prises en charges permet une durée de vie plus longue avec des pathologies de plus en plus lourdes, compliquant les soins à moyen ou long terme. Ceci pose de nombreux défis tant gériatriques qu"éthiques. Les équipes mobiles de gériatrie et de soins palliatifs sont de plus en plus sollicitées. Notre étude avait pour objectif de décrire la population pour laquelle les équipes mobiles sont demandées et leur travail commun auprès des équipes et des patients. Méthode: Une étude descriptive rétrospective observationnelle a été réalisée sur r ensemble des patients vus par les deux équipes mobiles au cours de la même hospitalisation, entre le premjer janvier 2015 et le 31 décembre 2016, au sein du Centre Hospitalier Universitaire de Besançon. Résultats : 67 patients ont pu être inclus. Nous avons pu mettre en évidence que les patients pour lesquels interviennent conjointement les équipes mobiles sont des patients fragiles ou pré-fragiles, polypathologiques, dénutris, dont l'évolution est rapide. Les équipes mobiles sont appelées tardivement dans la prise en charge (première équipe mobile à 15,59 jours d'hospitalisation) posant la question de l'identification des patients et des situations éthiques/palliatives. Mais elles sont très réactives (0,98 jours après l'appel). Elles font de nombreuses recommandations thérapeutiques et non thérapeutiques pour aider aux soins et interviennent généralement suite à la demande d'une autre équipe mobile. Elles assurent un suivi important des patients (consultation dans les services mais aussi à distance). Conclusion : Leur travail conjoint permet une prise en charge pluridisciplinaire et globale centrée sur des patients fragiles. Mais leur intervention est souvent trop tardive. Il serait intéressant de savoir dans quelle mesure les recommandations faites sont suivies par les équipes et comment pourrait être améliorée lïdentification des patients.

Dans la continuité du Plan triennal 2015-2018 qui consacre un axe d'intervention au déploiement des soins palliatifs à domicile, l'équipe mobile de soins palliatifs (EMSP) stéphanoise souhaite intensifier le soutien aux professionnels et aux aidants en ville. Cet article a pour objectif de décrire le fonctionnement et les bénéfices de trois EMSP de la région Rhône-Alpes, pour des patients en soins palliatifs, à domicile, non suivis par un service d'Hospitalisation à domicile (HAD). Méthodes. Étude de cas avec analyse qualitative des actions des dispositifs et d'un choix de consultations marquantes. Résultats. Sur l'ensemble des interventions physiques, le domicile représente entre 14% et 25% de l'activité des EMSP1&2. L'EMSP3 qui n'intervient qu'à domicile a réalisé 313 visites en 2016. Au regard de vignettes cliniques et des rapports d'activité, ces équipes sollicitées par les hôpitaux et les médecins traitants interviennent pour des problèmes éthiques, de douleur, ou autres symptômes d'inconfort, pour un soutien psychologique et social du patient et des aidants, pour une évaluation globale et pluridisciplinaire, ou un soutien aux équipes déjà en place. Conclusion. L'EMSP joue un rôle interstitiel fondamental cimentant un bricolage d'acteurs multiples, motivés, mais parfois aussi souffrants ou maladroits autour d'un malade en fin de vie à domicile. Maillon entre la ville et l'hôpital, l'EMSP essaye de respecter l'intimité des familles, les choix des patients, le professionnalisme des soignants. Ces consultations complexes demandent du temps, de la disponibilité, et de l'expérience, et exigent une approche transdisciplinaire.

Cette méthode d'évaluation peut être utilisée par d'autres EMASP. Elle a été adoptée, après modifications liées aux particularités d'exercice, par un réseau de soins palliatifs à domicile. L'EMASP de Meaux réfléchit actuellement à une analyse du contenu des interventions pour rendre l'outil complet

L'accompagnement de la fin de vie fait partie intégrante du quotidien de ces établissements. Chacun d'entre eux recense en moyenne une vingtaine de décès par an. Depuis 2010, l'intervention d'équipes mobiles de soins palliatifs y est autorisée. L'étude s'intéresse aux apports d'une de ces équipes, celle du GH-ICL, dans les EHPAD l'Accueil et Saint François de Sales (Nord). Matériel et méthode : étude qualitative observationnelle avec d'une part la réalisation d'entretiens semi-dirigés auprès de médecins traitants et d'autre part l'organisation de focus group avec les soignants des EHPAD concernés. Résultats : l'ensemble des médecins et soignants interrogés se sont montrés satisfaits du partenariat avec l'EMSP. Cette équipe leur apporte soutien et conseils dans les prises en charge de fin de vie : aide dans les prescriptions, aide à la prise de décision collégiale, soutien des soignants, accompagnement du patient et de ses proches et maintien du résident dans son lieu de vie. L'étude met aussi en évidence des perspectives d'amélioration notamment dans la communication entre les différents acteurs et la formation théorique des soignants. Conclusion : les soins palliatifs ont une place primordiale au sein des EHPAD, lieux de vie et de fin de vie. L'EMSP du GH-ICL apporte son expertise et sa disponibilité aux médecins traitants et aux équipes soignantes. Cette collaboration permet une amélioration de la prise en charge globale du patient à travers la diffusion de la culture palliative.
