Book excerpt: This study sought out to test the effectiveness of using electrical muscle stimulation (EMS) on increasing strength and endurance of the gluteus muscles compared to volitional exercise. Seventy-two female volunteers between the ages of 20 and 60 years old were assigned to either an EMS (n=24), volitional exercise (n=24), or control group (n=24). The EMS group underwent stimulation 5 days per week for 30 minutes each day at the highest tolerable level for 6 weeks. The volitional exercise group completed 3 sets of 10 quadruped hip extensions on each leg, 5 times per week, for 6 weeks. Subjects were tested at the beginning, mid-point, and end of the study. Isometric strength of the gluteal muscles was tested using a Chatillon CSD 200 Dynamometer; endurance was measured using a hip bridge movement. The stimulation group had a 15% increase in gluteal strength compared to only a 9% increase of gluteal strength in the volitional exercise group. Gluteal endurance increased by 29% in the stimulation group compared to 26% in the volitional exercise group. The findings of this study show that there were no significant differences in gluteal strength or endurance between groups.

Clinical presentation of GMed weakness during tasks like the single leg squat (SLS) is increased hip adduction, internal rotation, and knee valgus. It is essential to optimize the function of this muscle to prevent injury. Functional electrical stimulation (FES) is an emerging treatment to improve activation of impaired muscles. Objective: The purpose of this study was to examine the effectiveness of a strengthening intervention with or without FES on GMed function, when assessed by strength, muscle activation, and frontal plane kinematics. Design: Randomized Controlled Single-Blinded Controlled Laboratory Trial. Setting: Laboratory. Patients or Other Participants: 22 healthy adult females (Age: 21.8±1.4yrs, Mass: 76.9±18.8kg, Height: 1.7±.1m). Intervention(s): Participants were randomized to 2 resistance training groups. FES which administered a visible comfortable contraction of the GMed during therapeutic exercise or sham treatment with no stimulation during exercise. Protocol was composed of 4 exercises (3x10), 3 times a week for 2-weeks. Main Outcome Measure(s): All variables tested pre/post 2-week intervention. GMed torque was assessed via HHD (Nm/kg), normalized GMed activity during SLS (percentage MVIC), and frontal plane projection angle (FPPA) during a SLS. Cohens d effect sizes and 95% confidence intervals were calculated. Results: Groups were similar at baseline. Both groups demonstrated FPPA improvements (FES:Pre-16.13±8.63, Post-9.13±8.04,p=.006; Sham:Pre-16.28±5.97, Post-11.31±5.11,p=.012). No differences in hip abduction torque (FES:Pre-0.750.18,Post-0.760.18 Nm/kg; Sham:Pre-0.680.17,Post-0.750.15 Nm/kg,p=.148). Large effect size found in FES group for FPPA (d=.84[-.03,1.71]). Sham group demonstrated a large effect size for SLS EMG post-intervention (FES:d=.55[-.3,1.4];Sham:d=.93[.05,1.81]). Conclusions: Our findings imply that resistance training of 2-weeks with/without FES both elicited improvements in FPPA that exceeded the standard error of measure (3.2). Additionally, those in FES group had higher expectation of treatment thus solidifying use of FES in clinical practice.

Seventy-two females, ages 20 and 60 years, were randomly placed into one of three groups: control, exercise, or stimulation. The exercise group performed 3 sets of 10 quadruped hip extensions with each leg, 5 days per week, for 6 weeks. The stimulation group underwent stimulation for 30 minutes, 5 days per week, for 6 weeks using the Slendertone® Bottom Toner. Subjects were tested at baseline, 4 weeks, and 6 weeks. Muscle tone of the buttocks was measured using a Myotonometer®. Hip girth was measured using a spring-loaded tape measure and body satisfaction was determined using a series of questionnaires. Results were compared to a control group. There were no improvements in hip circumference or muscular tone. The exercise and stimulation group had significant improvements in score on both the Shape Evaluation and Body Satisfaction Questionnaires. When Overall Results questionnaires were summarized, 92% of subjects in the exercise group and 88% of subjects in the stimulation group felt that they had positive results following the 6-week study.

Book excerpt: The study was designed to evaluate the effects of self-administered neuromuscular electrical stimulation (NMES) on changes in abdominal muscle strength and endurance and core strength. Fifty-three adults were randomly assigned into high intensity (HI: n=27) or low intensity (LI: n=26) groups. The NMES device for the LI group had been altered so that subjects felt some tactile sensation, but the intensity was not sufficient to elicit a muscular contraction. All subjects stimulated their abdominals 5 days per week (30 minutes per session) for 6 weeks and refrained from engaging in any additional abdominal exercises during the study. Subjects were tested at baseline, 2, 4, and 6 weeks. The HI group had a significantly greater increase in strength at 4 weeks (19%) and 6 weeks (29%) compared to the LI group. The HI group completed more curl-ups than the LI group at 2 weeks (62%) and 4 weeks (118%). Both groups had a significant increase in core strength over the course of the study, with no difference between groups. Results of the current study indicate that high intensity NMES can significantly increase abdominal strength and endurance compared to LI intensity (control) stimulation.

As people age, their muscles progressively weaken, often leading to physical disabilities and functional limitations later in life. Eventually, muscle weakness can interfere with a person's ability to complete everyday activities, such as bathing, walking a few blocks down the road, or safely getting up from a chair. For elderly people, maintaining or improving muscle strength helps to maintain the ability to safely complete daily tasks. This ultimately leads to decreased healthcare costs and sustained independence. There are many well-established methods to improve muscle function in elderly people, including endurance exercise, strength training, and flexibility training. All of these modalities of exercise work through a variety of mechanisms to induce specific training effects that enhance muscle function. However, there are other forms of exercise that may provide unique benefits to muscle function that have been less extensively studied; chiefly, sprint exercise. Sprint exercise has been shown to promote an adaptation that has yet to be seen in response to any other form of exercise: improved calcium release from the sarcoplasmic reticulum of skeletal muscle. This adaptation is significant because calcium release within the muscle is an essential step in the process of muscle contraction. Surprisingly, however, sprint exercise is not even mentioned by the American College of Sports Medicine in its recommendations for older individuals. This is likely due to concerns that it would not be well-tolerated due to risks related to high intensity exercise and the stress that it places on the joints. Consequently, it may be of great benefit to develop a physical therapy intervention that could mimic the effects of sprint training for people that cannot safely execute sprint exercise. Such an intervention could be especially beneficial for older individuals, as traditional sprint exercise is seldom recommended. Russ and colleagues recently developed a treatment protocol that induces muscular adaptations in young people that are similar to those caused by sprint exercise. This protocol is based on an established physical therapy intervention known as neuromuscular electrical stimulation (NMES), which works by sending electric current through electrodes to stimulate nerves and activate specific muscles. NMES has been used in both laboratories and physical therapy clinics for decades and is commonly used as a treatment to improve muscle strength or endurance. The protocol developed by Russ et al., hereafter referred to as T.R.A.N.S.I.T. (Therapeutic Rapid Activation of the Neuromuscular System for Interval Training), applies NMES technology in a new way that improves peak rate of calcium release within the muscle (an essential step in muscle activation). This is of particular importance because improved calcium release has only been observed in response to sprint exercise training. Because calcium release declines in aging muscle, we expect that T.R.A.N.S.I.T. will improve muscle function more in elderly people than in young people. Therefore, for my thesis project, I proposed to test the effect of T.R.A.N.S.I.T. on muscle quality (muscle force / muscle size) in older individuals. Due to specific changes in muscles that occur with age, we expected that T.R.A.N.S.I.T. would be of even greater benefit to older individuals than it was to younger individuals.
