Book excerpt: Post-closure tailings dams are expected to remain on mine sites in perpetuity following mine closure. Combined with the lack of knowledge regarding how tailings dams will evolve over time, these long-time frames necessitate the development of risk management practices. The purpose of this research was to develop methodology to support the process of determining and assessing the long-term risks of tailings dams in Alberta as they transition into landforms. The research project evaluated the state of knowledge on the long-term behaviour of tailings dams and demonstrated the knowledge gap (and associated uncertainty) associated with the closure phase of a tailings dam. To address this, a risk management tool, referred to as the Generalized Failure Modes Effects Analysis (G-FMEA) framework, was developed. The G-FMEA framework was applied to a case study oil sands tailings dam to illustrate an example application for practitioners. Finally, seepage analysis was conducted on the case study oil sands tailings dam to demonstrate how modelling tools can be used in conjunction with risk management tools to reduce uncertainty. To aid in the development of the G-FMEA framework, a detailed literature review and interviews with industry professionals were conducted. This process allowed for available information on the physical performance of tailings dams undergoing closure in Canada to be synthesized. Further, it allowed for the identification of hazards, triggers, failure modes, and uncertainties associated with tailings dam closure, which was a key element to the G-FMEA development. The interviews themselves clearly showed the lack of consensus amongst practitioners regarding long-term dam evolution. This emphasizes the uncertainty associated with closure and is a primary reason why comprehensive risk management practices must be developed. The developed G-FMEA framework uses an element approach to conduct the FMEA, which allows for the relationships between different elements to be established. The G-FMEA framework includes four individual charts for the drainage system, foundation, dam body, and landform to assess the failure modes, triggers/causes, and failure effects for different elements. The G-FMEA framework requires the risk assessment to be conducted at four different temporal scales (immediate term, short term, medium term, and long term) that are anchored to different periods of management of the facility. A risk matrix was developed for use with the G-FMEA, which includes a likelihood rating table, consequence rating table, and risk rating table. The developed G-FMEA framework provides a systematic method to assess the risks following closure of tailings dams. The G-FMEA framework was applied to a case study oil sands tailings dam to illustrate how the framework could be used in practice. Two example failure modes were selected to show how the risk matrix could be used to assign risk ratings. For both of the example failure modes, the risk rating increased as the temporal scale increased, which is partially attributed to the increasing uncertainty over time. Long-term seepage modelling was conducted on a case study oil sands tailings dam to evaluate how the phreatic surface may evolve in response to various factors (drain failure, pond formation, downstream slope erosion, and climate change). The results suggest that the phreatic surface has the potential to rise in the long term in response to different events, including drain failure and ponds forming on the reclamation surface. The seepage modelling was used to show how modelling could be used to inform risk management decisions by reducing uncertainty and allowed for the development of a framework for guidance on conducting long-term seepage modelling.

We are left to question how we address the risks associated with tailings disposal, and in so doing, transform the image of the mining industry and perhaps the industry itself. With this as a backdrop, the Society for Mining, Metallurgy & Exploration (SME) challenged tailings and mining professionals to re-imagine the future of tailings. The Mine Tailings: Perspectives for a Changing World symposium, held at the SME 2020 annual conference, started that conversation. Over three days, tailings professionals from around the world gathered to discuss tailings storage practices and the changes both the industry and the world want and need. The discussions squarely focused on how we, as an industry, can collectively make changes that will eliminate catastrophic tailings dam failures and lead to better outcomes for the industry and society. Through sharing and conversation, the symposium participants recognized risks associated with our approach to tailings management and existing structures and discussed the gaps that need to be addressed, including how the behavior of tailings and mining professionals must change. The human element of risk must be recognized so it can be talked about openly, given the attention it deserves, and adequately addressed. We need to own this problem and the impact of our actions. We have the power to change this; when we own our actions, we can act differently for a different outcome.

The Act of 2002 states that the Director of the Federal Emergency Management Agency (FEMA), in cooperation with the National Dam Safety Review Board (Review Board), shall carry out a program of technical and archival research to develop and support: improved techniques, historical experience, and equipment for rapid and effective dam construction, rehabilitation, and inspection; devices for continued monitoring of the safety of dams; development and maintenance of information resources systems needed to support managing the safety of dams; and initiatives to guide the formulation of effective policy and advance improvements in dam safety engineering, security, and management. With the funding authorized by the Congress, the goal of the Review Board and the Dam Safety Research Work Group (Work Group) is to encourage research in those areas expected to make significant contributions to improving the safety and security of dams throughout the United States. The Work Group (formerly the Research Subcommittee of the Interagency Committee on Dam Safety) met initially in February 1998. To identify and prioritize research needs, the Subcommittee sponsored a workshop on Research Needs in Dam Safety in Washington D.C. in April 1999. Representatives of state and federal agencies, academia, and private industry attended the workshop. Seventeen broad area topics related to the research needs of the dam safety community were identified. To more fully develop the research needs identified, the Research Subcommittee subsequently sponsored a series of nine workshops. Each workshop addressed a broad research topic (listed) identified in the initial workshop. Experts attending the workshops included international representatives as well as representatives of state, federal, and private organizations within the United States. Impacts of Plants and Animals on Earthen Dams; Risk Assessment for Dams; Spillway Gates; Seepage through Embankment Dams; Embankment Dam Failure Analysis; Hydrologic Issues for Dams; Dam Spillways; Seismic Issues for Dams; Dam Outlet Works. Based on the research workshops, research topics have been proposed and pursued. Several topics have progressed to products of use to the dam safety community, such as technical manuals and guidelines. For future research, it is the goal of the Work Group to expand dam safety research to other institutions and professionals performing research in this field. The proceedings from the research workshops present a comprehensive and detailed discussion and analysis of the research topics addressed by the experts participating in the workshops. The participants at all of the research workshops are to be commended for their diligent and highly professional efforts on behalf of the National Dam Safety Program. The National Dam Safety Program research needs workshop on Embankment Dam Failure Analysis was held on June 26-28, 2001, in Oklahoma City, Oklahoma. The Department of Homeland Security, Federal Emergency Management Agency, would like to acknowledge the contributions of the Agricultural Research Service and the Natural Resources Conservation Service of the U.S. Department of Agriculture in organizing the workshop and developing these workshop proceedings.
