Les retraitements endodontiques orthogrades et/ou rétrogrades sont réalisés en première intention. Lors du suivi clinique, si la persistance de la lésion inflammatoire péri radiculaire d’origine endodontique est observée et que plusieurs facteurs contre-indiquent son retraitement endodontique conventionnel, l’avulsion de la dent peut être indiquée. L’avulsion atraumatique suivie de la réimplantation intentionnelle de la dent peut permettre de réaliser le retraitement endodontique rétrograde ex vivo ainsi que le curetage de la lésion péri apicale. Cette technique est décrite comme étant une nouvelle option clinique au sein du gradient thérapeutique à proposer au patient. Le but étant d’assurer le maintien de la dent sur l’arcade et la guérison de la lésion péri apicale.

Book excerpt: La désobturation canalaire est l'une des étapes clefs permettant le nettoyage minutieux et la remise en forme correcte du canal lors du retraitement endodontique. Il n'existe pas de technique universelle permettant de retraiter tous les canaux. Chaque cas est différent et la méthode doit être adaptée. Nous avons réalisé une étude comparative de deux techniques de désobturation afin d'en faire une critique. Trente dents extraites ont été traitées puis retraitées selon deux protocoles différents : le groupe A associant le GPXF® pour la désobturation au Protaper® pour la préparation canalaire, et le groupe B où le Protaper® est utilisé pour la désobturation et la préparation canalaire. Les résultats de l'étude mettent en évidence un gain de temps et une sécurité d'utilisation pour l'omnipraticien tout en assurant la qualité du retraitement endodontique avec l'utilisation du GPXF® associé au Protaper®.

Book excerpt: Grâce aux progrès réalisés les trente dernières années (utilisation des aides optiques et des inserts ultrasonores), la microchirurgie endodontique est devenue une option thérapeutique fiable avec un pronostic élevé. Néanmoins, cet acte technique est délicat. Il nécessite des compétences à la fois en endodontie et en chirurgie. Récemment, l'utilisation de guide chirurgicaux a été proposée afin de sécuriser et de simplifier la technique opératoire. Les principes de réalisations de ces guides dérivent des travaux réalisés en implantologie et en orthodontie grâce au flux numérique et à la Conception Fabrication Assistée par Ordinateur. L'objectif de cette thèse était de réaliser une étude pilote, évaluant la précision de ce type de guide pour la microchirurgie endodontique. Nous avons choisi d'utiliser un modèle animal ex-vivo (modèle porcin) permettant de mimer les procédures chirurgicales humaines. Le guide de microchirurgie endodontique a été réalisé après planification, modélisation et impression tridimensionnelle. Afin de quantifier la précision de ce guide par rapport à la modélisation virtuelle, une tomographie à faisceau volumique a été réalisée après ostéotomie. Les résultats ont été obtenus à partir de mesures réalisées sur des logiciels de traitement et d'analyse d'image. Le forage réalisé était situé plus mésial et apical que la planification. Les axes entre les deux étaient convergents dans le sens coronoapical et parallèles dans le sens mésio distal. Les déviations mesurées variaient de 0.05mm à 0.29mm. La différence moyenne mesurée était de 0.195 mm. Dans cet essai, le contrôle de profondeur de forage était précis à 0,03mm près. Cette étude a mis en évidence un certain nombre de biais et de limites, ce qui nous a permis de proposer un protocole modifié et optimisé afin de réaliser une étude sur un échantillon de plus grande ampleur. Ces travaux s'inscrivent dans une démarche d'évaluation des nouvelles technologies appliquées au cabinet dentaire.
