Book excerpt: On August 6, 1997, about 0142:26 Guam local time, Korean Air flight 801, a Boeing 747-3B5B (747-300), Korean registration 11L7468, operated by Korean Air Company, Ltd., crashed at Nimitz Hill, Guam. Flight 801 departed from Kimpo International Airport, Seoul, Korea, with 2 pilots, 1 flight engineer, 14 flight attendants, and 237 passengers on board. The airplane had been cleared to land on runway 6 Left at A.B. Won Guam International Airport, Agana, Guam, and crashed into high terrain about 3 miles southwest of the airport. Of the 254 persons on board, 228 were killed, and 23 passengers and 3 flight attendants survived the accident with serious injuries. The airplane was destroyed by impact forces and a postcrash fire. Flight 801 was operating in U.S. airspace as a regularly scheduled international passenger service flight under the Convention on International Civil Aviation and the provisions of 14 Code of Federal Regulations Part 129 and was on an instrument flight rules flight plan. The National Transportation Safety Board determines that the probable cause of the Korean Air flight 801 accident was the captain's failure to adequately brief and execute the nonprecision approach and the first officer's and flight engineer's failure to effectively monitor and cross-check the captain's execution of the approach. Contributing to these failures were the captain's fatigue and Korean Air's inadequate flight crew training. Contributing to the accident was the Federal Aviation Administration's (FAA) intentional inhibition of the minimum safe altitude warning system (MSAW) at Guam and the agency's failure to adequately manage the system. The safety issues in this report focus on flight crew performance, approach procedures, and pilot training; air traffic control, including controller performance and the intentional inhibition of the MSAW system at Guam; emergency response; the adequacy of Korean Civil Aviation Bureau (KCAB) and FAA over.
Book Synopsis Principles of Flight Simulation by : David Allerton
Download or read book Principles of Flight Simulation written by David Allerton and published by John Wiley & Sons. This book was released on 2009-10-27 with total page 492 pages. Available in PDF, EPUB and Kindle. Book excerpt: Principles of Flight Simulation is a comprehensive guide to flight simulator design, covering the modelling, algorithms and software which underpin flight simulation. The book covers the mathematical modelling and software which underpin flight simulation. The detailed equations of motion used to model aircraft dynamics are developed and then applied to the simulation of flight control systems and navigation systems. Real-time computer graphics algorithms are developed to implement aircraft displays and visual systems, covering OpenGL and OpenSceneGraph. The book also covers techniques used in motion platform development, the design of instructor stations and validation and qualification of simulator systems. An exceptional feature of Principles of Flight Simulation is access to a complete suite of software (www.wiley.com/go/allerton) to enable experienced engineers to develop their own flight simulator – something that should be well within the capability of many university engineering departments and research organisations. Based on C code modules from an actual flight simulator developed by the author, along with lecture material from lecture series given by the author at Cranfield University and the University of Sheffield Brings together mathematical modeling, computer graphics, real-time software, flight control systems, avionics and simulator validation into one of the faster growing application areas in engineering Features full colour plates of images and photographs. Principles of Flight Simulation will appeal to senior and postgraduate students of system dynamics, flight control systems, avionics and computer graphics, as well as engineers in related disciplines covering mechanical, electrical and computer systems engineering needing to develop simulation facilities.

Book Synopsis On-board Intellectual Aircraft Crew Support Systems by : Mikhail Krasilshchikov
Download or read book On-board Intellectual Aircraft Crew Support Systems written by Mikhail Krasilshchikov and published by Cambridge Scholars Publishing. This book was released on 2020-01-28 with total page 167 pages. Available in PDF, EPUB and Kindle. Book excerpt: The human factor remains one of the most important problems affecting flight safety. On the other hand, however, the complexity of onboard control and navigational systems grows constantly. As such, a great deal of research has gone into creating onboard systems which would interact with pilots and assist them in order to avoid flight accidents. This book provides unique insights into the building of such a system, using the so-called confidential approach for the analysis and synthesis of the dynamical systems, operating under the influence of various uncontrollable factors. As a result, the book details an intellectual expert system able to support the crew during standard flight modes. This system is able to detect a hazard in real-time mode, identify its source, and provide recommendations for the pilots. The book also investigates the confidence model, offering formulas and examples of expert systems’ functioning.
Book Synopsis AIR CRASH INVESTIGATIONS LACK OF EXPERIENCE The Crash of a Maryland State Police Helicopter by : Alistair Fitzgerald, Editor
Download or read book AIR CRASH INVESTIGATIONS LACK OF EXPERIENCE The Crash of a Maryland State Police Helicopter written by Alistair Fitzgerald, Editor and published by Lulu.com. This book was released on 2012-12-01 with total page 154 pages. Available in PDF, EPUB and Kindle. Book excerpt: On September 27, 2008, about 2358 eastern daylight time, an Aerospatiale Helicopter (Eurocopter) operated by the Maryland State Police (MSP) encountered instrument meteorological conditions was diverted to Andrews Air Force Base (ADW), Camp Springs, Maryland. About 3.2 miles north of the runway 19R threshold at ADW, during an instrument landing system approach, the helicopter impacted terrain and crashed. The pilot, one flight paramedic, one field provider, and one of two automobile accident patients being transported were killed. The helicopter was substantially damaged when it collided with trees and terrain in Walker Mill Regional Park, District Heights, Maryland. The National Transportation Safety Board determined that the probable cause of this accident was the pilot's lack of experience and lacking support of supporting institutions.

Available in PDF, EPUB and Kindle. Book excerpt: Why do people fear air travel, but text while driving? How were the travesties at the Abu Ghraib prison like a nuclear meltdown? What is the best way to throw a rocket at a robot? These are just a few questions addressed by the field of human factors psychology. These scientists use knowledge of how people think and why they act to improve the design of our world. In All Too Human, Dr. Anne McLaughlin introduces the field with vivid and topical stories that hinge on cognitive processes such as attention, memory, and decision-making. From the COVID-19 pandemic, to abandoned SCUBA divers, conspiracy theories, and the travails of online dating, McLaughlin draws on a century of research into the human mind to explain our past and predict our future.

Book Synopsis AIR CRASH INVESTIGATIONS: DEADLY MISTAKES The Crash of Air China Flight 129 by : George Cramoisi, editor
Download or read book AIR CRASH INVESTIGATIONS: DEADLY MISTAKES The Crash of Air China Flight 129 written by George Cramoisi, editor and published by Lulu.com. This book was released on 2012-04 with total page 330 pages. Available in PDF, EPUB and Kindle. Book excerpt: On April 15, 2002, Air China flight 129, a Boeing 767-200ER, operated by Air China, en route from Beijing, China to Busan, Korea, crashed on Mt. Dotdae, near Gimhae Airport, Busan. Of the 166 persons on board, 37 persons survived the crash, while 129 occupants were killed. The Korean Aviation Accident Investigation Board (KAAIB) determined that the probable cause of the crash was pilot error due to poor crew resource management and lost situational awareness during the circling approach of the runway. The Chinese investigation team pointed out that the Korean ATC was not fully licensed and mistakenly directed the airliner to descend to a wrong altitude and that the airport did not inform the crew of the weather conditions at the time. A contributing factor was that the airline made all announcements in Chinese and English, while most passengers were Korean.
