Book excerpt: Various NASA Langley Research Center and other center projects were attempted for analysis to obtain historical data comparing pre-phase A study and the final outcome for each project. This attempt, however, was abandoned once it became clear that very little documentation was available. Next, extensive literature search was conducted on the role of risk and reliability concepts in project management. Probabilistic risk assessment (PRA) techniques are being used with increasing regularity both in and outside of NASA. The value and the usage of PRA techniques were reviewed for large projects. It was found that both civilian and military branches of the space industry have traditionally refrained from using PRA, which was developed and expanded by nuclear industry. Although much has changed with the end of the cold war and the Challenger disaster, it was found that ingrained anti-PRA culture is hard to stop. Examples of skepticism against the use of risk management and assessment techniques were found both in the literature and in conversations with some technical staff. Program and project managers need to be convinced that the applicability and use of risk management and risk assessment techniques is much broader than just in the traditional safety-related areas of application. The time has come to begin to uniformly apply these techniques. The whole idea of risk-based system can maximize the 'return on investment' that the public demands. Book excerpt: Probabilistic Risk Assessment (PRA) is a comprehensive, structured, and logical analysis method aimed at identifying and assessing risks in complex technological systems for the purpose of cost-effectively improving their safety and performance. NASA's objective is to better understand and effectively manage risk, and thus more effectively ensure mission and programmatic success, and to achieve and maintain high safety standards at NASA. NASA intends to use risk assessment in its programs and projects to support optimal management decision making for the improvement of safety and program performance. In addition to using quantitative/probabilistic risk assessment to improve safety and enhance the safety decision process, NASA has incorporated quantitative risk assessment into its system safety assessment process, which until now has relied primarily on a qualitative representation of risk. Also, NASA has recently adopted the Risk-Informed Decision Making (RIDM) process. Over the years, NASA has been a leader in most of the technologies it has employed in its programs. One would think that PRA should be no exception. In fact, it would be natural for NASA to be a leader in PRA because, as a technology pioneer, NASA uses risk assessment and management implicitly or explicitly on a daily basis. NASA has probabilistic safety requirements (thresholds and goals) for crew transportation system missions to the International Space Station (ISS). NASA intends to have probabilistic requirements for any new human spaceflight transportation system acquisition.as a valuable addition to supplement existing deterministic and experience-based engineering methods and tools. During the past several decades, much has been written on PRA methods and applications. Several university and practitioner textbooks and sourcebooks currently exist, but they focus on applications of PRA to industries other than aerospace. Although some of the techniques used in PRA originated in work for aerospace and military applications, no comprehensive reference currently exists for PRA applications to aerospace systems. This PRA Procedures Guide, in the present second edition, is neither a textbook nor an exhaustive sourcebook of PRA methods and techniques. It provides a set of recommended procedures, based on the experience of the authors, that are applicable to different levels and types of PRA that are performed for aerospace applications. It therefore serves two purposes, to: 1. Complement the training material taught in the NASA PRA course for practitioners, and together with the Fault Tree Handbook, the Risk-Informed Decision Making Handbook, the Bayesian Inference handbook, the Risk Management Handbook, and the System Safety Handbook to provide quantitative risk methodology documentation, and to provide aerospace PRA practitioners in selecting an analysis approach that is best suited for their applications.

This tragic event led, as the Challenger accident had 17 years earlier, to an intensive government investigation of the technological and organizational causes of the accident. The year was marked by significant achievements in the Space Shuttle and International Space Station (ISS) programs and encouraging accomplishments by the Aerospace Technology Enterprise. Unfortunately, there were also disquieting mishaps with the X-43, a LearJet, and a wind tunnel. Each mishap was analyzed in an orderly process to ascertain causes and derive lessons learned. Both these accomplishments and the responses to the mishaps led the Panel to conclude that safety and risk management is currently being well served within NASA. NASA's operations evidence high levels of safety consciousness and sincere efforts to place safety foremost. Nevertheless, the Panel's safety concerns have never been greater. This dichotomy has arisen because the focus of most NASA programs has been directed toward program survival rather than effective life cycle planning. Last year's Annual Report focused on the need for NASA to adopt a realistically long planning horizon for the aging Space Shuttle so that safety would not erode. NASA's response to the report concurred with this finding. Nevertheless, there has been a greater emphasis on current operations to the apparent detriment of long-term planning. Budget cutbacks and shifts in priorities have severely limited the resources available to the Space Shuttle and ISS for application to risk-reduction and life-extension efforts. As a result, funds originally intended for long-term safety-related activities have been used for operations. Thus, while safety continues to be well served at present, the basis for future safety has eroded. Section II of this report develops this theme in more detail and presents several important, overarching findings and recommendations that apply to many if not all of NASA's programs. Seven months after the accident, the Columbia Accident Investigation Board (CAIB) released the first volume of its final report, citing a variety of technical, managerial, and cultural issues within NASA and the Space Shuttle Program. To their credit, NASA offered few excuses, embraced the report, and set about correcting the deficiencies noted by the accident board. Of the 29 recommendations issued by the CAIB, 15 were deemed critical enough that the accident board believed they should be implemented prior to returning the Space Shuttle to flight. Some of these recommendations were relatively easy, most were straightforward, a few bordered on the impossible, and others were largely overcome by events, particularly the decision by the President to retire the Space Shuttle by 2010. The Return to Flight Task Group (RTF TG, or simply, the Task Group) was chartered by the NASA Administrator in July 2003 to provide an independent assessment of the implementation of the 15 CAIB return-to-flight recommendations. An important observation must be stated up-front: neither the CAIB nor the RTF TG believes that all risk can be eliminated from Space Shuttle operations; nor do we believe that the Space Shuttle is inherently unsafe. What the CAIB and RTF TG do believe, however, is that NASA and the American public need to understand the risks associated with space travel, and that NASA must make every reasonable effort to minimize such risk. Since the release of the CAIB report, NASA and the Space Shuttle Program expended enormous effort and resources toward correcting the causes of the accident and preparing to fly again. Relative to the 15 specific recommendations that the CAIB indicated should be implemented prior to returning to flight, NASA has met or exceeded most of them the Task Group believes that NASA met the intent of the CAIB for 12 of these recommendations. A formal and, at that time, qualitative RM process known as Continuous Risk Management (CRM) was introduced to NASA in the latter half of the 1990s. More rigorous quantitative RM processes including Risk-Informed Decision Making (RIDM) and an enhanced version of CRM have only recently been developed for implementation as an integral part of systems engineering at NASA. While there will probably always be vigorous debate over the details of what comprises the best approach to managing risk, few will disagree that effective risk management is critical to program and project success and affordability. Since their introduction and until recently, NASA RM processes have been based on CRM, which stresses the management of risk during the Implementation phase of the NASA Program Project Life Cycle. In December of 2008, NASA issued NPR 8000.4A, which introduced RIDM as a complementary process to CRM that is concerned with analysis of important and or direction-setting decisions. In the past, RM was considered equivalent to CRM; now, RM is defined as comprising both CRM and RIDM. In April 2010, NASA issued NASA SP-2010-576, the NASA Risk-Informed Decision Making Handbook. This handbook introduced RIDM as the front-end of the RM process, described the details of how RIDM is conducted, and ended with a description of how the results of RIDM transition to and set the stage for CRM, the final portion of the RM process. The RIDM Handbook did not proceed to describe CRM, as the development of an enhanced version of CRM was still a work in progress in 2010. Now this handbook addresses the entirety of the NASA RM process, including both RIDM and CRM. The purpose of this handbook is to provide guidance for implementing the Risk Management (RM) requirements of NASA Procedural Requirements (NPR) document NPR 8000.4A, Agency Risk Management Procedural Requirements, with a specific focus on programs and projects, and applying to each level of the NASA organizational hierarchy as requirements flow down. This handbook supports RM application within the NASA systems engineering process, and is a complement to the guidance contained in NASA SP-2007-6105, NASA Systems Engineering Handbook. Specifically, this handbook provides guidance that is applicable to the common technical processes of Technical Risk Management and Decision Analysis established by NPR 7123.1A, NASA Systems Engineering Process and Requirements. These processes are part of the Systems Engineering Engine that is used to drive the development of the system and associated work products to satisfy stakeholder expectations in all mission execution domains, including safety, technical, cost, and schedule.
