Book excerpt: The Social Security Administration (SSA) administers two programs that provide benefits based on disability: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. This report analyzes health care utilizations as they relate to impairment severity and SSA's definition of disability. Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.

Book excerpt: This book provides a scholarly discussion of arguably the most important advance in U.S. public health services since Medicare 50 years ago - how the Federal program known as the Patient Care and Affordable Care Act of 2010 (ACA) or "Obamacare" became law. It addresses ACA in terms of its impact on improving health and behavioural health services for key diverse populations in America, including people with disabilities, consumers, women, racial and ethnic minorities, and veterans and their families. From the very beginning, ACA was controversial and the topic of heated political debate at both state and national levels. This book examines more closely how the legislation was developed, including the political history of the act; the many advocacy efforts at the national level and the community-based action strategies at the grassroots level; how ACA will affect a broad cross-section of America; the integration of health and behavioural health services as a key component of ACA; the financing of ACA and parity for behavioural health services. This book was originally published as a special issue of the Journal of Social Work in Disability & Rehabilitation.

Book excerpt: This comprehensive book examines the organization, financing, delivery, and outcomes of behavioral health (i.e., alcohol, drug abuse, and mental health) services from both U.S. and global perspectives. Addressing the need for more integrative and collaborative approaches in public health and behavioral health initiatives, the book covers the fundamental issues in behavioral health, including epidemiology, insurance and financing, health inequities, implementation sciences, lifespan issues, cultural responsiveness, and policy. Featuring insightful research from scholars in an interdisciplinary range of academic and professional fields, chapters fall into three distinct sections: Overview: Outlines the defining characteristics of behavioral health services and identifies significant challenges in the field At-Risk Populations: Explores critical issues for at-risk populations in need of behavioral health services, including children in school environments, youth in juvenile justice systems, and persons with developmental disabilities, among others Services Delivery: Presents a rationale for greater integration of health and behavioral health services, and contextualizes this explanation within global trends in behavioral health policy, systems, and services An in-depth textbook for graduate students studying public health, behavioral health, social work policy, and medical sociology, as well as a useful reference for behavioral health professionals and policy makers, Foundations of Behavioral Health provides a global perspective for practice and policy in behavioral health. It promotes better understanding of the importance of integrating population health and behavioral health services, with an eye towards improving and sustaining public health and behavioral health from national, regional, and global perspectives.

Book excerpt: The past half-century has been marked by major changes in the treatment of mental illness: important advances in understanding mental illnesses, increases in spending on mental health care and support of people with mental illnesses, and the availability of new medications that are easier for the patient to tolerate. Although these changes have made things better for those who have mental illness, they are not quite enough. In Better But Not Well, Richard G. Frank and Sherry A. Glied examine the well-being of people with mental illness in the United States over the past fifty years, addressing issues such as economics, treatment, standards of living, rights, and stigma. Marshaling a range of new empirical evidence, they first argue that people with mental illness—severe and persistent disorders as well as less serious mental health conditions—are faring better today than in the past. Improvements have come about for unheralded and unexpected reasons. Rather than being a result of more effective mental health treatments, progress has come from the growth of private health insurance and of mainstream social programs—such as Medicaid, Supplemental Security Income, housing vouchers, and food stamps—and the development of new treatments that are easier for patients to tolerate and for physicians to manage. The authors remind us that, despite the progress that has been made, this disadvantaged group remains worse off than most others in society. The "mainstreaming" of persons with mental illness has left a policy void, where governmental institutions responsible for meeting the needs of mental health patients lack resources and programmatic authority. To fill this void, Frank and Glied suggest that institutional resources be applied systematically and routinely to examine and address how federal and state programs affect the well-being of people with mental illness.

Available in PDF, EPUB and Kindle. Book excerpt: A major reappraisal of crime and punishment in America The huge prison buildup of the past four decades has few defenders, yet reforms to reduce the numbers of those incarcerated have been remarkably modest. Meanwhile, an ever-widening carceral state has sprouted in the shadows, extending its reach far beyond the prison gate. It sunders families and communities and reworks conceptions of democracy, rights, and citizenship—posing a formidable political and social challenge. In Caught, Marie Gottschalk examines why the carceral state remains so tenacious in the United States. She analyzes the shortcomings of the two dominant penal reform strategies—one focused on addressing racial disparities, the other on seeking bipartisan, race-neutral solutions centered on reentry, justice reinvestment, and reducing recidivism. With a new preface evaluating the effectiveness of recent proposals to reform mass incarceration, Caught offers a bracing appraisal of the politics of penal reform.

Available in PDF, EPUB and Kindle. Book excerpt: This book examines the structural dynamics of HIV among populations at heightened vulnerability to infection as the result of stigma, discrimination and marginalization. It first examines how the socio-structural context shapes HIV risk and how affected populations and national governments and programs have responded to these structural constraints. Chapters focus on structural determinants of HIV risk among transgender women in Guatemala, migrant workers in Mexico, Nigeria and Vietnam, and people who inject drugs in Tanzania. Next, the book examines resilience and community empowerment and mobilization among key populations such as female sex workers in the Dominican Republic and India, and young women and girls in Botswana, Malawi and Mozambique. A third set of chapters explores how national responses to HIV have addressed the role of structural factors in diverse political, geographic and epidemic settings including: Brazil, South Africa, Ukraine and the USA. Ultimately, effective and sustainable responses to HIV among marginalized groups must be grounded in an in-depth understanding of the factors that create vulnerability and risk and impede access to services. Throughout, this book brings together a rigorous social science research perspective with a strong rights-based approach to inform improvements in HIV programs and policies. It offers new insights into how to better address HIV and the health and human rights of historically excluded communities and groups.
