Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

This report focuses on two forces—citizen engagement and transparency—that hold the key to solving government failures by shaping how political markets function. Citizens are not only queueing at voting booths, but are also taking to the streets and using diverse media to pressure, sanction and select the leaders who wield power within government, including by entering as contenders for leadership. This political engagement can function in highly nuanced ways within the same formal institutional context and across the political spectrum, from autocracies to democracies. Unhealthy political engagement, when leaders are selected and sanctioned on the basis of their provision of private benefits rather than public goods, gives rise to government failures. The solutions to these failures lie in fostering healthy political engagement within any institutional context, and not in circumventing or suppressing it. Transparency, which is citizen access to publicly available information about the actions of those in government, and the consequences of these actions, can play a crucial role by nourishing political engagement.

The experience of parenting also impacts parents themselves. Until now, the majority of research has focussed on the most common histotype, high-grade serous ovarian carcinoma (HGSOC), which accounts for 70% of cases. The less common histotypes – including endometrioid, clear cell, mucinous, low-grade serous, carcinosarcoma, and non-epithelial histotypes – have received far less research attention. Accordingly, while major advances in our understanding of HGSOC have led to molecularly-directed therapies that improve patient outcomes, progress in less common histotypes has lagged behind. Notably, many of the uncommon histotypes demonstrate resistance to conventional chemotherapy regimens, and their inherent biological differences suggest most are unlikely to respond to emerging molecular therapeutics designed to target the biology of common ovarian cancer types.
