Son utilisation s’applique à la fois au cabinet dentaire et au laboratoire de prothèse. Parallèlement, le désir de pratiquer une dentisterie moderne et l’exigence esthétique croissante ds patients ont trouvé leur écho dans le développement du tout céramique. Aujourd’hui la CFAO permet justement de simplifier la mise en oeuvre et la fabrication du « tout céramique » au cabinet et au laboratoire. Cependant, si les qualités biologiques et mécaniques intrinsèques aux nouvelles céramiques sont de plus en plus étudiées et reconnues, la précision d’adaptation et la durée de vie des couronnes obtenues par cette modalité de conception et fabrication restent une préoccupation de tous les cliniciens. On peut se demander si les couronnes réalisées par CFAO sont tout aussi bien adaptées que celles obtenues via les méthodes traditionnelles. Dans la chaine de réalisation, les risques d’erreur ne se situent plus aux mêmes étapes, à la fois avant et pendant la réalisation prothétique. L’objet de cette thèse est d’établir si la précision d’adaptation d’une couronne tout céramique réalisée par CFAO est équivalente à celle d’une couronne réalisée par des techniques de conception et de fabrication traditionnelles.

Book excerpt: Les technologies de fabrication additive sont devenues une partie intégrante des applications possibles notamment pour les polymères, le métal ou la cire. Les restaurations prothétiques doivent répondre à un cahier des charges précis pour être cliniquement acceptable en chirurgie dentaire notamment concernant la précision d'adaptation au pilier support et les propriétés mécaniques. Les applications possibles pour les céramiques mises en forme par des techniques additives sont encore en développement. La stéréolithographie permet la mise en forme d'objet en céramique, par photopolymérisation de couche successive d'une barbotine. Les particules de céramiques sont associées à un monomère photopolymérisable afin d'être mises en forme. Pour acquérir ses caractéristiques finales, la pièce doit subir des traitements thermiques : le déliantage et le frittage. La pièce est d'abord débarrassée de sa trame organique puis elle est densifiée pour acquérir ses propriétés physiques et mécaniques. C'est durant cette dernière étape qu'un retrait en volume s'opère. Ce retrait est anisotrope, cela signifie que les taux de retrait dans les 3 sens de l'espaces sont différents et non proportionnels. Les taux de retrait, correspondant aux variations dimensionnelles entre la pièce crue et la pièce densifiée, doivent être anticiper afin de surdimensionner avec les bons facteurs le fichier STL servant à la mise en place de la pièce par stéréolithographie. Ce phénomène complique l'adaptation qui se veut très précise des restaurations prothétiques dentaires. Ce travail de revue de littérature s'intéresse aux taux de retrait constatés et aux précisions d'adaptations obtenues des céramiques mises en forme par stéréolithographie ainsi qu'aux différents protocoles et méthodes permettant d'évaluer ces paramètres.

L’objectif de ce travail a été d’étudier, sur la base de la littérature scientifique, l’ajustage marginal des couronnes céramo-céramiques issues de différents systèmes et d’identifier les facteurs pouvant l’influencer. Une recherche électronique à partir de PubMed a été réalisée en utilisant les combinaisons de mots (discrepancy OR fit OR gaps OR adaptation) AND disilicate OR ceramic) AND (coping OR crowns). La recherche était limitée aux articles en français et en anglais publiés et référencés avant le 1er mars 2011. Les références bibliographiques des articles sélectionnés ont été passées en revue pour potentiellement inclure des articles ne figurant pas dans les résultats de la recherche électronique. Un total de 424 articles ont été initialement regroupés et seulement 45furent finalement retenus à partir de critères d’inclusion et d’exclusion spécifiques. 39 de ces articles étaient des études in vitro et 6 d’entre eux des études in vivo. Les articles sélectionnés évaluaient le joint marginal et s’intéressaient à l’influence que pouvait avoir certains facteurs sur la valeur : le type de dent préparée, les différentes faces de la dent, le type de ligne de finition, l’espacement interne choisi, l’angle de préparation, le montage de la céramique cosmétique sur la chape et le scellement/collage de la couronne. L’ensemble des différents systèmes tout-céramique analysés au sein des articles a montré une adaptation marginale appartenant au domaine d’acceptabilité clinique (≤120μm). Il n’a pas été possible d’établir un réel classement entre les procédés ni de méta-analyse en raison de l’hétérogénéité des méthodes d’expérimentation. Seuls, l’espacement interne, l’angle de préparation, le montage de la céramique cosmétique et le scellement/collage, peuvent avoir une influence sur la valeur du joint marginal.

L’utilisation de la zircone en Prothèse semble pallier à ces défauts. Son association avec les techniques de CFAO (Conception et Fabrication Assistées par Ordinateur) paraît également séduisante en améliorant la précision de réalisation des prothèses fixées. Les propriétés physiques et la biocompatibilité de la zircone laissent présager une parfaite intégration et longévité. L’ association de ce matériau à la CFAO permet l’obtention d’une précision d’adaptation compatible avec les recommandations usuelles dentaires, l’outil informatique permettant de corriger au mieux les éventuelles erreurs du praticien. Même si les étapes sont longues et fastidieuses, la gestion informatisée du protocole permet une optimisation du travail. L’attrait pour cette association est manifeste. Cependant il reste actuellement le double problème de l’absence de normes européennes sur les produits employés et du coût de réalisation au laboratoire. La dernière partie de cet exposé présente les intérêts et les limites de l’association de la zircone et des systèmes de CFAO, ainsi que les perspectives d’une diffusion de cette association auprès des praticiens

Paul Getty, who started collecting in this area in 1938 and continued until his death in 1976. The Museum’s collection is particularly rich in examples created by the most talented cabinet masters then active in Paris, including Bernard van Risenburgh II (after 1696–ca. 1766), Jacques Dubois (1694–1763), and Jean-François Oeben (1721–1763). Working for members of the French royal family and aristocracy, these craftsmen excelled at producing veneered and marquetried pieces of furniture (tables, cabinets, and chests of drawers) fashionable for their lavish surfaces, refined gilt-bronze mounts, and elaborate design. These objects were renowned throughout Europe at a time when Paris was considered the capital of good taste.
