Book excerpt: Since 1948, the World Health Organization (WHO) has launched numerous programs aimed at improving health conditions around the globe, ranging from efforts to eradicate smallpox to education programs about the health risks of smoking. In setting global health priorities and carrying out initiatives, the WHO bureaucracy has faced the challenge of reconciling the preferences of a small minority of wealthy nations, who fund the organization, with the demands of poorer member countries, who hold the majority of votes. In The World Health Organization between North and South, Nitsan Chorev shows how the WHO bureaucracy has succeeded not only in avoiding having its agenda co-opted by either coalition of member states but also in reaching a consensus that fit the bureaucracy's own principles and interests. Chorev assesses the response of the WHO bureaucracy to member-state pressure in two particularly contentious moments: when during the 1970s and early 1980s developing countries forcefully called for a more equal international economic order, and when in the 1990s the United States and other wealthy countries demanded international organizations adopt neoliberal economic reforms. In analyzing these two periods, Chorev demonstrates how strategic maneuvering made it possible for a vulnerable bureaucracy to preserve a relatively autonomous agenda, promote a consistent set of values, and protect its interests in the face of challenges from developing and developed countries alike.

In Politics in the Corridor of Dying, Jennifer Chan documents the emergence of a diverse range of community-based, nongovernmental, and civil society groups engaged in patient-focused AIDS advocacy worldwide. She also critically evaluates the evolving role of these groups in challenging authoritative global health governance schemes put in place by what she describes as overcontrolling or sanctimonious governments, scientists, religious figures, journalists, educators, and corporations. Drawing on more than 100 interviews conducted across eighteen countries, the book covers a broad spectrum of contemporary sociopolitical issues in AIDS activism, including the criminalization of HIV transmission, the fight against "big pharma," and the politics of the President's Emergency Plan for AIDS Relief. Chan argues that AIDS activism disrupts four contemporary regimes of power—scientific monopoly, market fundamentalism, governance statism, and community control—by elevating alternative knowledge production and human rights. This multi-year venture has involved a research team of some forty chapter authors and commentators. The research has been accompanied by three major workshops on project methodology, initial chapter reviews and final discussions. A point was made of including both scholars and practitioners involved in power-sharing settlements in the review process, in the hope that more would be learned about the actual implementation of the settlements under investigation. The project team was united in its wish to explore whether long-standing secessionist conflicts have been addressed effectively through the significant number of self-determination settlements that were generated in response to the wave of internal conflicts of the 1990s. It was also committed to testing whether consociationalist and integrative techniques of conflict settlement really are as mutually exclusive as is sometimes supposed, or whether they can in fact be mutually reinforcing. Finally, the project derives its impetus from the necessity to critically rethink the doctrine of self-determination. One may question whether its traditional, restrictive interpretation will be adequate in confronting the wide variety of future challenges to the territorial integrity of states.
