This book was released on 2017-12 with total page 328 pages. Available in PDF, EPUB and Kindle. Book excerpt: Reconstructive surgery of the upper extremity has progressed significantly due to advances in microsurgical techniques, which include the increased popularity of perforator flaps, refinements in functional free muscle transfer, the application of nerve conduits and nerve allografts, motor nerve transfers, and hand transplantation. These innovations have helped surgeons achieve functional reconstruction goals and enabled patients' return to normal functioning earlier than previously possible. Reconstructive Surgery of the Hand and Upper Extremity presents a unique, color-coded roadmap for managing the complex upper extremity patient, based on a logical approach to surgical decision-making. Authors Gunter Germann, Randolph Sherman, and L. Scott Levin deliver seven systematically organized sections and 69 chapters with versatile, evidence-based guidelines and techniques. Strategic approaches and a video chapter covering patient examination reflect the collective wisdom gleaned from the authors' experience in the care of the upper extremity patient. Key Highlights Insightful guidance provided in 115 treatment-specific algorithms Patient histories and photos offer practical clinical information on a full continuum of cases-from gunshot wounds to tumors Over 500 high quality illustrations and graphics, many redrawn for this edition, delineate injuries and reconstruction techniques Richly illustrated atlas of flaps with nearly 140 new color photos details diverse approaches, including pearls and pitfalls Handy tables clearly outline rehabilitation protocols, classification, and zones of injury Includes bonus e-book version The text provides an invaluable framework to help both surgical residents and veteran upper extremity surgeons achieve optimal outcomes in their patients.

This function is made possible by a great freedom of movement of the first metacarpal and a highly developed and differentiated thumb musculature. The grasp function of the hand is dependent on the oppositional capacity and adductive power of the thumb, and is severely limited by a paralysis or dysfunction of the intrinsic thumb muscles. Whereas loss of the function of the adductor pollicis can be partially compensated for by the adductive action of the extensor pollicis longus, in paralysis or dysfunction of the radial thenar muscles compensation can only be provided by surgery. Since 1918, many methods of tendon transfer have been described for the restoration of thumb opposition, all of which bring about an improvement of the grasp function, albeit to different degrees. These methods vary in the selection of the motor, the direction of pull of the tendon, the use of a fulcrum, and the mode of insertion. The highly effective method of Bunnell (1938) is often used as the standard procedure. With this method, the flexor superficialis tendon of the ring finger is looped around the tendon of the flexor carpi ulnaris and passed subcutaneously across the thenar eminence, after which it is fixed on the thumb at the level of the metacarpo-phalangeal joint.

Available in PDF, EPUB and Kindle. Book excerpt: Reoperative Hand Surgery is a major contribution to the literature, offering a wide range of reoperative options for challenging problems that face the reconstructive hand surgeon after failed primary surgery. Concise and discussing all of the more common issues that hand surgeons often face in performing revision surgery, this invaluable title is unique in that it looks specifically at the problem of what to do when the original index procedure did not go as planned. Topics covered include the hand, wrist, forearm and elbow, and several sections of the book discuss some of the psychological and social factors that also go into the decision process to reoperate or not. The chapters are tightly focused so that the important aspects to consider for reoperation can be quickly reviewed. There is also a component that discusses how to avoid other pitfalls and to minimize as well as manage any complications that may occur with reoperative procedures. Importantly, the book is replete with original art work to help describe and demonstrate how to perform many reoperative procedures. The artwork is simple and straightforward but yet detailed enough to convey the important steps and aspects for the surgical intervention it is outlining. Developed by renowned experts in the hand surgery field, Reoperative Hand Surgery is an indispensable, state-of-the-art reference, offering the busy hand and upper extremity surgeon easily accessible information relating to the evaluation, diagnosis, and possible surgical interventions for the diagnoses discussed.

It can be performed with no preoperative testing, no intravenous insertion, and no monitoring. Like a dental procedure, the patient simply gets up and goes home after the procedure. Presented in an easy-to-read, bullet-point format, Wide Awake Hand Surgery guides surgeons through all aspects of WALANT. The book covers a wide variety of topics including minimal pain injection of local anesthesia, nerve and tendon decompression, wrist surgery, repair of lacerated tendons, tendon transfers, finger fractures, lacerated nerves, metacarpal fractures, arthritis surgery and complex reconstructions in hand surgery. The book includes more than 150 step-by-step surgical and instructional videos as well as numerous color clinical photographs. Color drawings clearly guide the surgeon to the correct anatomic locations for anesthetic injections, and the book includes an atlas of tumescent local anesthesia distribution anatomy. Featuring a complimentary eBook, this valuable resource offers chapters written by worldwide experts, making it the definitive guide to wide awake hand surgery.

Book excerpt: Treatment of symptomatic irreparable cuff tear is rare, but represents a difficult challenge for the surgeon. Tendon transfer from periscapular muscles group is a therapeutic option. Many tendon transfers are reported as latissimus dorsi, teres major, split of anterior deltoid and pectoralis major. The goal of this book is to remind the biomechanical and anatomical basis of these tendon transfers and to report tips and tricks and results of these techniques. A treatment algorithm is proposed in patients who have both pain and weakness or have loss of active forward elevation or external rotation. Moreover, tendon transfer can be combined with reverse shoulder prosthesis in patients with an unstable gleno humeral arthritis.

Book excerpt: In this book, globally renowned orthopedic, plastic, and hand surgeons provide the knowledge required in order to understand and resolve the full range of problems associated with diseases, anomalies, deformities, and trauma of the thumb. The opening section describes the history of “making a thumb” and covers the fundamentals of anatomy, embryology, and functional dynamics. After careful presentation of the surgical procedures for various developmental anomalies of the thumb, subsequent sections focus on the treatment of bone and joint, tendon, and nerve problems encountered in patients with different diseases and injuries. All aspects of the surgical management of benign and malignant tumors of the thumb are then described. The final section is devoted to current and emerging treatments for trauma, including amputation and microsurgical and non-microsurgical reconstruction. The text is supported by superb clinical photographs as well as high-quality schematic drawings and video clips. The book will be of value not only to practicing surgeons but also to residents and medical students.
