The report cited problems with CSB¿s governance, mgmt., policies, and procedures. It was recommended that CSB obtain the services of an existing office of Inspector General (IG). Since FY 2001, 3 IGs -- from FEMA, DHS, and EPA -- have provided oversight to CSB. This report examined: (1) how CSB has responded to the recommendations regarding CSB¿s investigative gap, data quality problems, human capital problems, and accountability and mgmt. problems for meeting its mission requirements; and (2) the merits of the current oversight approach using an existing office of IG and other approaches. This book was released on 2014-08-01 with total page 114 pages. Available in PDF, EPUB and Kindle. Book excerpt: On November 22, 2006, at about 2:45 am, a violent explosion at the CAI/Arnel manufacturing facility rocked the town of Danvers, MA. The explosion and subsequent fire destroyed the facility, heavily damaged dozens of nearby homes and businesses, and shattered windows as far away as two miles. At least 10 residents required hospital treatment for cuts and bruises. Twenty-four homes and six businesses were damaged beyond repair. Dozens of boats at the nearby marina were heavily damaged by blast overpressure and debris strikes. The fire department ordered the evacuation of more than 300 residents within a half-mile radius of the facility. Numerous residents could not return for many months while they waited for their houses to be rebuilt or repaired. Seventeen months after the explosion, six homes had yet to be reoccupied as repairs were not complete. The U.S. Chemical Safety and Hazard Investigation Board (CSB) determined that the explosion was fueled by vapor released from a 2000-gallon tank of highly flammable liquid. An open steam valve on the tank heater most likely caused the flammable liquid to overheat and accumulate in the building production area to what is calculated to have been a near-ideal vapor-air concentration. An unknown ignition source ignited the flammable atmosphere, causing the explosion. The rapidly expanding ignited vapor inside the building created a pressure wave that shattered the rigid, brittle brick walls—disintegrating the structure—and ignited thousands of gallons of flammable liquids stored inside the building and some 51,000 pounds of industrial-grade nitrocellulose material stored nearby. The resultant fire burned for more than 17 hours. On November 22, 2006, at about 2:45 am, a violent explosion at the CAI/Arnel manufacturing facility rocked the town of Danvers, MA. Available in PDF, EPUB and Kindle. Book excerpt: Even several years after the terrorist attacks of September 11, 2001, many organizations are ill-prepared to deal with crises, often opting to deal with them only after the fact. In Code Red in the Boardroom, Tim Coombs argues that crisis management should be a variety of activities that the organization performs daily to prevent crises from occurring. He defines the types of crises an organization might experience (both internal and external), draws from a wide variety of case examples, and showcases cutting-edge techniques that are being tested in the public and private sectors to demonstrate how crisis management can be hardwired into the corporate DNA, so that sensing, preventing, and responding quickly to crises become everyone's responsibility. In the process, he explores evolving roles for executives, managers, and front-line employees in communicating and implementing crisis plans. Ultimately, the book shows readers how proactive crisis management makes the company stronger, more resilient, and adaptable to change. A glossary of key terms and templates for establishing a crisis management program make this book an essential resource for all organizations. Even several years after the terrorist attacks of September 11, 2001, many organizations delude themselves about crisis management. Some enterprises, especially smaller ones, still believe that a crisis cannot happen to them. Others have gone through the steps of creating a crisis management plan, but really pay no more than lip service to the program, and may, in fact, be creating a false sense of security that leaves the company even more vulnerable to attack, accident, crime, or other sources of crisis. Tim Coombs argues that crisis management should not just be something you do when a crisis hits. It should be a variety of activities that the organization performs daily to prevent crises from ocurring. In Code Red in the Boardroom, Coombs defines the types of crises an organization might experience (both internal and external), draws from a wide variety of case examples, and showcases cutting-edge techniques that are being tested in the public and private sectors to demonstrate how crisis management can be hardwired into the corporate DNA—so that sensing, preventing, and responding quickly to crises become everyone's responsibility.
