Book excerpt: Although clinical observations abound, substantiated evidence concerning the effect of electroshock treatment on learning capacity, particularly in depressed patients, is scanty and experimental findings have been conflicting. The experiments reported here were accordingly conducted as a pilot investiga tion, visual learning tests involving spatial relations being used to examine in depressed patients: - (a) The extent to which a single electroshock (ES) impairs learning capacity, and the amount of its restoration 30 min, four hours and twenty-eight hours after shock. (b) The effect of repeated shocks (EOT) on learning capacity. (c) The relationship between possibly influential factors and changes in learn ing performance. (d) The extent of personality change recorded by "expressive movement" scores after three shocks. Before the experiments, extensive testing was carried out to determine the most suitable testing times. Important points emerging from an initial experiment were re-investigated with a more appropriate test. Finally, certain scores of expres sive movement derived from a greatly improved scoring system were used to assess personality changes resulting from electroshock.

Book excerpt: Since the development of pharmacoconvulsive therapy in 1934 and of electroconvulsive therapy (ECT) in 1938, ECT has proven far more valuable than just the intervention of last resort. In comparison with psychotropic medications, we now know that ECT can act more effectively and more rapidly, with substantial clinical improvement that is often seen after only a few treatments. This is especially true for severely ill patients -- those with severe major depression with psychotic features, acute mania with psychotic features, or catatonia. For patients who are physically debilitated, elderly, or pregnant, ECT is also safer than psychotropic medications. The findings of the American Psychiatric Association (APA) Task Force on ECT were published by the APA in 1990 as the first edition of The Practice of Electroconvulsive Therapy, inaugurating the development of ECT guidelines by groups both within the United States and internationally. Since then, advances in the use of this technically demanding treatment prompted the APA to mandate a second edition. The updated format of this second edition presents background information followed by a summary of applicable recommendations for each chapter. This close integration of the recommendations with their justifications makes the material easy to read, understand, and use. To further enhance usability, recommendations critical to the safe, effective delivery of treatment are marked with the designation "should" to distinguish them from recommendations that are advisable but nonessential (with the designations "encouraged," "suggested," "considered"). The updated content of this second edition, which spans indication for use of ECT, patient evaluation, side effects, concurrent medications, consent procedures (with sample consent forms and patient information booklet), staffing, treatment administration, monitoring of outcome, management of patients following ECT, and documentation, as well as education, and clinical privileging. Book excerpt: Since 2003, when spontaneous activity in cortical slices was first found to follow scale-free statistical distributions in size and duration, increasing experimental evidences and theoretical models have been reported in the literature supporting the emergence of evidence of scale invariance in the cortex. Although strongly debated, such results refer to many different in vitro and in vivo preparations (awake monkeys, anesthetized rats and cats, in vitro slices and dissociated cultures), suggesting that power law distributions and scale free correlations are a very general and robust feature of cortical activity that has been conserved across species as specific substrate for information storage, transmission and processing. Equally important is that the features reminiscent of scale invariance and criticality are observed at scale spanning from the level of interacting arrays of neurons all the way up to correlations across the entire brain. Thus, if we accept that the brain operates near a critical point, little is known about the causes and/or consequences of a loss of criticality and its relation with brain diseases (e.g. epilepsy). The study of how pathogenetical mechanisms are related to the critical/non-critical behavior of neuronal networks would likely provide new insights into the cellular and synaptic determinants of the emergence of critical-like dynamics and structures in neural systems. At the same time, the relation between the impaired behavior and the disruption of criticality would help clarify its role in normal brain function. The main objective of this Research Topic is to investigate the emergence/disruption of the emergent critical-like states in healthy/impaired neural systems.
