Available in PDF, EPUB and Kindle. Book excerpt: Reconstructive Surgery of the Esophagus allows the reader to take a fresh look at a subject that is mired in history and convention. The result is an honest examination of current dogma, providing a basis for rational decision making. The author makes use of a wide variety of both historical and contemporary original source materials, offering the reader a complete overview of reconstructive esophageal surgery. In addition to copious evidence-based information, the author shares some of his valuable experience-based observations to help guide the practitioner through details of complex procedures, avoid common problems, and manage complications when they occur. This book is organised to permit easy access to detailed information on all aspects of esophageal reconstruction, and is liberally illustrated with original line drawings that depict the relevant anatomy and steps associated with each procedure. Because the esophagus and related structures traverse three anatomic regions, reconstructive surgery requires knowledge of a host of different and sometimes arcane aspects of physiology and anatomy. General thoracic surgeons, general surgeons, cardiothoracic surgeons, head and neck surgeons, and plastic/reconstructive surgeons will all benefit from this well-organised succinct text.

Available in PDF, EPUB and Kindle. Book excerpt: While a surgeon many decades agotreated diseases from headtotoe, this concept has evolved, and today some degree of specialization is the rule worldwide. In many countries various boards for sub-specializations are designed, and after a broad training in general surgery, many young surgeons move on further into a specific field. Knowledge of anatomy and precise surgical technique remain the foundation of high quality surgery. A knowledgeable surgeon, equipped with excellent theore- cal and clinical skills, will only be accomplished when he or she masters the - erative techniques of the practice of surgery. The legacy of an academic surgeon or a surgical teacher relies in great part onthe transmission of his or her surgical abilities. Significant influence on the development in esophageal surgery arises from the surgical understanding of the anatomical and functional structures of the esophagus as well as the ongoing developments in the multidisciplinary m- agement in this challenging field of surgery. In bringing forth this atlas, we were motivated by the desire to create a comp- hensive and educational atlas on esophageal diseases, emphasizing all details of pathophysiologies, diagnostic strategies, pre- and postoperative management, and operative techniques covering minimally invasive and open procedures ranging fromstraightforward tomore complex procedures.

It also provides recommendations on obtaining high patient satisfaction and building relationships with referring physicians. It is intended as an “expert” level manual to help readers move to the next level in their practices. From patient assessment and selection, anticipation of perioperative challenges, intraoperative problem-solving and technique, and postoperative management of side-effects, to reducing long-term recurrence rates and resumption of antisecretory medication, these expert authors dig deep to share the pearls of wisdom that make the difference between the average surgeon and the expert. Written by experts in the field, Fundoplication Surgery: A Clinical Guide to OptimizingResults confronts the issues that have hindered the broader use of fundoplication nationally and is a valuable resource and reference guide for addressing ambiguous symptoms and test results in one’s practice.

This is especially true in esophageal surgery owing to difficulties in interpreting pathophysiology, staging malignant tumors with accuracy, and standardizing and tailoring the available surgical procedures to the individual patient. This book introduces the general and the thoracic surgeon to the latest developments in esophageal surgery; in each chapter the reader will find a concise background analysis of the topic in question and a state of the art review of diagnosis and treatment. The indications for surgery and the rationale for use of an innovative technique or technology are discussed in detail. Challenging and provocative questions and comments by selected panelists complement each chapter, enhancing the quality of information. Innovation in Esophageal Surgery will be highly informative for both the novice and the expert surgeon wishing to enter the arena of esophageal surgery. A multidisciplinary team of experts offers an evidence-based approach and critical analysis: endoscopic, surgical, and hybrid surgical techniques that will soon change the approach to esophageal disease are described in detail.

Available in PDF, EPUB and Kindle. Book excerpt: Esophageal cancer causes an estimated 386,000 deaths worldwide and is the sixth most common cause of death for men. The background characteristics of esophageal cancer treatment are markedly different between Asian and Western countries, however. In tumor histology, squamous cell carcinoma associated with smoking and alcohol consumption is overwhelmingly prevalent in Asia, whereas adenocarcinoma associated with Barrett’s metaplasia is markedly prevalent in the West. In Asia, especially in Japan, the key persons who play important roles in the management of esophageal cancer patients are surgeons; in the West those roles are filled by medical and radiation oncologists as well as surgeons. The philosophy of surgeons regarding cancer surgery varies from locoregional to local tumor control, particularly in focusing on lymph node dissection. Physicians’ approach to surgical adjuvant therapy differs, therefore, between Asia and the West. Considering these East–West differences in esophageal cancer treatment, the currently available results of Western evidence should not be considered directly applicable to esophageal cancer in Asia. In this book, the authors discuss the knowledge base in Japan in terms of treatment of esophageal squamous cell carcinoma. Since this volume contains a wide spectrum of current information and addresses topics surrounding the treatment of patients with esophageal squamous cell carcinoma, it is highly relevant to Asian physicians and researchers as well as to their counterparts in the West.
