Book excerpt: The Determinants of Discretionary Front-of-Package Food Labelling Anthea Christoforou Doctor of Philosophy Department of Nutritional Sciences University of Toronto 2017 ABSTRACT Front-of-package (FOP) nutrition labelling is pervasive in Canada and occurs at the discretion of manufacturers. While there is evidence to suggest FOP labelling can impact product sales, other work has consistently demonstrated no association between the presence of a FOP reference and the nutritional quality of a product. A comprehensive examination of how manufacturers choose to engage in FOP labelling is needed to better understand the implications of this practice for consumers. Drawing on a survey of packaged foods sold in national chain retailers in Toronto the aims of this thesis were 1) to examine how the presence and nature of FOP references relate to a) level of food processing, b) product innovation (focusing on products designed as substitutes for traditional foods), and c) brand; and 2) to assess the nature of unregulated references through a systematic comparison of these references to nutrition labelling regulations. FOP nutrition references were more likely to appear on highly processed products, innovative foods and products manufactured by transnational brands, but they were less frequently displayed on products targeted to discount shoppers. A more in-depth examination of the nature of FOP material revealed a greater propensity for references highlighting 'nutrients to limit' (e.g., 'trans fat free', 'low in sodium') amongst highly processed products and those of transnational brands whereas innovative foods displayed a greater proportion of references which relayed information of 'positive' constituents (e.g., 'good source of calcium'). Transnational brand products were more likely than other products to bear unregulated 'natural' references and less likely to display regulated 'organic' labels, potentially signalling their need to circumvent regulatory requirements that would vary across markets. Nearly a quarter of products surveyed bore unregulated nutrition references, and most of these relayed information for which regulated options exist. Taken together, the strategic distribution of FOP references observed on highly processed products, innovative products and those manufactured by transnational brands, and the myriad of unregulated text found on these products, suggest FOP labelling functions primarily as a marketing tool and point to the need for a more effective regulatory framework for nutrition communication to better support healthy food selection.

These symbols and the systems that underlie them have become known as front-of-package (FOP) nutrition rating systems and symbols, even though the symbols themselves can be found anywhere on the front of a food package or on a retail shelf tag. Though not regulated and inconsistent in format, content, and criteria, FOP systems and symbols have the potential to provide useful guidance to consumers as well as maximize effectiveness. As a result, Congress directed the Centers for Disease Control and Prevention (CDC) to undertake a study with the Institute of Medicine (IOM) to examine and provide recommendations regarding FOP nutrition rating systems and symbols. The study was completed in two phases. Phase I focused primarily on the nutrition criteria underlying FOP systems. Phase II builds on the results of Phase I while focusing on aspects related to consumer understanding and behavior related to the development of a standardized FOP system. Front-of-Package Nutrition Rating Systems and Symbols focuses on Phase II of the study. The report addresses the potential benefits of a single, standardized front-label food guidance system regulated by the Food and Drug Administration, assesses which icons are most effective with consumer audiences, and considers the systems/icons that best promote health and how to maximize their use.

Previous research has examined how front-of-package labeling influences consumers' decisions to purchase products; however, most studies focus on populations living in areas with adequate income levels, sufficient access to grocery stores, and grocery stores that provide a variety of foods. The purpose of this research is to measure the effects, if any, of front-of-package food labeling on food preferences in a lower socioeconomic area of DC, where food options and establishments may be fewer and food environments less inviting. One-hundred and ten residents shopping at two major grocery stores in Ward 7, one of the lowest-income areas of Washington, DC, were surveyed to better understand the factors that regularly influence their food choices. To measure the role that front-of-package food labeling may play, we worked with two larger chain grocery stores. Customers were surveyed in order to gain a better understanding of the factors that influence their food choices. This study sheds light on the role of front-of-package labeling in environments where food preferences and access to foods may be limited, and in areas where price may play a more important factor than health benefits, as compared to more advantaged areas. The majority of participants in this study stated that nutrition was the most important factor when making food choices compared to cost, convenience, and taste. They also believed that front-of-package labeling plays a role in their food choices, and symbols can influence purchases. These findings indicate that this population has intent to make healthy choices in the grocery store, and there are opportunities for nutrition education in low-income areas.

Six provisions were not to be preempted until a study on the adequacy of the federal implementation of those provisions was completed. Food Labeling is the result of that study. It presents recommendations concerning the Food and Drug Administration's implementation of the six provisions that were studied, suggestions for the future disposition of relevant state and local food labeling requirements, and views on the continuing importance of the working relationship among the various levels of government in assuring that consumers are protected from misleading label information.

Book excerpt: Facts Up Front style nutrition labels are a front-of-package (FOP) nutritional labeling system that present key nutrient information on the front of packaged food and beverage products in an easy-to-read format. The authors conduct a large-scale empirical study to examine the effect of adoption of FOP labeling on the nutritional quality of products. The authors assemble a unique dataset on packaged food products (21,096 products, 9,083 brands, and 4,408 firms) in the U.S. across 44 categories over a period of sixteen years. They individually examine each new product introduced during the data time period to record FOP nutrition label information and use product-specific nutrient information to measure nutritional quality of products. By using a difference-in-differences estimator, the authors find that FOP adoption in a product category leads to an improvement in the nutritional quality of products in the category. They further find support for the moderating role of brand and product category characteristics. They argue that adoption of FOP in a product category serves as a source of “nutritional information clearinghouse” and empirically test for the role of salience of nutritional information as the mechanism that drives the improvement in the nutritional profile of the products. They find that subsequent to FOP adoption, food manufacturers improve the nutritional quality by lowering the content of calories and the nutrients to limit such as sugar, sodium, and saturated fat. Supplementary analyses are performed to rule out potential self-selection issues and a battery of robustness checks and falsification tests are conducted. The authors discuss the implications of the findings for public policy makers, consumers, manufacturers and food retailers.

In efforts to better inform consumers of their food consumption, increasingly front-of-package (FOP) food labels are displayed on packaged food items. FOP nutrition labels have become more prevalent in the US to help consumers identify healthier options at the point-of-purchase. This study aimed to evaluate the effectiveness of two current FOP labels, the Facts Up Front and the Facts Up Front Extended labels. In addition, a generic binary label was created to represent a non-brand associated symbol, which does not currently exist in the US. Participants were randomized to one of three of the label conditions or the no-label Control group. A total of 161 adults, who are primary grocery shoppers in the US, ages 18-69 participated in this study. All participants were recruited through an online database, Qualtrics, in January 2016, and participated in the study. The main outcome measures were the healthiness assessment of two product variants for three common food categories, along with label liking, familiarity, ease of comprehension, purchase intention, and perceived label credibility. Additionally, participants’ nutrition attitude, self-reported nutrition knowledge, and label behaviors were examined.
