No one will deny that the delicacy of those relations that exist between medical people and their female patients, opposes, in a variety of cases, an insuperable barrier against any successful treatment whatever, for there are found individuals so fastidious as to prefer pain, disease, and even death itself, to any revelation concerning their disorders. Some women, who do not object to relate the history of their diseased sensations, compel us to rest satisfied with such barren histories as they themselves can give. Book excerpt: Excerpt from Diseases of the Uterus: A Series of Clinical Lectures Shortly after I entered the hospital La Pitte as an intern student, and became prosector to Lisfranc, be commissioned me to publish, in my own name, his Clinical Lectures upon Diseases of the Uterus, the notes of which he would previous ly revise. With this view, he delivered a course of lectures, which I noted down with the most scrupulous accuracy. The report of each lecture was then submitted to his approbation. The greater portion of the series was published in the Gazette Médicale, in four very full articles. The remainder, on Am putation of the Neck of the Uterus, did not appear; it was Lisfranc's wish personally to communicate his principles with regard to it, which he did, in June, 1834, in a Memoir read before the Academy of Sciences Experience soon showed me the fatality of amputation of the neck. Results, so different from those announced, excited painful doubts; and I prudently deferred the publication of this portion of my notes, until, in 1835, doubts became certainty. At this time, a list of the successful amputations was demanded by the Institute. Iwas commissioned to look up the materials for its formation. But, after examining the theses of former interns and Lisfranc's private papers, the number of cases found was very small in proportion to the eighty/four suc cessful cases announced. Book excerpt: Excerpt from A Practical Textbook of the Diseases of Women Some progress has been made towards deciding whether the supra-vaginal amputation of the cervix, or total extirpation of the uterus, should be the treatment for cancer of the cervix in cases suitable for radical treatment. For cancer beginning in the vaginal portion there can be no doubt that the supra-vaginal amputation of the cervix is the right treatment. For cancer beginning in the cervix above the vaginal portion, it seems probable that, if the case is seen early enough, vaginal hysterectomy ought to be preferred. For primary cancer of the body there is, of course, only total extirpation. All authorities are not yet agreed that vaginal hysterectomy for cancer is more dangerous than the supra-vaginal amputation of the cervix. My own opinion, however, based on a considerable experience of both operations, is that total extirpation is de cidedly more dangerous, both as regards the risks necessarily attending the operative manipulations, and also as being a severe test of the vital powers of the patient. My experience of it so far has not been unfavourable - viz., seven cases with one death. In four of these cases the Operation was undertaken for primary cancer of the body of the uterus, and all these recovered; in the other three it was done for cancer of the cervix, and one of these died. It must be borne in mind that the operation itself, as an operation, is very much more difficult for primary cancer of the body, inasmuch as in such cases the body of the uterus is so often greatly enlarged.
