La recommandation du Collège National pour la Qualité des Soins en Psychiatrie (CNQSP) propose la standardisation du contenu des échanges par courrier entre MG et psychiatres lors d'un premier adressage afin d'améliorer cette communication. Méthode : Il s'agit d'une étude qualitative où les psychiatres publics lillois ont pu échanger des courriers-types avec 70 MG pendant 6 mois. Ces courriers étaient inspirés de la recommandation du CNQSP. A son terme, 11 psychiatres ont pu donner leur ressenti concernant ces courriers-types lors d'un entretien semi-dirigé. Résultats : Peu de courriers-types ont été échangés. Les psychiatres expliquent ce phénomène comme une conséquence de l'exhaustivité du courrier, ayant impressionné les MG manquant de temps pour la rédaction. Les psychiatres incriminent également l'ambivalence des MG vis-à-vis de la pratique de la psychiatrie, influençant la collaboration ultérieure avec le psychiatre. La qualité de l'adressage apparaît importante dans la poursuite de la collaboration avec le MG. Le courrier-type de réponse au MG divise, telle la recommandation du CNQSP que peu connaissaient: il constitue pour certains psychiatres une base d'échange pertinente avec le MG, pour d' autres il ne s'agit que d'une contrainte administrative chronophage supplémentaire. Conclusion: Le courrier-type apparaît comme un élément parmi d'autres pouvant améliorer la communication entre MG et psychiatres, tous les autres moyens de communication pouvant être mis à contribution selon la situation du patient. Il serait nécessaire d'améliorer en amont la formation des MG afin de renforcer leurs connaissances en psychiatrie et de son fonctionnement sectoriel. Les rencontres entre professionnels d'un même secteur pourraient aider à retisser un réseau local afin d'améliorer la collaboration.
