Book excerpt: This dissertation examines how Accountable Care Organizations [ACOs] are integrating with other types of providers such as social services, behavioral health and long term care organizations to address a full range of patient needs in order to achieve an improvement in the health outcomes of a population of patients. It is a study to determine how ACOs integrate with other providers and what the results are for patients. The Patient Protection and Affordable Care Act [PPACA] created a new model of health care delivery called the Accountable Care Organization. The ACO is designed to give doctors and hospitals incentives to become accountable for a population of patients and to re-design processes that provide for coordinated care, high quality and efficient service delivery. Personal behavior and social and environmental factors have great impact on overall health. ACOs which are made up of doctor and hospital organizations must look to coordinate and integrate with other types of providers in order to impact overall population health. This is difficult for a number of reasons including sharing financial incentives, communicating and sharing patient health information and overcoming the cultural managerial aspects of different organizations. ACO leaders describe how they have integrated with other providers and whether the results show an improvement for a population of patients served by their organization. This research includes a review of the literature concerning how ACOs are achieving integration with other providers and the results of the integration. The result is recommendations on the best practices for achieving provider integration based on evidence of improved patient health.

Book excerpt: Policy makers hope that the development of accountable care organizations (ACOs) -- organized groups of physicians, hospitals or other providers jointly accountable for caring for a defined patient population -- can improve health care quality and efficiency. An examination of existing provider efforts to improve care delivery illustrates that substantial financial and time investments are required to accomplish changes in care delivery, even among groups of providers affiliated with one another for many years, according to a new qualitative research study from the Center for Studying Health System Change (HSC). A common challenge for health care organizations' efforts to improve care -- similar to what the ACO concept seeks to encourage -- is implementing changes with minimal disruption to patients and productivity. To achieve improvements in care delivery, the seven provider organizations studied tapped existing financial reserves or external grant funding. They also sought strong physician and organizational leadership and encouraged transparency and flexibility when making changes. As payers develop guidelines to contract with ACOs, they will need to support providers' capability to develop and sustain improvements in care delivery, such as new health information technology and data reporting systems.

Book excerpt: This dissertation studies how performance risk-based (i.e., value-based) reimbursement over total costs of care in health insurance contracting affects how providers, specifically physician groups and health systems, determine marginal treatment choice and set the level of care provided to patients. Utilizing the widespread adoption of Accountable Care Organization (ACO) contracts by both commercial payers and Medicare, presented research explores changes in care delivery and health system organization when risks for total costs of care and satisfactory attainment of specific quality metrics (i.e. an ACO contract) are offered to providers. This dissertation proceeds in five parts. First, I review the substantial literatures related to the specific characteristics of ACO contracts in addition to the institutional details of such contracts themselves. Next, leveraging optimal procurement and auction theory, I present a theoretical foundation for considering ACOs as a form of incentive contract auction by Medicare and other insurers. This theoretical foundation motivates three principal empirical analyses of ACO contracts, each briefly explained in the preface, focused on changes in physician behavior following ACO contract adoption.

Improving social conditions remains critical to improving health outcomes, and integrating social care into health care delivery is more relevant than ever in the context of the pandemic and increased strains placed on the U.S. health care system. The report and its related products ultimately aim to help improve health and health equity, during COVID-19 and beyond. The consistent and compelling evidence on how social determinants shape health has led to a growing recognition throughout the health care sector that improving health and health equity is likely to depend â€" at least in part â€" on mitigating adverse social determinants. This recognition has been bolstered by a shift in the health care sector towards value-based payment, which incentivizes improved health outcomes for persons and populations rather than service delivery alone. The combined result of these changes has been a growing emphasis on health care systems addressing patients' social risk factors and social needs with the aim of improving health outcomes. This may involve health care systems linking individual patients with government and community social services, but important questions need to be answered about when and how health care systems should integrate social care into their practices and what kinds of infrastructure are required to facilitate such activities.
