Book excerpt: This Annual Report of the Department of Defense (DOD) Chemical and Biological Defense Program (CBDP) provides information in response to several reporting requirements. First, this report is provided in accordance with 50 U.S. Code Section 1523. (The complete reporting requirement is detailed at Annex L) This report is intended to assess: (1) the overall readiness of the Armed Forces to fight in a chemical biological (CB) warfare environment and steps taken and planned to be taken to improve such readiness; and, (2) requirements for the chemical and biological warfare defense program, including requirements for training, detection, and protective equipment, for medical prophylaxis, and for treatment of casualties resulting from use of chemical and biological weapons. The CBDP provides an integrated collection of chemical and biological defense systems to U.S. forces. The overall readiness of U.S. forces is dependent on many factors. One key factor is the availability of equipment. Chapter 2 summarizes equipment requirements and the status of research, development, test and evaluation (RDT & E) and acquisition efforts across all capability areas. Chapter 3 details the logistics status of CB defense systems. The overall logistical readiness status of the Department's CB defense equipment has improved slightly. Several factors have had an adverse effect on the overall DOD readiness and sustainment status: increased demands by the Services for some CB defense equipment; the increased overall Service requirements in order to support operations in Iraq and Afghanistan; the reorganization and the approved strength increase of the Army; and equipment modernization efforts in all of the Services. Another key factor in overall readiness is the education, training, and exercises conducted by U.S. forces to remain prepared for chemical and biological threats. Education, training, and exercises are detailed in Chapter.

According to a new congressionally mandated report from the Institute of Medicine and National Research Council of the National Academies, to successfully develop these drugs, vaccines, and other medical interventions against biowarfare agents, Congress should authorize the creation of a new agency within the Office of the Secretary of the U.S. Department of Defense. The committee recommended that Congress should improve liability protections for those who develop and manufacture these products, to stimulate willingness to invest in new research and development for biowarfare protection. Giving Full Measure to Countermeasures also identifies other challengesâ€"such as the need for appropriate animal models and laboratories equipped with high-level biosafety protectionsâ€"that will require attention if DoD efforts to develop new medical countermeasures are to be successful.

Book excerpt: Due to the thermal stress induced by intensely hot environments, military operations conducted in these environments are difficult and demanding. The extreme thermal conditions found in deserts, for example, force soldiers to drastically alter their modes of operation. Here, heat stress and dehydration become real threats to the ability of troops to operate. The US Army's medical community, through the Program Office for Physiological and Psychological Effects of the NBC Environment and Sustained Operation on Systems in Combat has pursued various avenues in an effort to determine the extent of the Army's thermal stress problem and to develop ways of minimizing that problem. A recent attempt was the conduct of a Detailed Equipment Decontamination (DED) test, in which soldiers decontaminated chemically dirtied vehicles according to procedures outlined in Technical Manual FM 3-5. At the test's beginning, subjects ingested radio pills which tracked biomedical parameters -- core temperatures, skin temperatures, and pulse rates. The purpose of the test was to evaluate, under realistic conditions, the DED operation as a complete system. Eleven issues were addressed; these involved the interactions and effects among the human element, doctrine and eguipment. The issue of relevance to this analysis is thermal stress as it relates to a DED operation.
