A consensus has not been reached regarding potential risk factors related to the loss of short implants.Purpose: This large-sample retrospective study concentrated not only on patient characteristics and medical procedures but also on the features of implants in order to uncover the risk factors associated with short implants.Methods: Between 2014 and 2017, a total of 7001 implants were inserted at Nanjing Stomatological Hospital, Medical School of Nanjing University. Among the all, 1236 short implants were included after being evaluated according to the inclusion and exclusion criteria. In organizing the detailed information, seven variables including bone grafting procedure, age, gender, diameter of the implant, implant position, surface treatment and definitive restorations were taken into consideration. Book excerpt: Background : Short implants are considered to be the simpler and more effective alternative to complicated bone graft surgery in clinical situations with reduced alveolar bone height. But, a considerable number of clinicians still hesitate to use short implants questioning about their prognoses mainly due to the reduced contact area between the bone and implant and unfavorable crown to implant ratio compared to longer implants. Aim : The aim of the study is to evaluate the clinical and radiographic outcomes of short implants supporting fixed prostheses in posterior regions. Methods : A retrospective study design was adopted. 69 short implants(intra-bony length u2264 8 mm) supporting fixed prostheses in posterior regions of 56 patients were included. The implant success rate and periimplant marginal bone loss were evaluated. The effects of associated factors on the implant performance were analyzed. Results : A total of 3 implants failed. 2 implants were lost before loading and 1 implant was lost at 7 months after loading. The mean follow up was 30.1 months(SD=11.8 months). Success rate was 95.7% and 94.6% for the implant and patient-based analysis respectively. The average marginal bone loss after 1 year of follow-up was 0.02 u00b1 0.16 mm at mesial and 0.03 u00b1 0.14 mm at distal aspect. No relationship was observed between the studied variables and the marginal bone loss. Conclusions: High survival rates for short implants in posterior regions could be achieved with minimal marginal bone loss in this study. Within the limits of the short term follow up, a short implant (u2264 8 mm ) may be considered as a predictable treatment modality for posterior region with reduced bone height.

Book excerpt: Purpose: The main objective of this retrospective study was to compare the clinical outcomes of 6mm implants (Test group) vs. 8mm implants (Control Group) in terms of success, survival, marginal bone loss and periodontal parameters.Material & Methods: Implants placed in the Clinica Universitaria Odontologica at Universitat Internacional de Catalunya between 2005 and 2015 were evaluated. Control visits were performed evaluating the following parameters (present complications, periodontal parameters, probing depth, bleeding on probing, plaque index, suppuration), periapical radiographs were performed to determine bone loss from prosthesis delivery to check-up visits, and clinical records were reviewed for possible complications. Results: A total of 114 implants were reviewed, 68 implants of 6mm and 46 implants of 8 mm. Total survival rate of the test group was is 94,1%, having 4 failures. A single failure was found in the control group, corresponding to a survival of 97,8%. Regarding success rates, according to Buser criteria, in the control group 87,5% of the the implants meet the requirements to be considered successful vs only 64,2% of implants in the test group. The mean marginal bone loss in the control group was 0,43 u00b1 0,24 mm in mesial and 0, 59 u00b1 0,44 mm in distal, while 0,81 u00b1 0,33 mm in mesial and 0.59 u00b1 0,36 mm in distal were found in the test group. All factors were analyzed with a mean follow-up period of 4.5 and 5 years respectively between test and control group. Conclusions: Within the limitations of the study, 6mm implants show more unfavorable results regarding survival, success and levels of marginal bone loss compared to 8mm implants in an average follow-up time of of 4.5 and 5 years respectively between test and control group. 8mm implants could be considered a more valid therapeutic option.

Book excerpt: Significant progress in the field of implant dentistry has led to various trend shifts with regard to treatment indications, implant parameters as well as surgical procedures. Compared to edentulous jaws in the past, the single-tooth gap represents the primary indication for implant placement today. Advances in oral regeneration have enabled implant therapy for more and more patients, and new implant designs as well as novel surgical techniques have broadened the spectrum of implant therapy.Recognizing changes in the field implant dentistry is crucial for the development of the field. Thus, we aimed to perform an analysis of a large patient pool to identify current trends and tendencies as well as possible risk factors for complications and early implant failure.We performed a retrospective analysis of patients referred to the Department of Oral Surgery of the Medical University of Vienna, University Clinic of Dentistry between January 2016 and November 2018. First, we evaluated subject characteristics (e.g., demographics, systemic conditions, medication). We then analyzed indications for implant placement (single-tooth gap, extended gap, distal extension, edentulous jaw), implant parameters (length, diameter, type, material), the timing of surgery (immediate, early, late) as well as the exact surgical procedures (e.g., bone augmentation, computer-aided navigation). Finally, we assessed postoperative complications (hematoma, local infection, swelling, nerve damage) as well as early (i.e., before loading) implant failures.A total of 1,132 patients (16u201390 years, mean 50.6 u00b1 16.5 years, 55.4% female, 19.3% smokers) received 2,413 implants. The majority of patients had 1 implant (n = 525, 46.7%). The maximum number of implants per patient was 10. A total of 1,028 implants (42.6%) were used in single-tooth gaps, 379 (15.7%) in extended edentulous gaps, 469 (19.4%) as distal extensions, and 537 (22.2%) in completely edentulous jaws. Implants 11.5 mm in length and 4.3 mm in diameter were the most common (19.6% of all implants). No implants shorter than 8 mm were used, and 86.4% of all implants were between 3.5 and 4.5 mm in diameter. A total of 1,879 (77.87%) implants were placed late (i.e., more than 6 months after first edentulousness) and 732 implants (30.3%) were supported by an augmentative measure. Computer-aided navigation was used for the placement of 226 implants (9.4%). A total of 71 patients (6.27%) reported a postoperative complication. Early implant failure occurred in 16 implants (0.7%).The data show a largely conservative approach to implant dentistry, as evidenced by the absence of short implants, the late time point of surgery, the low prevalence of computer-aided navigation as well as the reservation of bone augmentation for cases where they were indispensable. Our findings highlight a trend shift towards single-tooth replacements, similar to the recent literature. However, our data show a significantly lower prevalence of augmentative measures than comparable studies.

Data was used to identify possible factors associated with multiple dental implant failures and compare these factors between individuals with single and multiple implant failures. Associations between various factors and multiple implant failure were reported. Results: Excluding history of implant failure, the following factors were found to be associated with MIF: machined surfaces, post-operative infections, presence of certain prostheses opposing the implant, periodontitis, alcohol consumption, history of chemotherapy, and use of antidepressants. Conclusions: Provision of implant-based care for patients presenting with factors associated with multiple implant failure should be undertaken with caution.

No obvious trend of increasing implant failure rates over time has been observed yet, much attention has lately been focused on mucosa health and bone loss at the implants, referred to as u201cperi-implantitisu201d. It can be observed that available inclusion parameters for diagnosis of peri-implantitis show an obvious variation in the present literature, and it would be of value to further analyze and report the problems with peri-implantits and implant failures in another larger patient groups in routine practice. Aim :The aim of the present study was to report problems with peri-implantitis and late implant failures in a large private implant referral clinic where patients have been treated on a routine basis over a period of time. The aim was also to analyze the impact of different clinical and patient factors over time with regard to clinical observations related to peri-implantitis, surgery related to peri-implantitis and late implant failures.Material and Methods :A total of 1 017 patients were consecutively provided with 3 082 implants with an anodized surface between 2000 and 2011. All patients with three events of problems were identified; u201cperi-implantitisu201d, u201csurgery related to peri-implantitisu201d and u201clate implant failuresu201d. A logistic multivariate analysis was performed to identify possible factors with an association to the three events.Results :Altogether, 66 patients (92 operations) and 30 patients (34 operations) were identified with u201cperi-implantitisu201d and u201csurgery related to peri-implantitisu201d after first year in function. The statistical analyses identified five and three clinical parameters significantly associated to the two events, respectively. Two-stage surgical technique (with GBR technique; HR1.82; 95% CI: 1.32 u2013 2.52) and immediate grafting at implant surgery (HR 3.34; 95% CI: 1.06-10.54) were the factors with the highest risk associated to these two events, respectively. Risk of peri-implantitis increased by year of inclusion from year 2000 (HR 1.28; 95% CI; 1.16-1.41). u201cLate implant failuresu201d were associated to u201csmokingu201d (HR 2.11; 95% CI: 1.26-3.55), u201ctwo-stage surgeryu201d (with immediate grafting; HR 1.67; 95& CI: 1.11 u2013 2.59) and u201ctype of implantu201d (NobelActive CC; HR 2.48; 95& CI: 1.05 -5.85). The use of short implants has been suggested as an alternative treatment option to vertical bone augmentation. However, there is still a lack of information concerning the long-term predictability of short implants.Aim: The aim of this retrospective study is to evaluate the survival rate of short SLA-surfaced implants with 6- and 8 mm lengths and at least 1-year follow up.Materials and Methods: All patients that had dental implant therapy with short implants between 01 January 2015 and 31 December 2018 that complied with mandatory follow ups were included in this retrospective study. Following ethic committee approval, data was retrieved from treatment records concerning patient characteristics, total or partial edentulism, implant characteristics, anatomical location of the implant, type of regeneration procedure, time of loading, date of placement, failure and final follow-up appointment. All patients in the study were treated by 3 experienced surgeons with Straumann Tissue Level comprising 6- (TL6) or 8 mm (TL8) implants, using a standardized surgical procedure. Early failure was defined as an implant that failed to integrate and was removed before 6 or 3 months for implants placed with and without regenerative procedures, respectively. Late failure was defined as implant removal with prosthetic loading following the osteointegration period.Results: A total of 199 patients received 191 TL6 and 156 TL8 dental implants with a follow up that ranged between 1 and 4 years. Most of the implants (93.7%) had a diameter of 4.1 mm, with the others (6.3%) having a diameter of 3.3 mm. The most frequent indication for short implant placement was the restoration of a partially edentulous arch (59.8%). The most common locations were the posterior mandible (63.4%) and the posterior maxilla (32.6%). The success rates for early failure were 98.4% and 99.4%, and for late failure (n=326) 99.4% and 99.3% for TL6 and TL8, respectively. No significant statistically differences were found regarding the different lengths for early and late failure (P>0.05).Conclusions and Clinical Implications: Within the limitations of this retrospective study, the 6- and 8 mm SLA implants evaluated demonstrated an overall medium-term survival rate with no significant differences between them, which may be compared to that of standard-length implants.

It is estimated that every year, over two million implants are placed throughout the world. However, dental implants may exhibit immediate or delayed complications that could lead to implant failure. The aim of this study was to explore the factors coincidental with removal of failed implants at the UCSF Postgraduate Periodontology Clinic (PGPC) by reviewing electronic health records (EHR) for the last 19 years.Methods: EHR data from Axium were searched for the dental code D6100 (implant removal) from January 2001 - April 2019. Furthermore, code D7140 (simple tooth removal) was used to include cases that were suspected to be misclassified. Inclusion criteria was focused on patients who had dental implant(s) removed and had sufficient records for analysis. Patient and surgical data were gathered from dental records and analyzed. Exclusion criteria focused on patient data that was incomplete or not accessible.Results: Upon review of 3,426 simple tooth extractions and 81 implant explantations, it was determined that 114 implants were explanted from 88 patients over 19 years. Fifteen patients were removed from the data analysis based on exclusion criteria. Analysis was performed on 73 patients who had a total of 96 implants removed. Of these, 59% of patients had periodontal disease and 32% were smokers. Amongst the medical conditions reviewed, 32% of patients reported cardiovascular disease, 23% had a bone disorder, and 18% had emotional disorders. In terms of the implants, 41 implants were placed in grafted bone; 54% being allograft. Out of 96 implants removed, 58 were late failures and 33 implants failed before loading. Furthermore, we examined the reasons for tooth extractions at sites with subsequent implant failure and found that 6 teeth had endodontic treatment and 3/6 had periapical abscesses. Forty-five percent of cases that had a late failure did not report regular recall and maintenance appointments.Conclusion: Implant failure was coincidental with a history of periodontal disease and other risk factors, such as cardiovascular disease and emotional disorders. A lack of consistent maintenance recalls was frequently found in cases with late implant failure. Not every patient is a good candidate for implant therapy and risk factors have to be carefully evaluated and presented to the patient. This research highlights the need for a systematic evaluation of patients planning to receive implants and the need to inform them of possible complications and risks of implant treatment.
