Texte et images se retrouvent dans l'édition numérique associée (avec une quarantaine de vidéos) permettant à la fois une navigation instantanée dans l'ensemble de l'ouvrage et une consultation ubiquitaire. Le plan reste classique : les aspects techniques et réglementaires de l'échographie pendant la grossesse, l'utérus gravide et les ovaires, le début de la grossesse (développement embryonnaire, datation, pathologie et GEU), le placenta et les annexes du foetus, l'examen du foetus normal, la surveillance de la croissance et de la vitalité foetale, la pathologie malformative (huit chapitres), les anomalies chromosomiques, les infections foetales et les grossesses multiples. Certains points ont fait l'objet de développements plus importants en raison de leur position critique dans la pratique actuelle : les aspects réglementaires de l'examen et de son compte rendu, les recommandations et les bonnes pratiques (françaises mais aussi étrangères), l'usage du Doppler et l'étude du col utérin, et surtout le dépistage des anomalies chromosomiques (trisomie 21 en particulier) qui, à tort ou à raison, focalise une grande partie du diagnostic anténatal et donc de l'échographie. L'ambition et l'esprit de ce manuel restent les mêmes qu'il y a vingt-cinq ans : un bon manuel de radiologie pour les accoucheurs (-euses) et un bon livre d'obstétrique pour les radiologues. C'est le résultat d'une confrontation amicale, synthèse et compromis, entre les deux spécialités, mais aussi entre le praticien de terrain (l'urgence clinique, le travail de nuit, la salle d'attente pleine ...) et le théoricien rigoureux, les deux personnages étant souvent confondus au sein de chaque auteur.

Corréler directement la réalisation de l'échographie et ses résultats à la morbi-mortalité périnatale ou maternelle est irréaliste mais qui nierait qu'il est utile de connaître l'éventualité d'un placenta praevia, voire accreta, d'un retard de croissance ou d'une macrosomie à terme : sans doute aucun lecteur raisonnablement instruit des enjeux de la périnatalité aujourd'hui. Cependant ces suspicions ou " diagnostics " portés à 32 semaines peuvent entraîner des attitudes cliniques inconséquentes et parfois iatrogènes et le chemin qui inexorablement mènera de l'échographie obstétricale à l'obstétrique échographique passe par une meilleure compréhension de l'échographie du premier, deuxième et troisième trimestre dans sa réalisation mais aussi dans l'utilisation optimale de ses résultats. Il paraît alors difficile de scinder les fonctions d'échographistes et d'obstétriciens afin que les premiers soient informatifs et raisonnables dans leurs conclusions et que les seconds puissent choisir le bon chemin entre scepticisme ignorant et crédulité iatrogène. The main risk factor for abnormal invasive placentation is the repeated cesarean, although other factors were identified. There are specific techniques to provide a high confidence diagnosis. However, precise skills must be acquired to recognize detailed diagnostic signs, to avoid common technical mistakes, and also to know when, how and why it is necessary to use each of them. Presurgical study provides diagnosis, extension and compromise of neighboring structures such as the bladder or the parametrium. Knowledge of placental invasion extension is needed to plan any resective surgery such as hysterectomy or one-step conservative surgery. Due to the fact that topography of the invaded area has direct relation with the specific arterial pedicles, a map of the invasion is required to know which type of proximal vascular control can be more effective. Leaving the placenta in situ seems to be the best option when resources or a skilled team are not available, but it requires intensive postoperative controls to detect infection, bleeding or coagulation disorders. Hysterectomy can be an easy solution for non-experimented operators; however, it is usually a very complicated procedure with demonstrated morbidity and mortality due to hemodynamic and hemostatic problems. This book gathers the latest knowledge in relation with the etiology, diagnosis, treatment and also the authors personal experience in more than 500 cases. All aspects of this condition have been analyzed to provide an accurate management, which includes vascular control, urology, anesthesia and hemodynamic management among others.
