Book excerpt: The Community Living Assistance Services and Supports (CLASS) program would provide persons with functional limitations cash assistance to help them remain living in their communities. CLASS is financed solely by enrollee premiums, with no federal subsidy. The program is unsustainable and will add substantially to the budget deficit in the coming years. Without major program changes, CLASS will face a financial crisis that could lead to a financial bailout rivaling anything we have seen to date.

Book excerpt: Long-term care in the United States and other countries suffers multiple problems. Many people find it difficult to afford the high costs of services available and there is often inadequate care coordination, which compromises care quality, particularly amongst those eligible for multiple public programs. Recruitment and retention of a well-trained, stable workforce is also considered a challenge that needs to be addressed. The policy debate leading up to the Patient Protection and Affordable Care Act (ACA) drew attention to prevailing deficiencies in the way long-term care is delivered, regulated, and financed in the United States. This collection reviews what was accomplished by the legislation and what still remains to be done. Just how effective is the ACA likely to be in addressing the challenges plaguing the long-term care sector? Did it result in meaningful change or make little impact? This book answers these questions, drawing contributions from among the most eminent long-term care experts in the United States. This book was originally published as a special issue of the Journal of Aging & Social Policy.

Book excerpt: The Patient Protection and Affordable Care Act (PPACA, P.L. 111- 148, enacted March 23, 2010) established the Community Living Assistance Services and Supports (CLASS) program, a new federally administered voluntary insurance program to help adults age 18 and over with disabilities pay for long-term services and supports (LTSS). Added as a new title XXXII of the Public Health Service Act, the CLASS program is a departure from the way the federal government currently supports LTSS. The program is administered by the federal government [the Department of Health and Human Services (HHS)] but, unlike other federal LTSS programs, CLASS program benefits will be financed entirely by individuals' age-adjusted premiums. Individuals who are eligible for CLASS program benefits will receive cash payments to help them pay for services and supports they need to live in the community, or in a residential or institutional setting. This publication describes the major provisions of the CLASS Act legislation, including requirements for enrollment and eligibility, benefits and services, premium amounts, the CLASS Act trust fund, and timelines.

Available in PDF, EPUB and Kindle. Book excerpt: Individuals with disabilities, chronic conditions, and functional impairments need a range of services and supports to keep living independently. However, there often is not a strong link between medical care provided in the home and the necessary social services and supports for independent living. Home health agencies and others are rising to the challenges of meeting the needs and demands of these populations to stay at home by exploring alternative models of care and payment approaches, the best use of their workforces, and technologies that can enhance independent living. All of these challenges and opportunities lead to the consideration of how home health care fits into the future health care system overall. On September 30 and October 1, 2014, the Institute of Medicine and the National Research Council convened a public workshop on the future of home health care. The workshop brought together a spectrum of public and private stakeholders and thought leaders to improve understanding of the current role of Medicare home health care in supporting aging in place and in helping high-risk, chronically ill, and disabled Americans receive health care in their communities. Through presentations and discussion, participants explored the evolving role of Medicare home health care in caring for Americans in the future, including how to integrate Medicare home health care into new models for the delivery of care and the future health care marketplace. The workshop also considered the key policy reforms and investments in workforces, technologies, and research needed to leverage the value of home health care to support older Americans, and research priorities that can help clarify the value of home health care. This summary captures important points raised by the individual speakers and workshop participants.

Available in PDF, EPUB and Kindle. Book excerpt: Creating Aging-Friendly Communities (CAFC) examines the need to redesign America's communities to respond to the realities of our rapidly aging society. The text focuses on the interface between individuals and their environments, and the ways in which communities can enhance individual and community well-being. What differentiates CAFC from other books is its breadth of focus, its comprehensive and evidence-based consideration of key concepts, its inclusion of social as well as physical infrastructure characteristics, and its intensive examination of models of community change for fostering aging-friendliness. It presents a conceptually and empirically-based model of aging-friendliness, identifies environmental modifications that could enhance individual and community well-being, outlines a typology of community change approaches, and considers the potential efficacy of those approaches. This book identifies practical implications for policies, programs, and knowledge development designed to help communities become more aging-friendly.

Book excerpt: This first-of-its-kind textbook surveys rehabilitation and vocational programs aiding persons with disabilities in remote and developing areas in the U.S. and abroad. Contributors discuss longstanding challenges to these communities, most notably economic and environmental obstacles and ongoing barriers to service delivery, as well as their resilience and strengths. Intersections of health, social, structural, and access disparities are shown affecting rural disabled populations such as women, racial and sexual minorities, youth, and elders. In terms of responses, a comprehensive array of healthcare and health policy solutions and recommendations is critiqued with regard to health, employment, and service effectiveness outcomes. Included among the topics: Healthcare initiatives, strategies, and challenges for people with disabilities in rural, frontier, and territory settings. Challenges faced by veterans residing in rural communities. The Asia and Pacific region: rural-urban impact on disability. Challenges after natural disaster for rural residents with disabilities. Meeting the needs of rural adults with mental illness and dual diagnoses. Capacity building in rural communities through community-based collaborative partnerships. Disability and Vocational Rehabilitation in Rural Settings makes a worthy textbook for graduate students and upper-level undergraduates in the fields of social work, community and environmental psychology, public health, sociology, education, and geography. Its professional audience also includes vocational rehabilitation counselors serving these dynamic populations.

Available in PDF, EPUB and Kindle. Book excerpt: For patients and their loved ones, no care decisions are more profound than those made near the end of life. Unfortunately, the experience of dying in the United States is often characterized by fragmented care, inadequate treatment of distressing symptoms, frequent transitions among care settings, and enormous care responsibilities for families. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. Dying in America evaluates strategies to integrate care into a person- and family-centered, team-based framework, and makes recommendations to create a system that coordinates care and supports and respects the choices of patients and their families. The findings and recommendations of this report will address the needs of patients and their families and assist policy makers, clinicians and their educational and credentialing bodies, leaders of health care delivery and financing organizations, researchers, public and private funders, religious and community leaders, advocates of better care, journalists, and the public to provide the best care possible for people nearing the end of life.
