Older adults accumulate large amounts of ST and low amounts of physical activity, which may compound the health risks. There is little research examining strategies to reduce ST among older adults, especially within assisted living residences (ALRs). The purpose of this study was to develop and pilot-test an evidence-informed intervention to reduce ST within ALRs. Stand to Strengthen is a flexible, multi-level intervention that can be delivered in a variety of ALRs. A 6-week feasibility trial using Stand to Strengthen was conducted to examine changes in function, quality of life (QoL), and ST patterns. There was a trend towards less self-reported weekday ST post intervention (p = 0.086) but no significant differences in other variables overall or for higher function participants. Lower function participants demonstrated significant, clinically meaningful improvements in physical function post-intervention (p=0.035, Cohen's d = 0.89). This study demonstrated that a multi-level intervention to reduce ST may delay functional declines as older adults transition to assisted living.

Book excerpt: Sedentary behavior has emerged as a major public health threat because accumulating evidence links excessive sitting with deleterious health consequences, independent of physical activity. On average, older adults spend more than 60% of their waking hours each day sitting -- the most out of any segment of the population. Understanding the factors that contribute to this high volume health behavior and its consequences will facilitate the development of effective interventions to reduce sedentary time that also promote healthy and successful aging. This dissertation aims to (1) identify the motivational processes that contribute to sedentary behavior, (2) test relations between sedentary behavior and life satisfaction -- an important indicator of well-being and successful aging in older adults, and (3) evaluate the feasibility, acceptability, safety, and preliminary efficacy of a brief psychoeducational intervention designed to reduce older adults' sedentary behavior. A 14-day ecological momentary assessment study with ambulatory monitoring was conducted to capture time-varying and time-invariant motivation, behavior, and evaluations among older adults (n = 100). These data were used to test hypotheses about the motivational antecedents of daily sedentary behavior and hypotheses about relations between daily sedentary behavior and life satisfaction. Next, a brief psychoeducational intervention to reduce older adults' sedentary behavior was developed and implemented at local senior centers (n = 5). This intervention was evaluated in terms of feasibility, acceptability, safety, and preliminary efficacy. Results of these studies led to five major conclusions. First, older adults' sedentary behavior is regulated by both controlled and automatic motivational processes. Second, both between-person and within-person motivational processes were associated with sedentary behavior. Third, sedentary behavior and life satisfaction were negatively linked through within-person processes. Fourth, older adults found the psychoeducational intervention approach to be engaging. Finally, the intervention reduced older adults' self-reported weekday sedentary behavior by approximately two hours more per day compared to the control group. Ultimately, this dissertation informs the development of interventions designed to reduce older adults' sedentary behavior to promote healthy and successful aging. Prioritizing healthy aging interventions of this nature may reduce health care costs associated with treatment and management of chronic diseases and improve care for older adults.

However, the factors that influence movement patterns in older adults, especially for those residing in the growing assisted living (AL) setting, are poorly understood. I acquired measures from 114 AL tenants of movement patterns from waist-mounted accelerometers worn for at least 3 days. On average, participants spent 86% of their waking hours in sedentary behaviour and 13.84% in light physical activity. The time spent sedentary was higher in males than females, and correlated with scores on the Timed-Up-and-Go and Modified Fall Efficacy Scale, but not with the Montreal Cognitive Assessment or Geriatric Depression Scale. These results indicate that both physical function and psychological factors influence sedentary behaviour in AL tenants. Future research should examine whether interventions targeted at intrinsic or environmental factors decrease sedentary behaviours.

Book excerpt: Background: Sedentary behavior (SB) is associated with impaired physical function, frailty, falls, and higher mortality in older adults, the most sedentary age demographic in the US. There is limited research on the efficacy of interventions to reduce SB and improve physical function and mobility in older adults of moderate-to-low physical function. Purpose: To examine the efficacy of a 12-week intervention, Stand Up Now (SUN), to reduce SB and improve physical function and mobility in older adults of moderate-to-low physical function in Continuing Care Retirement Communities (CCRCs). Methods: SUN was conducted in four CCRCs that were randomized to two intervention groups with different SB change goals: one group focused on reducing total sedentary time (SUNSL) and one group focused on increasing sit-to-stand (STS) transitions (SUNSTS). Participants (N = 71; Mage=87±7yrs) in both SUN groups had weekly health coaching sessions over 12 weeks. SB, physical function, and mobility were measured at baseline, 6, and 12 weeks via the activPAL, Short Physical Performance Battery (SPPB), and the 8-foot up-and-go (8ft UG), respectively. Linear Mixed Models (LMM) were used to examine the efficacy of SUN on activPAL measured variables (i.e., sitting, standing, STS transitions, stepping, and steps), physical function, and mobility at 6 and 12 weeks. Results: Both SUN groups significantly decreased sedentary time (1.3±0.3hrs, p0.001) and increased standing time (0.5±0.2hrs, p0.02) at 6 weeks that was maintained at 12 weeks, compared to their baseline. At workplaces, managers and companies should encourage sitting breaks and work standing options, and for the special population such as children or older adults, new strategies must be considered to reduce sitting time.
