Book excerpt: Introduction : un médecin urgentiste doit être capable de mobiliser des compétences techniques mais également non-techniques pour assurer une qualité de soins constante aux patients malgré l'augmentation incessante du flux de patients aux urgences et des ressources matérielles et humaines invariables. Les études récentes ont permis d'identifier des compétences non-techniques mobilisées par les médecins urgentistes mais sans établir de manière quantifiable le rapport entre ces compétences et la qualité des soins. Objectif : l'objectif de notre étude fut l'identification et l'analyse des compétences non-techniques mobilisées qui permettraient une amélioration de la qualité des soins. Méthode : nous avons réalisé une étude prospective observationnelle de simulation dans un service d'urgence virtuel. Deux analyses ont été effectuées à partir de cela, une quantitative sur l'association entre les compétences non-techniques et la qualité de soins et l'autre qualitative sur la description de ces compétences non-techniques selon les caractéristiques des médecins amenant à une théorisation du processus de qualité de soins. Résultats et discussion : au vu des résultats quantitatifs, il n'y a aucune association formelle entre les compétences mobilisées par les médecins et la qualité de soins administrés. A partir de l'analyse qualitative, nous avons préjugé d'une amélioration de la qualité de soins par la mobilisation de compétences relatives aux facteurs humains avec la relation aux patients et à l'équipe.

De nombreuses optimisations organisationnelles sont étudiées pour améliorer la qualité des soins. Cependant l'apport des compétences non techniques est peu étudié en médecine d'urgence. Objectif : Montrer la faisabilité d'une simulation permettant de collecter l'ANTS, la durée de prise en charge, l'orientation. Méthode : Il s'agit d'une étude pilote interne, observationnelle, prospective visant à évaluer la mobilisation des compétences non techniques et la qualité des soins dans un service d'urgence standardisé virtuel. Les situations simulées sont représentatives de la population prise en charge aux urgences. Résultats : Nous avons pu recueillir 100% des données de compétences non techniques et d'orientation, 99% des données de durée de prise en charge. Nous avons réuni un panel de 10 médecins urgentistes exerçant en Midi-Pyrénées depuis plus de 2 ans. Le groupe de compétences : " connaissance de la situation " est le moins mobilisé par les participants. Discussion : La perte de 10% des données concernant la durée de séjour correspond à la littérature. Il existe un risque de perte de données sur ce critère de jugement induit par un recueil non automatisé d'une quantité importante de données. Le protocole nécessite un chercheur supplémentaire afin d'améliorer l'exhaustivité du recueil de données. Il est techniquement faisable de mener un protocole de recherche utilisant un environnement virtuel de simulation dont l'objectif final est de corréler la qualité des soins aux compétences non techniques mobilisées par les médecins urgentistes.
