Book excerpt: We welcome the publication of this volume, which discusses the diagnosis of bone tumours with particular reference to children and adolescents. As founder members of the International Skeletal Society we are delighted to learn that the book had its inception at one of the Society's meetings. It reflects, moreover, the combined presentation of radiological and pathological diagnostic information which has been such a feature of the meetings of the International Skeletal Society. We commend it to all readers with an interest in tumours of the skeleton. Hubert A. Sissons Ronald O. Murray Preface The diagnosis of primary bone tumors is often difficult. There are several reasons for this. As primary bone tumors are rare in childhood, practitioners in a number of pediatric subspecialties are not familiar with them. The clinical symptoms and signs are often elusive, the biochemical investigations usually normal and the radio graphic features often uncharacteristic. Even the pathologist, who is the final step in arriving at the proper diagnosis and who has all the available clinical, biochemical and radiographic data, may encounter difficulties. A good tissue sample is the basis for microscopic investigation. However, bone tumors often show an extreme variety of structures which confuse even experienced bone pathologists. Therefore, histo pathologic analysis must take into account all available clinical, biochemical and radiographic data. The close cooperation of the pathologist with clinicians and, especially, radiologists is of the utmost importance.

Book excerpt: Bone Cancer, Second Edition comprehensively investigates key discoveries in the field of bone biology over the last five years that have led to the development of entirely new areas for investigation, such as therapies which combine surgery and biological approaches. The Second Edition expands on the original overview of bone cancer development (physiology and pathophysiology), with key chapters from the first edition, and offers numerous new chapters describing the new concepts of bone cancer biology and therapy, for both primary bone tumors as well as bone metastases. Each chapter has been written by internationally recognized specialists on the bone cancer microenvironment, bone metastases, osteoclast biology in bone cancer, proteomics, bone niche, circulating tumor cells, and clinical trials. Given the global prevalence of breast and prostate cancers, knowledge of bone biology has become essential for everyone within the medical and cancer research communities. Bone Cancer continues to offer the only translational reference to cover all aspects of primary bone cancer and bone metastases – from bench to bedside: development (cellular and molecular mechanisms), genomic and proteomic analyses, clinical analyses (histopathology, imaging, pain monitoring), as well as new therapeutic approaches and clinical trials for primary bone tumors and bone metastases. - Presents a comprehensive, translational source for all aspects of primary bone cancer and bone metastases in one reference work - Provides a common language for cancer researchers, bone biologists, oncologists, and radiologists to discuss bone tumors and how bone cancer metastases affects each major organ system - Offers insights to research clinicians (oncologists and radiologists) into understanding the molecular basis of bone cancer, leading to more well-informed diagnoses and treatment of tumors and metastases - Offers insights to bone biologists into how clinical observations and practices can feed back into the research cycle and, therefore, can contribute to the development of more targeted genomic and proteomic assays

With a thorough update, new chapters, a clear presentation of tips and tricks to address complications, as well as supplementary online videos and tutorials, this book addresses the challenges of limb preservation that are posed to the surgeon in this new era of cancer treatment characterized by higher survival rates and longer survival times. The authors are experienced practitioners of Cañadell's technique, but the book also explores, thanks to the focused contributions of experts from other fields of medicine, a range of topics from the molecular biology and histological aspects of bone tumors, to the use of imaging techniques and surgical options for epiphyseal preservation. Abundant illustrations and accounts of clinical results make this book an indispensable guide for orthopedic surgeons and pediatric oncologists and give the book an interdisciplinary appeal to practitioners in more general orthopedics, oncology, pediatrics and surgery.

Book excerpt: The field of pediatric oncology encompasses four groups of malignancies - acute leukemias, brain tumors, lymphomas and solid tumors. 1'he history, diagnosis and management of children with acute leukemias and lymphomas has been thoroughly examined in several excellent textbooks of pediatric hematology and oncology. Bl"ain tumors have historically been managed by neurosurgeons and radiation therapists. 1'he role of the pediatric oncologist in the management of these patients is evolving. This book was written to provide a thorough historical evaluation of the most frequent solid tumors of children. A detailed examination of the natural history of these tumors is essential to the design and evaluation of therapeutic trials. The highly lethal nature of many of these tumors, the occurrence of some of them at several different primary sites and the rarity of these tumors have made systematic study of them difficult. Conclusions regarding the efficacy of a particular modification of the therapeutic strategy can be strongly influenced by the assumed natural history of the tumor. I have tried to develop as accurateJy as the literature would allow a picture of the natural history of the common malignant solid tumors, knowing that the image would be imperfect. I adopted a convention which was employed in all graphs constructeil from case reports summarized from the literature.

Book excerpt: Detection and characterization of bone tumors with imaging remains a big challenge for every radiologist notwithstanding the impressive progress achieved by the introduction of several new imaging modalities. Moreover, new concepts in surgical and oncological treatment of these lesions require from the radiologist appropriate and focused answers to the specifc questions asked by the referring physicians in order to choose the best therapeutic approach for the in- vidual patient. Tis comprehensive textbook describes in detail the possibilities and limits of all moda- ties, including MRI, CT, nuclear medicine and interventional radiological procedures, employed for the modern imaging of tumoral and tumor-like lesions of bone. Teir role in the diagnosis, surgical staging, biopsy and assessment of response to therapy is discussed in detail, covering all tumor subtypes as well as their specifc anatomical location.Well selected and technically imp- cable illustrations strongly enhance the didactic value of this work. I am very much indebted and grateful to the three editors: A. Mark Davies, Murali Sundaram and Steven L. J. James, world authorities in musculoskeletal radiology, for their superb scientifc achievement in preparing and editing this wonderful volume as well as for their individual ch- ters. I would also like to thank the large international group of collaborating authors, who are also widely acknowledged for their specifc expertise in the area of bone tumors, for their outstanding contributions.

In contrast, secondary bone cancer is where the cancer started in another part of the body but has then spread to the bones. The most common types of primary bone tumor are osteosarcoma and Ewing's sarcoma, both of which are most frequently diagnosed in children and young adults. Other less common types of bone cancer include: Chondrosarcoma (a cancer arising in cartilage cells, usually found in adults between ages 50-75, though the less common mesenchymal-chondrosarcoma is more frequent in younger patients), Malignant Fibrous Histiocytoma of bone (MFH), Chondoma (a rare low grade malignancy occurring mostly between ages 30 -70), and other rare tumours. This new book brings together leading research from throughout the world in this important area of research.
