Book excerpt: Preface During the past two decades, a significant international research exchange has taken place in order to understand the pathobiology of corneal diseases, and to develop more sophisticated treatments. For this purpose, many Japanese cornea specialists have been abroad to the United States, the United Kingdom, etc., to perform collaborative works in basic and clinical cornea research. As a result, many original concepts and thoughts have been produced from Japan, and some of them are now accepted world wide. Further to promote the international exchange of current knowledge and research on corneas in both health and disease, the First Annual Meeting of the Kyoto Cornea Club was held in Kyoto City on December I and 2, 1995. The goal of this meeting was to provide each speaker's updated knowledge that would be of value to all active corneal researchers who are interested in 'state-of-the-art' information from Japan. To achieve this objective fully, 41 selected Japanese cornea specialists registered as active participants in this meeting. James D. Zieske, PhD, from Schepens Eye Research Institute and the Department of Ophthalmology at Harvard Medical School, Boston, MA, USA, and Tadatsugu Taniguchi, PhD, from the Department of Immunology, Faculty of Medicine, Tokyo University, Japan, participated as special guests at this meeting, and both gave plenary lectures. This volume was created to provide each speaker's current opinions on the subjects they presented at this meeting, and also to provide a scientific reference for cornea research. We hope that the first issue will serve as a useful source of current knowledge and insight on Japanese cornea research. In fact, increasing resistance to these drugs is already exacting a terrible price. Every day in the United States, approximately 172 men, women, and children die from infections caused by antibiotic-resistant bacteria in hospitals alone. Beyond those deaths, antibiotic resistance is costing billions of dollars through prolonged hospital stays and the need for doctors to resort to ever more costly drugs to use as substitute treatments. Extending the Cure presents the problem of antibiotic resistance as a conflict between individual decision makers and their short-term interest and the interest of society as a whole, in both present and future: The effort that doctors make to please each patient by prescribing a drug when it might not be properly indicated, poor monitoring of discharged patients to ensure that they do not transmit drug-resistant pathogens to other persons, excesses in the marketing of new antibiotics, and the broad overuse of antibiotics all contribute to the development and spread of antibiotic-resistant bacteria. The book explores a range of policy options that would encourage patients, health care providers, and managed care organizations to serve as more responsible stewards of existing antibiotics as well as proposals that would give pharmaceutical firms greater incentives to develop new antibiotics and avoid overselling. Book excerpt: When the eyeball is indented in total darkness, within less than 200 mil liseconds an oval or quarter-moon shaped spot of light is perceived in the part of the visual field corresponding to the indented region of the retina. In the seconds following, this phosphene extends across the whole visual field and alters in structure during further eyeball indentation. It is then seen as irregular large bright spots of light, finely structured moving light grains ('light nebula') and stationary bright stars. Regular geometrical patterns appear only when both eyes are indented simultaneously [1]. When the eyeball deformation is released, part of the retina again lights up for another one or two seconds and curved light lines are seen following the course of the larger retinal vessels (Fig. 1). In the following we will review the history of this phenomenon, which played an important role during the first 2200 years of vision theories and in the development of models to explain normal vision. 2. Pre-Socratic philosophers, Plato and Aristotle Alcmaeon of Croton (6-5th century B. C. ), who was a member of the Pythagoraean sect and one of the founders of Greek medicine, was the first to describe mechanical deformation of the eyeball leading to light sensa tions. According to Aristotle's pupil Theophrast of Eresos, Alcmaeon report ed that 'the eye obviously has fire within,for when the eye is struckfireflashes out' [2, p. 88].
