Book excerpt: This study examined the effects of types of therapist disclosure and their interaction with various combinations of observer, therapist, and client gender-dyads on observer ratings of the working alliance. Participants were 357 undergraduate students (60.2% women) from two Midwestern universities who were randomly assigned to one of 12 conditions. Each condition required students to read one of 12 printed scenarios differentiated by all possible combinations of three types of therapist self-disclosure (similar, dissimilar, no disclosure), two levels of therapist gender, and two levels of client gender. Students rated the scenarios on the perceived working alliance between the therapist and the client, using the 36-item Working Alliance Inventory-Observer (WAI-O). A 2 (student sex) x 2 (therapist sex) x 2 (client sex) x 3 (disclosure type) ANOVA revealed no significant effects on the WAI-O total scale score. In addition, no main effects or interactions were found on WAI-O total scale when male and female student scores were pooled. A 2 x 2 x 2 x 3 MANOVA performed on the WAI-O subscales indicated female observers perceived a stronger client-therapist bond for similar than dissimilar disclosures with male clients. Female observers rated male clients with a stronger bond than female clients, but only in the similar disclosure condition. A main effect was also found for observer sex on the Task and Bond subscales. Although this study did not find gender of the observer, type of therapist disclosure, and the gender of the therapist and their client to influence overall working alliance ratings, results suggest that these factors have an impact on female observer ratings of the bond and task agreement between the therapist and their client. Specifically, two findings emerged: (a) women, not men, observed a stronger bond for male client recipients of similar versus dissimilar disclosure; (b) women, not men observed a stronger bond for male client versus female recipients of a therapist's similar disclosure. Results are discussed in terms of disclosure and gender research.

Book excerpt: This study examined the effects of counselor level of training and gender on counseling outcome in a university counseling center environment. Data was collected from an archival database of approximately 4500 clients seen over a six-and-a-half year time period at a mid-sized Southeastern university counseling center. The Outcome Questionnaire 45.2 was used to measure client outcome, which consists of 45-items scored on a five-point Likert scale. The OQ produces three subscale scores (Symptom Distress, Interpersonal Relations, and Social Role) and a Total Score. The Social Role subscale was found to have low reliability in this study, and was omitted from further consideration. Data on client and counselor gender were also included in this study. Two analyses were performed on the archival data. In the first analysis, 338 clients who completed an OQ at intake and after their fourth session were assigned to groups based on the training level of their assigned counselor (Senior Staff, Pre-Doctoral Intern, and Master's Extern). A series of repeated-measures and one-way ANOVAs were conducted. Total Scores and subscales decreased significantly for clients in all three groups between sessions. There were no significant differences on OQ scores between the groups at intake. However, Master's level clients showed a significantly greater decrease than Senior Staff or Pre-Doctoral clients on the Interpersonal Relations subscale at fourth session. A significant difference was also found for client gender on the IR subscale at both intake and fourth session, with males scoring higher than females. There were no client/counselor gender matching effects observed. In the second analysis, 2772 clients were assigned to groups based on whether or not they returned to counseling after their intake session (Returnees and Non-Returnees). A one-way MANOVA showed a small, but significant difference between the groups at intake, with Returnees showing slightly higher OQ scores than Non-Returnees. Follow-up ANOVAs showed this was due to a significant difference on the IR subscale. Implications for counseling center outcome assessment and treatment and directions for future research were discussed.

Book excerpt: Investigations into what makes counseling an effective mode of treatment for mental health concerns have pointed to the counseling relationship as the strongest determinant of success. Unfortunately, the counseling relationship has not been adequately studied. The purpose of this research was to explore counselors' perceptions and experiences of their relationships with clients. A qualitative research design was employed, using purposeful sampling for maximum variability. In this study, data was collected from three rounds of interviews with ten diverse counselors from the southeastern region of the United States. Using grounded theory methodology, the tentative theory that emerged from first round data analysis was deepened and refined in subsequent rounds of interviews, using questions generated by the data and designed to elaborate conceptualizations. Relationships between categories and properties were then illuminated by process coding techniques. The grounded theory that developed in this study described the counseling relationship as a dynamic process of negotiation between clients and counselors. Counselors perceived clients' disclosures and behaviors on an emotional level and communicated their understanding to clients, to elicit clients' commitment. At the same time, counselors interpreted their experiences of clients' behaviors, comparing their interpretations to their store of client conceptualizations developed from personal and professional experiences. Depending on their level of self-awareness, counselors also considered their own personal life experiences that could influence their interpretation of clients' behaviors. Counselors then used the information gained from this process of filtering their perceptions and experiences through their conceptualization of themselves and their clients to negotiate individualized treatment. Treatment negotiation included supportive and challenging interventions and reflected counselors' judgments of appropriate interventions and appropriate timing of interventions. Success was measured using depth of clients' disclosures and eventual increase in clients' comfort levels.
