Federal Medicaid rules require that the program cover many, but not all, important preventive screening services, but states also have considerable latitude in establishing which preventive services are covered for adults and whether or not to charge enrollees copayments for these services. As such, there is sizable variability by state in Medicaid coverage of preventive services for adults. As of 2014, these state policy choices will affect millions more women who will be newly eligible for Medicaid after the implementation of the Affordable Care Act (ACA). The ACA will also provide an enhanced federal matching payment to state Medicaid programs that cover certain recommended preventive services without cost-sharing starting in 2013. The Kaiser Commission on Medicaid and the Uninsured (KCMU) and Health Management Associates (HMA) surveyed Medicaid officials in all 50 states and the District of Columbia in 2010 to collect baseline data on state coverage of preventive services prior to ACA implementation. This brief reviews Medicaid's role in covering preventive care for women, presents findings of importance to women from the survey, and discusses the implications for women on Medicaid following the implementation of health reform.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

One of these provisions goes into effect on January 1, 2013, when state Medicaid programs will have the opportunity to receive a one percentage point increase in their Federal Medical Assistance Percentage matching rate ("FMAP") if they cover the immunizations recommended by the Centers for Disease Control and Prevention's Advisory Committee on Immunization Practices ("ACIP") and the preventive services rated "A" or "B" by the U.S. Preventive Services Task Force ("USPSTF") without cost-sharing. The USPSTF is an independent panel of experts that reviews scientific evidence on clinical preventive health care services and issues recommendations for primary care providers and health systems. This study evaluates Medicaid coverage across the country for twenty-four of those services that focus on non-pregnant adults. There was significant variation among states in which services were covered under Medicaid for non-elderly, non-pregnant adults. While some research has been published examining Medicaid coverage of select preventive services, there has not been a comprehensive look at state-level Medicaid coverage of preventive services for adults. This study is intended to provide a better understanding of Medicaid coverage of preventive services for adults in the current state programs, help guide federal policymakers as they make decisions about the criteria qualifying a state for the FMAP increase, and inform state policy makers as they consider the level of preventive benefits and services to offer should they expand Medicaid in 2014.

Clinical Preventive Services for Women reviews the preventive services that are important to women's health and well-being. It recommends that eight preventive health services for women be added to the services that health plans will cover at no cost. The recommendations are based on a review of existing guidelines and an assessment of the evidence on the effectiveness of different preventive services. The services include improved screening for cervical cancer, sexually transmitted infections, and gestational diabetes; a fuller range of contraceptive education, counseling, methods, and services; services for pregnant women; at least one well-woman preventive care visit annually; and screening and counseling for interpersonal and domestic violence, among others. Clinical Preventive Services for Women identifies critical gaps in preventive services for women as well as measures that will further ensure optimal health and well-being. It can serve as a comprehensive guide for federal government agencies, including the Department of Health and Human Services and the Center for Disease Control and Prevention; state and local government agencies; policy makers; health care professionals; caregivers, and researchers.

Book excerpt: Background: Breast cancer is the most common cancer in women aside from non melanoma skin cancer (CDC, 2009a) All women are at risk for cervical cancer, but the disease usually occurs in women 30 years and older (CDC, 2010a). Access standards for breast and cervical cancer screenings in the Medicaid program appear to be limited. The study hopes to address standards that define adequate access to breast and cervical cancer screenings for prevention specifically in the Medicaid program. Objectives: To discern if there are prevailing standards for access for breast and cervical cancer screening, to understand accessibility and its various components more thoroughly, to gain a deeper insight into breast and cervical cancer prevention, and to explore and document methods for research on screening standards in order to guide others who may follow up on research or use for other services. Methods: This study consisted of literature searches and reviews specific to access standards, Medicaid, breast cancer, and cervical cancer. Online searches for specific contracts and contacts were targeted to various state departments of health, medical societies for breast and cervical cancer, and national organizations. For information that could not be searched, emails and phone calls were the best ways to obtain the information needed. Results: Given the research, the hypothesis was proven true. Access standards for breast and cervical cancer prevention do not exist for the organizations and departments researched. The importance of creating standards is that it allows for the measurement of access to care. The primary recommendation from this study is that access standards should be developed to measure the effectiveness of breast and cervical cancer screenings and prevention services. The second major recommendation is to develop access standards by researching the actual experience with wait-times for breast and cervical cancer screening. Since the need for access to care standards for breast and cervical cancer screening is addressed, the next question would be how to go about developing standards. Conclusions: The findings suggest there is a gap in information on breast and cervical cancer preventive services. The main recommendation is that access to care standards for breast and cervical cancer preventive services be developed in order to measure access to care in the Medicaid program. This would allow the Medicaid program's breast and cervical cancer preventive services for women to be assessed.
