As South Asians comprise about 25% of the world's population their high susceptibility is of global public health and clinical importance. Eluding researchers across the globe, this phenomenon continues to be a subject of intensive enquiry. As Ban Ki-moon, the United Nations Secretary-General, points out, the epidemics of chronic diseases, which he describes as a public health emergency in slow motion, can be restrained but not stopped. With a focus on the global South Asian population, Epidemic of Cardiovascular Disease and Diabetes: Explaining the Phenomenon in South Asians Worldwide is a critical review of current literature investigating the increase in cases of CVD and DM2, and the data underpinning them. The book argues that the high risk of CVD and DM2 in urbanised South Asians is not inevitable, genetic, or programmed in a fixed way. Rather, exposure to risk factors in childhood, adolescence, and most particularly in adulthood, is the key to unravelling its cause. There are many potentially modifiable and non-modifiable risk factors associated with the same. Although with the recent advances in management and preventive strategies the mortality rates have reduced, but no patient actually achieves an adequate control of the CVD risk factors with the declining quality of life. In addition, growing obesity and DM in younger age groups has further undermined the improvements achieved in CVD. Diabetes and CVD share a "common soil" in their etiology and the causative factors for these diseases are termed as "cardiometabolic risk factors." Cardiometabolic risk (CMR) is the global risk of developing type 2 diabetes and CVD. CMR factors include overweight or obesity, high blood glucose, HTN, dyslipidemia, inflammation and hypercoagulation, physical inactivity, smoking, age, race and ethnicity, gender, and family history. Among these, age, race/ethnicity, gender, and family history are non-modifiable risk factors. The remaining are modifiable risk factors and closely interrelated. Recently, systematic prospective studies have shown evidences that moderate lifestyle modifications help in reducing the metabolic risk factors. The major principles include cessation of smoking, enhanced physical activity, and reduction of excess weight. Healthy diet also has a major role in controlling overweight and maintaining ideal weight. Each of the risk factors poses a danger to good health; the propensity increases with multiple risk factors. It is also shown that the CMR factors tend to cluster as the metabolic syndrome. This book is designed to address such questions with supportive typical clinical scenarios, with which all readers will be able to identify. Thus it provides an excellent opportunity to widen one’s perspective in this area.

Book excerpt: BACKGROUND: Impaired glucose regulation (IGR) is associated with detrimental cardiovascular outcomes such as cardiovascular disease risk factors (CVD risk factors) or intima-media thickness (IMT). Our aim was to examine whether these associations are mediated by body mass index (BMI), waist circumference (waist) or fasting serum insulin (insulin) in a population in the African region. METHODS: Major CVD risk factors (systolic blood pressure, smoking, LDL-cholesterol, HDL-cholesterol) were measured in a random sample of adults aged 25-64 in the Seychelles (n=1255, participation rate: 80.2%). According to the criteria of the American Diabetes Association, IGR was divided in four ordered categories: 1) normal fasting glucose (NFG), 2) impaired fasting glucose (IFG) and normal glucose tolerance (IFG/NGT), 3) IFG and impaired glucose tolerance (IFG/IGT), and 4) diabetes mellitus (DM).
