Available in PDF, EPUB and Kindle. Book excerpt: Recently, several studies have shown the crucial role of viscoelastic properties of the aorta and large arteries in determining blood pressure values. A good knowledge of cardiovascular pathophysiology and vascular hemodynamics is necessary to understand these phenomena. However, available texts on this topic are very complex and difficult for medical practitioners to understand. The aim of this book is to enable the reader to comprehend the relation between cardiovascular pathophysiology and vascular hemodynamics and to become familiar with the more recent guidelines. While very complex physical concepts are involved, care has been taken to ensure that the explanations are as straightforward as possible. Complex analyses are presented separately and can be skipped by the reader without impairment of understanding. It is envisaged that the knowledge imparted will prove invaluable in improving the approach to hypertensive patients.

Book excerpt: High Blood Pressure: Its Variations and Control, A Manual for Practitioners covers aspects and consideration of blood pressure estimation and control. The book discusses the advantages and limitations of sphygmomanometry in clinical medicine and the best practical method of taking arterial pressures. The text describes the instruments employed in the estimation of arterial pressure; the fundamental physiological and physical factors on which clinical variations of blood pressure depend; and the physiological variations in arterial pressure. The classification of hyperpiesis; the nature, clinical features, types, and the pathology of hyperpiesia; and hypertonia are also considered. The book then tackles the causes and pathology of arteriosclerosis and the causation, significance, symptomatology, diagnosis, prognosis, prevention, and control of high arterial pressure. Simultaneous arterial pressures (brachial) in pulmonary tuberculosis; the relation of arterial pressure to life assurance; and the epochs in the clinical estimation of blood pressure are also looked into. Practitioners, teachers, and students of medicine will find the book invaluable.

Book excerpt: The Social Security Administration (SSA) uses a screening tool called the Listing of Impairments to identify claimants who are so severely impaired that they cannot work at all and thus immediately qualify for benefits. In this report, the IOM makes several recommendations for improving SSA's capacity to determine disability benefits more quickly and efficiently using the Listings.

Available in PDF, EPUB and Kindle. Book excerpt: In addition to standardized casual blood pressure readings, ambulatory blood pressure monitoring (ABPM) - using automatic noninvasive (= indirect) devices for home readings and fully automated monitors for 24-h profiles - have become a widely used necessary tool in clinical research. This book summarizes the state of the art in the whole field of indirect blood pressure monitoring. It is based on two international meetings and on invited papers. We have divided the subject matter into two main areas: 1) Automatic blood pressure devices for discontinuous registration, and 2) Portable, fully automated programmable monitors for continuous monitoring. The availability of all new technologies is described in detail and current technical and physiological problems have been covered in depth. Both topics have been subdivided into a) Methods and Techniques, and b) Clinical Applications. Both parts are updated and have critically evaluated available automatic sphygmomanometers and portable computers equipped with different techni ques (e. g. , auscultation, oscillometry, plethysmography). Reliability in the intensiv Care unit as well as in outpatients management, common clinical problems, clinical relevance compared to casual blood pressure are described in the first part. In the second part, ten years of experience on fully automated noninvasive methodology - compared to intraarterial techniques - have been elaborated by international experts; the possibilities and limitations are clearly demonstrated. Analyses in different clinical fields in the diagnosis of primary and secondary hypertension are given. Different statistical analyses of blood pressure variability and circadian rhythms are discussed.

Available in PDF, EPUB and Kindle. Book excerpt: Angiotensin and Blood Pressure Regulation examines the role of angiotensin in blood pressure regulation, with emphasis on its ability to influence the inotropic state directly via activation of myocardial receptors and indirectly by potentiating the release of catecholamines from sympathetic nerve terminals. The book also explains how angiotensin acts directly to regulate thirst and sodium appetite, and via antidiuretic hormone to control water reabsorption in the kidney. This book is comprised of eight chapters and begins with a review of the neuronal actions of angiotensin, primarily those in the brain, as well as the location and function of brain angiotensin II receptors. The effects of angiotensin on elements of the vascular wall and myocardium are also discussed, along with its function in hypovolemia, reproduction, and neurotransmission. Subsequent chapters explore some basic and functional considerations of the renin-angiotensin system in the brain; the biochemical processing that is required for angiotensin activation and inactivation; how angiotensin affects peripheral resistance; and the potency of angiotensin III as a pressor and dipsogenic agent in normotensive and hypertensive animal models. This monograph will be a useful resource for pharmacologists and physiologists.

Available in PDF, EPUB and Kindle. Book excerpt: This e-book will review special features of the cerebral circulation and how they contribute to the physiology of the brain. It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.
