Book excerpt: "A major current concern in total hip arthroplasty is the generation of polyethylene wear particles at the articulating surfaces and resulting macrophage-mediated, periimplant osteolysis. There has been revived interest in metal-metal bearings for hip implants because of their potential for improved wear performance. Cobalt chromium molybdenum metal-metal hip implants of two materials (alloy types) and two carbon content levels were custom-manufactured and tested in a hip simulator under various loading conditions. In general, accelerated wear occurred within the first million cycles of testing followed by a decrease in wear rate to low steady-state values. The volumetric wear at three million cycles was up to several orders of magnitude less than the wear of conventional metal-polyethylene hip implants. Increased head-cup clearance and surface roughness resulted in increased wear. Independent effects on wear of material and carbon content were not identified. The low wear may be related to fluid film lubrication at the bearing surfaces. A time-varying elastohydrodynamic lubrication model was developed to predict fluid film thickness for the experimental metal-metal hip implants during simulator testing. Decreased clearance, increased lubricant viscosity, and increased cycle frequency resulted in increased film thickness. A strong independent effect on film thickness of load magnitude was not found. The lambda ratio (of film thickness to combined surface roughness) was used to indicate the extent of fluid film lubrication. For the experimental implants, total wear decreased with increasing lambda ratio. These studies provided important new information on the design and testing parameters that control wear performance and lubrication of metal-metal bearings for hip implants." --

Book excerpt: To determine what the trends for the use of total hip arthroplasty bearings have been in the United States and how the Food and Drug Administration (FDA) warning on the use of metal on metal bearings have affected surgeon's decisions concerning their use, a survey was performed on the fellows of the American Academy of Orthopaedic Surgeons (AAOS). Two thousand randomly selected practicing fellows were invited to take part in the AAOS 2010 survey. Of those invited, 170 (8.5 % response rate) completed the survey. Confidence interval (CI) values at 95 % were provided for each survey item. There were no significant differences between invitees and respondents in terms of practice setting and gender. The majority of respondents (79.2 %) match their patients' function/activity to the bearing surface used in total hip arthroplasty (THA). Respondents mostly use peer reviewed publications (92.9 %), and personal experience (81.7 %) when making a decision on bearing use in THA. Almost 80 % of the respondents indicated that their hospitals do not limit their choice of implants. Cross-linked polyethylene has been the most used type of bearing surface before 2010 (74.80 %) and currently (78.62 %). Metal-on-metal and ceramic-on-ceramic use decreased; however, there are no significant differences between the uses of all types of bearings before 2010 and currently. Given the current debated issues, 75.6 % of respondents are less likely to use metal-on-metal bearing in the future; 22 % are not affected. The majority of respondents (70.3 %) indicated that ceramic-on-ceramic bearing have fewer wear particles. More than half (56.7 %) indicated that metal-on-metal (MoM) has the longest survivorship. This survey indicated that surgeons responded that their use of MoM bearings will likely decrease in the future but the numbers in this survey show no significant difference in their use before and after the FDA warning for MoM bearings. The survey does show that most surgeons believe that they will have less choice as directed by their hospital or insurance companies in their bearing use in the near future due to health economics and the efforts to decrease cost. Only time will tell if surgeons who indicate the use of MoM will decrease over time actually comes to fruition or if the use of MoM will continue at its current rate. This survey indicates that although the trend shows a decrease in their use, it was not statistically significant.

Over the last 40 years, with the development of new materials and better designs, great strides have been made in the technologies of total joint replacement. The continued success of total hip replacement has led to operations on younger patients, who are placing higher and higher demands on their total hip replacement. Thus, the implants being put in today are expected to last longer than ever before, potentially upwards of 30 years. In the pursuit of these long lasting implants, numerous alternative bearing materials have been considered, one of which consists of a metal-on-metal (MoM) bearing couple using CoCrMo alloy. The first generation of MoM implants were implanted in the mid 20th century, before being eclipsed by the success of polyethylene bearings. While many of these implants saw early failure attributed to variability in manufacturing and surgical practices, other 1st generation MoM implants performed well for 25 years or more. As demand grew for a very low wearing bearing couple, the concept of MoM bearings was revisited. After simulator testing showed extremely low wear rates, a second generation of MoM implants was introduced in the early 2000s, with MoM bearing use reaching as high as 30% in 2010. Clinical performance of these new MoM bearings did not live up to pre-clinical testing. Reports of higher-than-expected revision rates and bearing wear leading to metal toxicity led to the recall of one design, and widespread controversy regarding the performance of all MoM devices. This study aims to identify mechanisms for MoM failure by analyzing a set of 109 MoM retievals obtained by the Dartmouth Biomedical Engineering Center. The bearing surfaces will be examined for evidence of in vivo damage and measured using a coordinate measuring machine. Finally, potential mechanisms for observed damage and wear will be explored with finite element analysis. The retrieval analysis showed that MoM devices were not performing in vivo as expected from in vitro testing. In the 109 MoM devices in the set of retrievals, all of the devices showed unexpected damage. The mean linear wear rate was greater than predicted from in vitro testing. The dam[age and wear on the retrieved bearings indicates that the hydrodynamic lubrication layer was failing in vivo. Damage and wear do not appear to vary between different device designs, indicating that the issue may lie with MoM articulation. Modeling of edge loading scenarios identified roll-out edge loading, a new mechanism of micro-separation and edge loading, as a potential mechanism for damage and wear. Finite element analysis showed that linear damage features commonly observed on retrieved femoral devices could be due to edge loading due to micro-separation. While these edge loading events were not correlated to increased wear within our set of retrievals, it is clear that edge loading due to micro-separation has the potential to damage both bearing surfaces, possibly leading to inferior performance of these devices. The mechanisms of edge loading in all total hip replacement devices need to be fully understood and accounted for during device design and testing.
