The Porro operation has fallen in its rate of mortality since 1884 from 58 to less than 20 per cent. And the Conservative or Improved Caesarean, from 45 per cent. To a general average of 20 per cent., and for Continental Europe of 12. The exsective Operation named had had but one case prior to 1885, but has now had five without the death of a mother. These facts are not mentioned in the last English edition. Laparo-elytrotomy, which was attracting considerable attention four years ago, has almost ceased to exist, by reason of the diminished death-rate under the improved Caesarean section, which in Germany has been one case lost In eight. Laparo-elytrotomy has therefore not been performed since September 18, 1887. The Editor has brought up the work to date upon these subjects, and their statistical records to the close of the year 1888. All Of the American additions, except the article upon the forceps, have been either rewritten or remodelled, and many new and short notes have been added where required. The work has been sufficientlv Americanized, upon the points Where English and American Obstetri cians differ in Opinion and practice, to fit it for the uses of American medical students and Obstetricians. Book excerpt: Excerpt from The Principles and Practice of Midwifery With Some of the Diseases of Women There are not wanting many admirable works on the principles and practice of Obstetrics; but there are various principles and rules which the Author has found of inestimable service in his own practice, which are either not mentioned in these, or are referred to only in terms of disapprobation. These principles and rules have reference, more particularly, to the management of the second and third stages of labor. There is assuredly a wide and growing belief, not only among the younger practitioners, but also among not a few veteran brethren who have been able to emancipate themselves from the shackles of doubtful, if not injurious, traditions, that too great delay in the second stage of labor especially has hitherto been inculcated in the text-books, and that, had earlier interference been more widely taught and practiced, maternal and foetal mortality would have been greatly diminished. Recent investigations strengthen this view, bringing out, as they do very strikingly, the fact that delay has proved much more injurious than interference, and, moreover, has given a false complexion to operative procedure not necessarily attended with danger. These views, therefore, the Author has endeavored to give expression to, in the hope that good may gradually accrue. The seed he believes to be good, and he trusts that it may fructify in the course of time, and yield the welcome blossom of maternal safety. That is the harvest all obstetricians are earnestly striving after. Prominent notice has been taken of the operations of cephalotripsy and spondylotomy - operations which the Author believes are destined to limit considerably the difficulties and maternal dangers of craniotomy; and reference has been made, though briefly, to the influence of zymotic disease on gestation and the puerperal state. Had space permitted, he should have liked exceedingly to have entered more fully into these important topics. As regards the subject of Diseases of Women, he has to remark that, while an account of the great mass of these would have been quite foreign to the purpose of the work, he has yet introduced more of them than is usual in such manuals. He has, however, only touched on those closely connected with pregnancy and the puerperal state. He has not inserted many cases in illustration of principles and practice, but the reason is the want of space, and not the want of such cases. He has numerous notes; and he writes within gunshot of many brethren who have seen him operating in the more difficult cases of labor. These examples, if opportunity offers, he may yet bring together, either as a supplementary or separate work. Book excerpt: Excerpt from Smellie's Treatise on the Theory and Practice of Midwifery, Vol. 2 The following Collections are intended to confirm and illustrate the method of practice recommended in my former Treatise, or first volume upon Midwifery; and are placed in the same order observed in that performance, to which they have references, as well as to a series of tables, to the number of thirty-nine, now ready for publication, which I have prepared, with a view to demonstrate and explain what otherwise might not be so well understood. These will, in some measure, supply the want of proper references in the former impressions of the first volume; though that defect is remedied in this edition. Between the years 1722 and 1739, while I practised in the country, I took notes of all the remarkable cases that occurred in midwifery; but in London, since the year 1740 to the present time, I have been more careful and minute in forming a Collection, with a view to make it public. From a great number of instances, I have selected only the most material, and such as were best adapted to the nature of my plan; for I was unwilling to tire the reader with a succession of parallel histories that contain nothing essentially different from one another. In order to render the performance still more complete, I have taken from authors of the best authority a few extraordinary cases which seldom occur, as well as borrowed some medical transactions from the most approved modern physicians.
