Book excerpt: This book looks at experience in government restructuring and devolution from a variety of national and international perspectives, both within the European Union and elsewhere, focusing on lessons learned and ways forward.Since the 1980s, there has been a global trend to give more power to local governments. Even in Korea and the United Kingdom, the most centralised countries in the OECD, local government powers have increased, with substantial economic benefits. Within the European Union, the principle of subsidiarity has enshrined the idea of devolution. New member states, particularly in central and eastern Europe, have significantly created new and self-sufficient local and regional governments. However, this process has been complicated. Devolution is not a panacea in its own right, and need not lead to economic growth. While it can encourage savings through collaboration, it can also lead to confused lines of authority and can complicate policy formation and implantation. Devolution can strain local budgets, forcing local governments to rely on their own sources of finance, rather than central government transfers. Suburbanisation, rural depopulation, the growth of some regions, and the decline of others have raised new problems, particularly related to inter-governmental cooperation among local governments and different levels of government. In many cases, an increased number of governments has increased administrative costs.

They expand the discussion of personalised medicine beyond its usual scope to include many other highly topical issues, including: human nuclear genome transfer ('three-parent IVF'), stem cell-derived gametes, private umbilical cord blood banking, international trade in human organs, biobanks such as the US Precision Medicine Initiative, direct-to-consumer genetic testing, health and fitness self-monitoring. Although these technologies often prioritise individual choice, the original ideal of genomic research saw the human genome as 'the common heritage of humanity'. The authors question whether personalised medicine actually threatens this conception of the common good.

Network Infrastructure and the Urban Environment is a policy area of growing importance. Sustainable cities and sustainable transport systems are necessary for attaining a sustainable development. The research and policy field, represented in this volume, comprises a number of challenging contrasts: - the contrast between infrastructure investments, mobility and environmental sustainability; - the contrast between policy contexts, modelling traditions and available decision support systems in various parts of the world; - the contrast between available best practice methods and the majority of models applied in planning; the contrast between static models of cross-sectionary equilibria and dynamic models of disequilibrium adjustments; and the contrast between state-of-the-art operationalland-use/transport models and new demands for land-use/transportlenvironment models due to changing policy contexts. Bridging some of these gaps constitutes important research tasks, that are discussed in the twenty-two chapters of this book. Due to the prevalent drug resistance of these strains and lack of antimicrobial alternatives, the treatment of infections caused by these MDR Gram-negative pathogens rely on tigecycline, polymyxins (colistin and polymyxin B) or new β-lactam/β-lactamase inhibitor combinations (such as ceftazidime-avibactam, CAZ/AVI). However, the critically ill patients with antimicrobial infection are associated with high mortality. Hence, the prevalence and optimal therapy for critically ill patients with antimicrobial resistance are worth further study.
