The field has well-intended goals emphasizing collaboration and dialogue between population health, public health, clinical care, and other disciplines to address socioeconomic determinants of health, and employing interdisciplinary approaches to address health inequities wherever they exist. Despite these lofty ideals, there are concerns that the field itself has historically served to reinforce rather than deconstruct colonialism and power imbalances. At this point in time, the field has evolved toward a vision of a community of practice between institutions across the income spectrum (HIC, LMIC, LIC) working in bidirectional and multidirectional ways to develop staff, stuff, space, systems and strategies to eliminate health disparities. However, with deeply rooted colonial assumptions, racism, elitism, and other forms of bias underlying institutions and individuals, initiatives operating under the auspices of Global Health are all too often antithetical to the pursuit itself.

However, during the 1990s several challenges emerged in relation to levels of inequality and environmental degradation. This evaluative report on the relative effectiveness of World Bank assistance to China focuses on the period 1993-2002, although it includes some references to developments in earlier and more recent years also. Findings include that the World Bank has made important contributions to economic reform, poverty reduction, infrastructure development, and environmental protection, but has fallen short of its objectives in promoting fiscal and financial reforms to reduce inequality and risk.

Available in PDF, EPUB and Kindle. Book excerpt: During fiscal 2003-05, World Bank lending and administrative budgets to fragile states amounted to $4.1 billion and $161 million, respectively. This report assesses the effectiveness of this Bank support. The report finds that the Bank and the donor community have improved their operational readiness to engage with fragile states, and made substantial progress on donor coordination at the international policy level. Significant challenges remain, however. Donor agendas have been overly ambitious and need to be made more selective, the effectiveness of donor programs needs to be improved after the immediate post-conflict phase in war-ravaged countries when structural change is needed, and donors need to develop transparent aid allocation criteria that ensure that fragile states will be neither under- nor over-aided. The report makes recommendations to overcome these challenges and distills lessons for the Bank and other donors.
