State and federal policies for inpatient treatment and reimbursement have followed similar trends. As a result, mental health planners have attempted to develop programs to deal with this trend of restricted inpatient care shifting treatment of children and adolescents to home and community settings. More recently, the heuristic value of behavioral methods with substance abuse, pediatric pain, and child maltreatment has been documented as well. Because of the multifaceted and complex challenges posed by children and adoles cents who reach inpatient units, behavioral clinicians have worked close ly with other professionals (e. g. , psychiatrists, social workers, psychi atric nurses) in an effort to provide broad-spectrum, comprehensive intervention. However, as a burgeoning body of clinical research attests, the significant contribution of behavior therapy to successful treatment outcome is now indisputable. Yet, surprisingly few texts have been published concerning inpa tient treatment of children and adolescents. The few extant efforts in this area have not covered the broad array of issues and diagnostic popula tions treated, nor have they considered the pragmatic day-to-day diffi culties encountered by behaviorally oriented practitioners working in child and adolescent inpatient settings. This text is an attempt to remedy such shortcomings by providing a comprehensive yet practical approach to the functions of behavior therapists on inpatient psychiatric services for children and youth.

The most recent data indicate that more than 25% of children receiving mental health services in 1997 were treated in this type of setting (United States Department of Mental Health - Substance Abuse and Mental Health Services Administration, 2002). While there is a paucity of research investigating the efficacy of milieu treatment programs, the data reviewed indicate favorable results. Literature on milieu treatment with children and adolescents, however, dates back approximately 60 years. Cotton (1993), Gunderson (1978), and Miskimins (1990), provide conceptual frameworks for structuring a milieu and for implementing therapeutic practices of milieu treatment. In addition to providing the architecture for obtaining desirable treatment outcomes, each of the authors highlights milieu staff members' roles and suggests ideal characteristics of the staff. Moreover, these authors and several others assert that direct care staff members should be well trained, although none of the works specifies a training protocol for direct care personnel. The present work consists of the development of a training manual for direct care staff that can be applied to child or adolescent populations being treated in inpatient psychiatric hospitals, residential treatment programs, or clinical day treatment programs. The training manual is developed from an integration of both milieu treatment literature and the literature supporting three growing trends in children's mental health care. These include: the use of strength-based interventions, reduction in the use of restraint/seclusion (R/S), and the implementation of trauma-sensitive treatment. The training is designed to be implemented by a mental health professional who serves as the director of clinical services within the group care facility. The training also includes strategies for the administrator to facilitate the transfer of learning into daily practice. The implications and limitations of this training, as well as areas of future research are discussed.
