Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

Book excerpt: The Veteran's Health Administration (VHA) healthcare system which is the largest integrated health care system in the U.S. serves a large majority of Veterans who are at an increased risk for suicide. It is estimated that Veterans have a 50% higher rate of suicide compared to the U.S. general population (McCarthy et al., 2009). Posttraumatic stress disorder or PTSD is a mental health disorder common to the military population. Of Veterans receiving care through the VHA, 11% have been diagnosed with PTSD making it the most common mental health diagnosis seen in the VHA (Barnett et al., 2014). More than 17% of returning Veterans also meet criteria for a depressive disorder; and having either PTSD or major depressive disorder increases the odds more than threefold of also having a drug use disorder in the Veteran population (Seal et al., 2009). The major aim of this research is to investigate if there is a statistically significant association of suicide risk in a Veteran suffering from PTSD, depression, or substance abuse in the VA southern California Healthcare System. The total sample size needed would be 682 Veterans. A multinomial logistic regression analysis will be used to determine a statistical significant association of the dependent variable suicide risk by the independent variables of PTSD, depression, and substance abuse. Frequency distribution for covariants such as age, gender, ethnicity, and other variables of interest will also be explored.

Book excerpt: Concurrent Treatment of PTSD and Substance Use Disorders Using Prolonged Exposure (COPE) is a cognitive-behavioral psychotherapy program designed for patients who have posttraumatic stress disorder (PTSD) and a co-occurring alcohol or drug use disorder. COPE represents an integration of two evidence-based treatments: Prolonged Exposure (PE) therapy for PTSD and Relapse Prevention for substance use disorders. COPE is an integrated treatment, meaning that both the PTSD and substance use disorder are addressed concurrently in therapy by the same clinician, and patients can experience substantial reductions in both PTSD symptoms and substance use severity. Patients use the COPE Patient Workbook while their clinician uses the Therapist Guide to deliver treatment. The program is comprised of 12 individual, 60 to 90 minute therapy sessions. The program includes several components: information about how PTSD symptoms and substance use interact with one another; information about the most common reactions to trauma; techniques to help the patient manage cravings and thoughts about using alcohol or drugs; coping skills to help the patient prevent relapse to substances; a breathing retraining relaxation exercise; and in vivo (real life) and imaginal exposures to target the patient's PTSD symptoms.
