Book excerpt: INTRODUCTION : En 2018, la publication de la Survival Sepsis Campaign « Hour-1 bundle » détaille cinq recommandations de bonne pratique clinique à mettre en œuvre dans l'heure suivant l'arrivée du patient. L'objectif principal de notre étude est d'apprécier l'adhésion des praticiens urgentistes aux recommandations de la Surviving Sepsis Campaign de 2018. MATERIELS ET METHODES : Il s'agit d'une étude observationnelle mono-centrique rétrospective d'évaluation des pratiques professionnelles. Nous avons inclus sur une période de janvier 2020 à janvier 2022, les patients admis en réanimation et passés par le SAU dans les 48 heures précédentes, dont le score SOFA est > 2 et pour lesquels on retrouve le codage « sepsis » ou « choc septique » sur le courrier de sortie. RESULTATS : Sur la période concernée, 54 patients (dont 32 hommes, âge médian de 68 ans) ont été inclus, dont 68 % de choc septique et 32% de sepsis. La mortalité hospitalière a été de 47%. L'étiologie probable du sepsis était majoritairement urinaire dans 33%, pulmonaire dans 22%, % et digestifs dans 20%. L'adhésion à l'ensemble des recommandations était de 3,7%. Seuls 3 patients ont pu bénéficier à postériori d'un calcul du score qSOFA, ce qui met en lumière que le dépistage du sepsis dès le triage infirmier pourrait être nettement amélioré. Le non-respect des recommandations concernait : l'absence de recontrôle du lactate sanguin (11%), la prescription d'un support catécholinergique (35%) et l'antibiothérapie probabiliste dans l'heure (9%). L'adhésion aux recommandations dans les 3 heures devient conforme dans 38% des cas et en s'affranchissant du facteur temps, l'adhésion globale aux recommandations est de 58%. LIMITES : Les résultats de cette étude sont à considérer en tenant compte des biais de celle-ci : l'étude est rétrospective , l'inclusion des patients ne s'est faite qu'à partir des dossiers codés avec un code CIM-10 de « sepsis » ou « choc septique » uniquement sur la base des comptes rendus de sortie de réanimation ; les heures ou délais d'administration ou de prélèvement n'étaient pas renseignés de manière exacte, en effet, seules les heures et les dates des signatures informatiques apparaissaient, pouvant de ce fait fortement affecter les résultats finaux de l'étude . CONCLUSION : L'adhésion à l'ensemble des recommandations pour la prise en charge du sepsis était faible. Il semble de ce fait nécessaire de poursuivre la sensibilisation des soignants, d'intensifier leur formation, afin d'évaluer l'amélioration des pratiques à moyen ou long terme.
