Pour aider les autorités locales à mettre en œuvre la vaccination anti-papillomavirus, nous fournissons des estimations détaillées du fardeau du cancer du col de l'utérus et de l'impact prévu de la vaccination anti-papillomavirus chez les jeunes filles d'aujourd'hui qui pourraient développer un cancer du col de l'utérus si elles ne sont pas vaccinées. Avec la base de données GLOBOCAN 2018, les projections démographiques des Nations Unies et une méta-analyse estimant la proportion des différents sous-types de papillomavirus dans les cancers du col, nous quantifions le nombre de cancers du col de l'utérus attendus et évitables par la vaccination chez les filles nées entre 2005 et 2014 qui sont ou seront éligibles à la vaccination anti-papillomavirus. Nous fournissons des estimations régionales et nationales pour différents niveaux d'efficacité des programmes de vaccination et les différents types de vaccins. A l'échelle mondiale, en l'absence de vaccination, nous estimons à environ 11,6 millions le nombre de cas de cancers du col de l'utérus chez les femmes nées entre 2005 et 2014 au cours de leur vie. Environ 75% du futur fardeau mondial du cancer du col de l'utérus sera concentré dans 25 pays, principalement situés en Afrique et en Asie, où respectivement 5,6 et 4,5 millions de cas devraient se produire. Une vaccination mondiale avec un vaccin contre le papillomavirus ciblant les sous-types HPV16,18, avec une protection croisée contre les sous-types HPV31,33,45, pourrait permettre d'éviter environ 8,7 millions de cas. Les estimations détaillées à court terme du fardeau du cancer du col de l'utérus et de l'impact prévu de la vaccination anti-papillomavirus présentent un intérêt immédiat pour les décideurs en matière de politique de santé publique. Il est fondamental de réorienter les projections vers les jeunes filles nées récemment et en âge d'être vaccinées, qui risquent de développer un cancer du col de l'utérus si elles ne le sont pas, afin de surmonter les hésitations des principaux décideurs en santé publique à l'égard de la vaccination contre le papillomavirus.
