Its close relation to sedentarism, unhealthy diets and obesity, increase the risk of diabetes, mainly in genetically susceptible populations. Achieving metabolic control goals is the main objective in treating diabetes, although only around 30% of affected patients achieve a goal HbA1c. Innovative approaches are needed to increase metabolic control in type 2 diabetes, favoring healthier lifestyles.AimTo evaluate the effect of a nutritional intervention and diabetes education multimedia on type 2 diabetes control and promoting healthier lifestyle.MethodA clinical trial was conducted during 12 months in patients with type 2 diabetes at the Mexican Institute of Social Security in Mexico. Subjects with end stage kidney disease, amputation or retinopathy were excluded. The study was approved by the local IRB and all patients that agreed to participate signed an informed consent form. The intervention consisted in providing diabetes education with multimedia software including interactive modules with topics of nutrition, exercise, diabetes myths, metabolic control parameters, family support and complications of the disease. After reviewing each educational module, the patient received personalized nutritional therapy, considering age, sex, physical activity and present comorbidity. In each session, counseling was provided to reinforce the knowledge acquired through multimedia software and to promote favorable lifestyle changes. Every month during a six-month period, the patients reviewed an educational module and received nutritional counseling and weight, body composition, waist circumference, capillary glucose and blood pressure were measured. On month 8 all patients had an appointment for reinforcement and review of a particular module if needed. On month 12, a new assessment was done, including clinical examination, biochemical, anthropometric and body composition measurements. Lifestyle was measured at the beginning of the study and at the end (12 months), using a previously validated questionnaire for assessing lifestyle in patients with diabetes (IMEVID). This questionnaire includes 25 closed questions grouped in 7 dimensions: nutrition, physical activity, smoking, alcohol consumption, diabetes information, emotional management, and treatment compliance. According to the score obtained in the analysis of the questionnaire, lifestyle was categorized in unfavorable (0-59), slightly favorable (60-80) and favorable (> 80). Continuous variables were compared with a paired Student t test, and categorical variables with a chi squared.ResultsA total of 219 patients completed the study, 157 (72%) were women, the mean age was 55.9u00b18.2 years, the mean duration of the disease was 7.1u00b15.3 years, and the most common treatment were oral hypoglycemic agents (79%). Only 37% of the population had received nutritional therapy and 34% education on diabetes. At the end of the study, HbA1c decreased from 8.3u00b11.9 to 7.0u00b11.9%; (p= 0.001) and the body weight from 74.5u00b113.6 to 73.8u00b113.6 kg, (p=0.015). The proportion of patients with favorable lifestyle at baseline was 7%, slightly favorable 57% and unfavorable 36%. At the end of the study favorable lifestyle increased to 13%, slightly favorable to 65% and unfavorable decreased to 22% (p=0.001).ConclusionEducation in diabetes through multimedia software and reinforcement with nutritional therapy promotes healthier lifestyle, and as a consequence improves HbA1c and body weight in patients with type 2 diabetes.

This meeting followed the three earlier symposia on Menière’s disease, which have been organized every six years since 1981 in Düsseldorf, Boston and, most recently, in Rome. 1999 saw the bicentenary of Menière’s birth, and to commemorate this, François Legent organized a historical symposium on the different aspects of Prosper Menière’s life. Together with Michel Portmann, the two honorary presidents, Robert Kimura and Jean-Marc Sterkers, gave excellent lectures on the two main topics of Menière’s disease: on experimental endolymphatic hydrops, and the role of both vestibular neurectomy and sac surgery. Numerous papers were presented on research and clinical experiences, and were particularly devoted to the better understanding of both the development of experimental endolymphatic hydrops and the evolution of Menière’s disease. Attention was also drawn to the local treatment of Menière’s disease by the application of drugs to the round window, and furthermore, the indications for and results of surgery for incapacitating vertigo were well defined. Finally, we are pleased to say that this was a very successful meeting and we would like to express our thanks to all the companies, and in particular Solvay Pharmaceuticals, that supported this event. Diabetes is a major chronic health problem affecting 13 to 14 million people in the United States (U.S.) with approximately five percent of these diabetic patients over the age of 65 years. Complications from diabetes are continuing to rise in the U.S. each year. Diet, exercise, and medications are the three main components of the treatment for people with non- insulin dependent diabetes mellitus (NIDDM). Non-adherence to a diabetic regimen leads to end organ damage, co-morbidity, decreased quality of life, and eventually death. Adherence to this regimen leads to the greatest health improvement, yet these critical components are the most difficult to change. The success of the diabetic regimen often depends on the patient's available resources that influence or initiate a change in behavior. Family and social support could be one of the resources needed to help improve diabetes management and glycemic control. The purpose of the proposed innovation is to initiate a comprehensive diabetic education program for NIDDM patients using Orem' s Self-Care Theory that will incorporate the patient's family and social support into the treatment care plan. The proposed innovation will be conducted in a five-week self-management series designed for the individual with NIDDM and his/her family or the support system. The education program will include a pre-education needs assessment and both a knowledge and physiological component. A post-education knowledge test will be repeated at six months and the physiological component will be repeated at three months and six months. The pre- and post-education values will be compared at six months.
