This authoritative resource applies lessons learned in past and present conflicts to ill and injured pediatric casualties. Topics include critical care, surgical treatment, and general medical management of acute and chronic conditions. Individual chapters address concerns such as anesthesia and intraoperative resuscitation, mechanical ventilation, aeromedical evacuation, thoracic surgery, surgery of the abdominal wall and diaphragm, fluid management, status epilepticus, care of the newborn, and emergency nutrition. Written and compiled by experts in the field of pediatric trauma, this book is constructed in an easy-to-read bullet format for quick reference, and includes up-to-the moment treatment recommendations for ill or injured children in theaters of war and austere environments. Military medical providers, civilian medical providers (pediatricians, pediatric surgeons, nursing staff, etc.) and first responders performing emergency medicine to children resulting from traumatic and hostile environments may find this pragmatic resource helpful in diagnosing, assessing care, and treating many of the most common medical and surgical conditions of childhood.

Book excerpt: "Pediatrician Soldier" tells COL (Dr.) Garbarino's amazing true story of how he left the safety and security of a successful medical practice to enter a combat zone. Experience a different side of war-the human side-with this deeply emotional first hand account of this pediatrician's overseas deployment for Operation Iraqi Freedom. View the war from the perspective of someone who is there to take care of our soldiers. "Pediatrician Soldier" is an easy-to-read collection of essays based on Dr. Garbarino's diaries and e-mails while in Iraq and Kuwait. Some of the stories are humorous, some are inspiring; throughout, Dr. Garbarino provides a blunt assessment of what has worked what has failed to meet the needs of our soldiers. Compassionate and powerful, this book reveals this childrens' physician's incredible dedication and devotion to caring for our troops, who he describes as just 'big kids', using humor, empathy and love. In addition, there are essays from others who have been affected by the war who have served in Iraq and those from home. Each offers the author's own experiences and persective on the Iraq war. Get to know two soldiers - one fallen, Captain John J. McKenna killed by sniper fire while leading his soldiers during a mission - and - one forever traumatized, Sgt. Daniel Tallouzi who because of a small piece of shrapnel from a mortar attack returned in an "eyes open" coma. Feel for those family members living each day, never knowing if their loved ones will return in a box, disabled or forever changed. COL Garbarino , the soldier, has seen how war's devastation has brought death and destruction to all in its path. His book allows insight into his two OIF tours as seen through his eyse, written with deep passion and revealing a human face to war. Dr. Charlie, the pediatrician, is looking into the future, to see if there will be a world for tomorrow's children.

Available in PDF, EPUB and Kindle. Book excerpt: "Medical Professionals and Their Work" conveys how medical people shape and organize the knowledge, perception, and experience of illness, as well as the substance of illness behavior, its management, and treatment. It is now well established that the unique symbolic equipment of the human animal is intimately connected with the functioning of the body. Freidson and Lorber believe that the proper understanding of specifically human rather than generally "animal" illness requires careful and systematic study of the social meanings surrounding illness. The content of social meanings varies from culture to culture and from one historical period to another. As important as the content of those social meanings, is the organization of groups who serve as carriers and, sometimes, creators. In the case of illness, a critical difference exists between those considered to be competent to diagnose and treat the sick and those excluded from this special privilege - a separation as old as the shaman or medicine-man. Such differences become solidified when the expert healer becomes a member of an organized, full-time occupation, sustained in monopoly over the work of diagnosis and treatment by the force of the state, and invested with the authority to make official designation of the social meanings to be ascribed to physical states. The medical profession in advanced nations is in a vise between professional needs and political demands. Its organization and its knowledge establish many of the conditions for being recognizably and legitimately ill, and the professional controls many of the circumstances of treatment. It thus plays a central role in shaping the experience of being ill. With this fact of modern life in mind, this collection on the character of experts or professionals in general and of medicine as a profession in particular is uniquely fashioned.

Book excerpt: For the first time, Pediatric Clinics is devoting one issue to two clinically focused topics: Pediatric Palliative Care and Pediatric Hospital Medicine. Dr. Ottolini has organized her section to focus on a variety of issues of relevant to all pediatricians, but which pose special challenge to the Pediatric Hospitalists. As pediatric care has advanced, children who would not have survived infancy are growing into young adults with complex chronic diseases and dependence upon technology. They frequently require hospitalization to address exacerbation of underlying disease processes and procedures to improve their quality of life. The articles are devoted to patient care challenges of troubleshooting malfunctioning technology, co-managing medically complex patients pre and post-op with surgical colleagues, and the Hospitalist's evolving role in performing procedures and sedation in this population of vulnerable patients. Also discussed rare strategies to maximize communication with parents, patients and primary care providers during hospitalization, especially for medically complex patients. Dr. Ullrich and Dr. Wolfe Pediatric have worked to bring relevant articles on palliative care to the pediatrician. While life-threatening conditions in childhood are rare, children with LTC account for a high proportion of pediatric hospital care, and about one half of such children die in the inpatient setting. The number of hospital-based pediatric palliative care programs has increased dramatically over the past decade to meet the palliative care needs of hospitalized children including symptom management, facilitation of communication, decision-making and advance care planning support, and coordination of care. Given these considerations, it is evident that the topics of pediatric palliative care and hospital medicine are fitting counterparts for this comprehensive issue.
