Veterans who have CMI often have physical symptoms (such as fatigue, joint and muscle pain, and gastrointestinal symptoms) and cognitive symptoms (such as memory difficulties). For the purposes of this report, the committee defined CMI as the presence of a spectrum of chronic symptoms experienced for 6 months or longer in at least two of six categories-fatigue, mood, and cognition, musculoskeletal, gastrointestinal, respiratory, and neurologic-that may overlap with but are not fully captured by known syndromes (such as CFS, fibromyalgia, and IBS) or other diagnoses. Despite considerable efforts by researchers in the United States and elsewhere, there is no consensus among physicians, researchers, and others as to the cause of CMI. There is a growing belief that no specific causal factor or agent will be identified. Many thousands of Gulf War veterans1 who have CMI live with sometimes debilitating symptoms and seek an effective way to manage their symptoms. Estimates of the numbers of 1991 Gulf War veterans who have CMI range from 175,000 to 250,000 (about 25-35% of the 1991 Gulf War veteran population), and there is evidence that CMI in 1991 Gulf War veterans may not resolve over time. Preliminary data suggest that CMI is occurring in veterans of the Iraq and Afghanistan wars as well. In addition to summarizing the available scientific and medical literature regarding the best treatments for chronic multisymptom illness among Gulf War veterans, Gulf War and Health: Volume 9: Treatment for Chronic Multisymptom Illness recommends how best to disseminate this information throughout the VA to improve the care and benefits provided to veterans, recommends additional scientific studies and research initiatives to resolve areas of continuing scientific uncertainty and recommends such legislative or administrative action as the IOM deems appropriate in light of the results of its review.

Book excerpt: Thirty-seven chronic disease patients were randomized to either a biofeedback-assisted stress management (BFSM) experimental group or a usual care (UC) control condition. It was hypothesized that participants enrolled in the BFSM treatment group would demonstrate lower levels of norepinephrine and depression than those in the UC control condition. It was further hypothesized that training acquisition would modify the main effect of group assignment on depression and norepinephrine. The BFSM group demonstrated significantly lower levels of norepinephrine than the UC group. There were no main effects of BFSM on depression. The training acquisition X group assignment interaction was not significant. Results are discussed in terms of providing support for a common mechanism for the effects of BFSM on health outcomes in chronic diseases with ANS dysfunction, and the implications of assessing training acquisition in both clinical and research settings.

Book excerpt: This study was undertaken to evaluate the effectiveness of biofeedback and relaxation skills training to help alleviate college students' stress symptoms. Data was collected from 659 full-time college students who attended a total of 1,170 biofeedback sessions over the course of two years. Results of the study indicated that the top three stress-related symptoms students experienced were (a) feeling overwhelmed, (b) feeling anxious, and (c) difficulty concentrating. Furthermore, the top three stressors rated by students were (a) too much to do, (b) homework, and (c) classes and school. In addition, the top three coping strategies utilized by students to manage their stress were (a) prayer, (b) exercise, and (c) talking to friends. Moreover, results also showed significant differences on all three pre and post measures regarding skin temperature readings, EMG readings, and students' subjective self-report ratings of degree of stress. Specifically, students' skin temperature increased and their muscle tension decreased, indicating less physiological tension at the end of the session. Furthermore, comparisons of students' self-report ratings of their degree of stress before and after the session indicated that they felt less stressed and more relaxed at the conclusion of the session. These findings lend support to the utility of colleges providing biofeedback and relaxation skills training as an intervention for college students to utilize. In this way students become empowered to self-regulate their stress symptoms and optimize their health. Indeed, students can utilize and benefit from these skills both during and beyond their college years.

Book excerpt: College counseling centers are searching for innovative and short-term mental health interventions that promote wellness, especially given the growing number of college students who report serious and chronic stress. Researchers and clinicians have tested two promising interventions, self-compassion meditation and biofeedback training, in their search to find effective tools to assist people in handling stress. Some evidence shows that these interventions can help individuals to increase mindfulness and self-compassion and to decrease self-criticism and anxiety. The purpose of the study was to investigate whether HRV biofeedback training using the emWave and Self Compassion Meditation would help participants to attain a more positive state as measured by the questionnaires. Forty participants were randomly assigned to one of three conditions: Biofeedback, Self-Compassion and Control. The dependent variables and the scales used to measure them included mindfulness (5-Facet Mindfulness Questionnaire; FFMQ), self-compassion (Self-Compassion Scale; SCS), self-criticism (Levels of Self-Criticism Scale; LOSC) and anxiety (State-Trait Anxiety Inventory, STAI-S and STAI-T). The procedures took place as follows: The Biofeedback Condition participated in Biofeedback training with the emWave, the Self-Compassion Condition listened to an adapted version of the Self-Compassion/ Loving Kindness meditation, created by Kristin Neff, and the Control Condition listened to a recording of an adapted version of the history of cookies by Linda Stradley (2004). It was hypothesized that participants in the Biofeedback Condition would report significantly greater outcome means on the FFMQ as well as significantly lower outcome means on the STAI-S and STAI-T when compared to the Control Condition and the Self-Compassion Condition. Participants in the Biofeedback Condition were also expected to report significantly higher mean exit scores on the SCS as well as significantly lower mean exit scores on the LOSC when compared to the Control Condition. The participants in the Self-Compassion Condition were expected to report significantly greater post-intervention means on the SCS as well as significantly lower mean exit scores on the LOSC when compared to the Biofeedback Condition and the Control Condition. The participants in the Self-Compassion Condition were also expected to report significantly higher mean exit scores on the FFMQ as well as significantly smaller mean exit scores on the STAI-S and STAI-T when compared to the Control Condition. A one-way between-groups ANOVA with post-hoc test revealed significantly lower STAI-S mean exit scores in the Biofeedback Condition and the Self-Compassion Condition when compared to the Control Condition. However, conducting a set of ANCOVAs did not reveal any significant differences between the exit scores of the three interventions (Biofeedback training, self-compassion meditation and control) on the FFMQ, the SCS, the LOSC or the STAI-T. The present study consisted of pilot work, thus, the small number of participants in each condition may have impacted statistical power. Future studies should try to replicate the results with greater numbers of participants.
