Book excerpt: The study was designed to evaluate the effects of self-administered neuromuscular electrical stimulation (NMES) on changes in abdominal muscle strength and endurance and core strength. Fifty-three adults were randomly assigned into high intensity (HI: n=27) or low intensity (LI: n=26) groups. The NMES device for the LI group had been altered so that subjects felt some tactile sensation, but the intensity was not sufficient to elicit a muscular contraction. All subjects stimulated their abdominals 5 days per week (30 minutes per session) for 6 weeks and refrained from engaging in any additional abdominal exercises during the study. Subjects were tested at baseline, 2, 4, and 6 weeks. The HI group had a significantly greater increase in strength at 4 weeks (19%) and 6 weeks (29%) compared to the LI group. The HI group completed more curl-ups than the LI group at 2 weeks (62%) and 4 weeks (118%). Both groups had a significant increase in core strength over the course of the study, with no difference between groups. Results of the current study indicate that high intensity NMES can significantly increase abdominal strength and endurance compared to LI intensity (control) stimulation.

As people age, their muscles progressively weaken, often leading to physical disabilities and functional limitations later in life. Eventually, muscle weakness can interfere with a person's ability to complete everyday activities, such as bathing, walking a few blocks down the road, or safely getting up from a chair. For elderly people, maintaining or improving muscle strength helps to maintain the ability to safely complete daily tasks. This ultimately leads to decreased healthcare costs and sustained independence. There are many well-established methods to improve muscle function in elderly people, including endurance exercise, strength training, and flexibility training. All of these modalities of exercise work through a variety of mechanisms to induce specific training effects that enhance muscle function. However, there are other forms of exercise that may provide unique benefits to muscle function that have been less extensively studied; chiefly, sprint exercise. Sprint exercise has been shown to promote an adaptation that has yet to be seen in response to any other form of exercise: improved calcium release from the sarcoplasmic reticulum of skeletal muscle. This adaptation is significant because calcium release within the muscle is an essential step in the process of muscle contraction. Surprisingly, however, sprint exercise is not even mentioned by the American College of Sports Medicine in its recommendations for older individuals. This is likely due to concerns that it would not be well-tolerated due to risks related to high intensity exercise and the stress that it places on the joints. Consequently, it may be of great benefit to develop a physical therapy intervention that could mimic the effects of sprint training for people that cannot safely execute sprint exercise. Such an intervention could be especially beneficial for older individuals, as traditional sprint exercise is seldom recommended. Russ and colleagues recently developed a treatment protocol that induces muscular adaptations in young people that are similar to those caused by sprint exercise. This protocol is based on an established physical therapy intervention known as neuromuscular electrical stimulation (NMES), which works by sending electric current through electrodes to stimulate nerves and activate specific muscles. NMES has been used in both laboratories and physical therapy clinics for decades and is commonly used as a treatment to improve muscle strength or endurance. The protocol developed by Russ et al., hereafter referred to as T.R.A.N.S.I.T. (Therapeutic Rapid Activation of the Neuromuscular System for Interval Training), applies NMES technology in a new way that improves peak rate of calcium release within the muscle (an essential step in muscle activation). This is of particular importance because improved calcium release has only been observed in response to sprint exercise training. Because calcium release declines in aging muscle, we expect that T.R.A.N.S.I.T. will improve muscle function more in elderly people than in young people. Therefore, for my thesis project, I proposed to test the effect of T.R.A.N.S.I.T. on muscle quality (muscle force / muscle size) in older individuals. Due to specific changes in muscles that occur with age, we expected that T.R.A.N.S.I.T. would be of even greater benefit to older individuals than it was to younger individuals.

Book excerpt: This study sought out to test the effectiveness of using electrical muscle stimulation (EMS) on increasing strength and endurance of the gluteus muscles compared to volitional exercise. Seventy-two female volunteers between the ages of 20 and 60 years old were assigned to either an EMS (n=24), volitional exercise (n=24), or control group (n=24). The EMS group underwent stimulation 5 days per week for 30 minutes each day at the highest tolerable level for 6 weeks. The volitional exercise group completed 3 sets of 10 quadruped hip extensions on each leg, 5 times per week, for 6 weeks. Subjects were tested at the beginning, mid-point, and end of the study. Isometric strength of the gluteal muscles was tested using a Chatillon CSD 200 Dynamometer; endurance was measured using a hip bridge movement. The stimulation group had a 15% increase in gluteal strength compared to only a 9% increase of gluteal strength in the volitional exercise group. Gluteal endurance increased by 29% in the stimulation group compared to 26% in the volitional exercise group. The findings of this study show that there were no significant differences in gluteal strength or endurance between groups.

Book excerpt: Neuromuscular electrical stimulation (NMES) has been shown to be effective for recovery of motor function following injury or pathology, however, NMES can impart rapid fatigue and the specific parameters of stimulation that maximize force output and delay the onset of fatigue remain unclear. Frequency, intensity, and pulse pattern are a few of the parameters that can be manipulated to achieve desired outcomes. Strong evidence supports the use of higher frequencies of stimulation to maximize performance of fatigued or paralyzed muscle. Likewise, several studies advocate the use of varied pulse patterns, rather than constant pulses, to maximize force output as well. Much of the research literature regarding NMES use with a pathological population involves spinal injured individuals. Much less is known about the effect of NMES on motor recovery, especially when delivered through varied pulse patterns, in the post-stroke population. The three studies contained herein addressed these issues. In Study 1, submaximal and supramaximal intensities of two variable frequency stimulation patterns and one constant frequency pattern were applied to the thenar muscles of a young healthy population. Results showed marked differences in force output between the two intensities. Submaximal stimulation enhanced the effect of the variable pulses and a greater force response was produced. In Study 2, the same three patterns were applied to the thenar muscles of a post-stroke group and an age-matched able-bodied group at submaximal intensities. Again, differences in force output were evident between the healthy and pathological group, and the variable pulses may have further depressed already weakened hemiplegic muscle. Study 3 compared the effects of a NMES rehabilitation program using a high (40 Hz) and a low (20 Hz) frequency to determine if task-specific improvements were related to frequency used. Results showed that those in the high frequency condition demonstrated greater improvements in strength, dexterity and force accuracy; those in the low frequency condition showed improvements in motor endurance. The results of this work suggest that the intensity, frequency and stimulation pattern of NMES used have a significant impact on the resultant muscle contraction and functional skills gained following stroke and should be carefully considered when implementing a clinical regimen for motor recovery.

Available in PDF, EPUB and Kindle. Book excerpt: In many cases of neuromuscular disorders the physician is faced with a complete lack of therapeutic approaches. This helplessness places the doctor in a position of conflict between his desire to help and his awareness that there is no treatment. In this situation it is tempting to indiscriminately use any procedure that avoids an admission of medical helplessness while satisfying the patient's demand for treatment. Electrical interventions are often used to avoid this situation. Due to the random use of therapeutic approaches it is not known what really happens. Presumptions and biased empirical observations have led to the exten sive use of different forms of electrical stimulation regimes in neuromuscular diseases. Due to this unsatisfactory situation it is necessary to know more about appropriate methods that are being used in particular disorders. The search for a better understanding of nerve-muscle interaction has shown that certain activity patterns can influence muscle. These experi mental results provide a rational basis for a possible therapeutic use of electrical stimulation of nerve and muscle. Previously most research has been conducted in normal tissue, and little is known regarding the re sponses of diseased muscle. In an interdisciplinary approach to this, it is our intention to present the current knowledge about basic principles of electrical stimulation in normal muscle. Before electrical stimulation can be accepted as a therapeutic tool, we felt it necessary to summarize the effects of activity in normal and diseased muscle and nerve.
