This book was released on 1997-03-31 with total page 376 pages. Available in PDF, EPUB and Kindle. Book excerpt: While joint replacements at the lower extremity, particularly at the hip and knee, are performed on a daily basis in most departments of Orthopaedics, with great clinical success and long longetivity of the implant, the indications are much less frequent at the upper limb. Basic research follows the same tendency: while there are numerous basic or clinical research projects devoted to the hip or knee, very scarce reports are published on upper extremity joint replacements. The aims of publishing this volume are to promote the exchange of ideas, and to forster collaborative investigations among clinicians, manufacturers, engineers and other basic scientists involved with the problems of replacement arthroplasty in upper extremity joints, from the finger to the shoulder. The specific aims are: (1) to summarize scientific knowledge in the area of upper extremity joint replacement; (2) to discuss currently unsolved clinical problems and potential solutions based on current scientific knowledge; (3) to review new prosthetic designs; and (4) to explore future directions of investigation. In the first section, general concepts of arthroplasty and of upper extremity reconstruction are presented. Ronald Linscheid presents a short history of finger joint replacement. Alain Potaznik and Franz Burny discuss the problems of friction, wear and biological responses after the implantation of artificial joint replacement. Laurie Faro presents the European legislation for medical devices, such as arthroplasties. Finally, Frédéric Schuind and Franz Burny review the indications and contraindications to upper extremity arthroplasty. The following sections discuss for each upper extremity joint the indications of joint replacement, including the most frequent diseases, the prostheses designed for that particular joint, the biomechanical evaluation after prosthetic implantation, and the early or long-term clinical results. We made a special effort to include at the end of each section some alternative procedures of reconstruction. We are convinced that this unique book fills an important gap in the medical literature.

Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this book was to offer an overview of recent insights into the current state of arthroplasty. The tremendous long term success of Sir Charnley's total hip arthroplasty has encouraged many researchers to treat pain, improve function and create solutions for higher quality of life. Indeed and as described in a special chapter of this book, arthroplasty is an emerging field in the joints of upper extremity and spine. However, there are inborn complications in any foreign design brought to the human body. First, in the chapter on infections we endeavor to provide a comprehensive, up-to-date analysis and description of the management of this difficult problem. Second, the immune system is faced with a strange material coming in huge amounts of micro-particles from the tribology code. Therefore, great attention to the problem of aseptic loosening has been addressed in special chapters on loosening and on materials currently available for arthroplasty.

Available in PDF, EPUB and Kindle. Book excerpt: Arthroplasty of the upper extremity is an established surgical intervention in the management of arthritis of the elbow, wrist and hand. The anatomy, kinematics and demands of the elbow, wrist, thumb CMC, and finger MCP and PIP joints pose unique surgical challenges. Implant design considerations are important in providing a joint that mimics the native joints and maximizes survivorship. However, outcomes are less predictable in these upper extremity joints when compared to the hips and knees. Each joint also carries its own set of potential complications and salvage options for revision and failed arthroplasty. This unique text helps the orthopedic and hand surgeon understand the surgical approaches, unique anatomic considerations, and both the historical and current designs related to each respective joint, enabling the surgeon to better appreciate the benefits and limitations of each arthroplasty. Presenting the current state of the art, the seven sections proceed anatomically from the elbow to the fingers, with each section comprised of three thematic chapters discussing implant design considerations, primary arthroscopy techniques and revision arthroscopy techniques, including non-surgical options for treating these often difficult problems. This consistent approach, accompanied by plentiful figures, radiographs and intraoperative photos, ensures that this will be a user-friendly resource for orthopedic and hand surgeons, residents and trainees.

Book excerpt: The U.S. Census Bureau has reported that 56.7 million Americans had some type of disability in 2010, which represents 18.7 percent of the civilian noninstitutionalized population included in the 2010 Survey of Income and Program Participation. The U.S. Social Security Administration (SSA) provides disability benefits through the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. As of December 2015, approximately 11 million individuals were SSDI beneficiaries, and about 8 million were SSI beneficiaries. SSA currently considers assistive devices in the nonmedical and medical areas of its program guidelines. During determinations of substantial gainful activity and income eligibility for SSI benefits, the reasonable cost of items, devices, or services applicants need to enable them to work with their impairment is subtracted from eligible earnings, even if those items or services are used for activities of daily living in addition to work. In addition, SSA considers assistive devices in its medical disability determination process and assessment of work capacity. The Promise of Assistive Technology to Enhance Activity and Work Participation provides an analysis of selected assistive products and technologies, including wheeled and seated mobility devices, upper-extremity prostheses, and products and technologies selected by the committee that pertain to hearing and to communication and speech in adults.

Book excerpt: Advances made in microsurgical techniques during the last three decades have dramatically improved the treatment of complex limb injuries, such as a closed fracture covered by healthy tissue and a complex open fracture of the tibia. Microvascular transfers from the foot have revolutionized the treatment of digital amputations, and vascularized bone transfers are used in various indications. The microvascular techniques are still evolving. Many imaginative new flaps are being proposed to meet precisely the functional requirements of the various defects. Alternative anastomotic techniques are being investigated and the no-flow phenomenon has been extensively studied. Allotransplantation may be feasible in the future. This unique book is a result of the exchange of ideas among clinicians of various specialties noted for their innovations and basic researchers involved with the problems of microvascular reconstruction of upper and lower extremities. While most of the chapters are invited contributions, some are based on presentations made during an international symposium held in Brussels, Belgium, in 1997.
